Thyroid gland flap minimizes mucosal defects at supracricoid partial laryngectomy with cricohyoidoepiglottopexy.

Wound infection is a major complication after supracricoid partial laryngectomy with cricohyoidoepiglottopexy (CHEP) for radiation therapy failure. A 60-year-old man received chemoradiotherapy for a glottic carcinoma. CHEP, reusing the thyroid gland flap (TF), was performed because the cancer recurred after a salvage vertical partial laryngectomy following radiation therapy failure. The TF was sutured to the supraglottis and cricoid cartilage mucosa to minimize mucosal defects before the hyoid bone and cricoid cartilage were sutured. Wound healing after CHEP was good without infection. After d…

Laryngeal anatomical variants and their impact on the diagnosis of mechanical asphyxias by neck pressure.

The aims of this investigation were to determine the characteristics and prevalence of anatomical variants of the larynx apparatus and to ev … if the thyroid cartilage was affected, with a higher proportion of false positives comparing to the hyoid bone (p<0.001). The higher frequency of thyroid fractures in neck pressure together with the prevalence and location of triticeal cartilage on the lower third of the lateral thyrohyoid ligament are the main reasons for these results. Further studies shou …

Severe laryngeal injuries masked by fractured mandible.

UNLABELLED: A 17-year-old male was referred to the maxillofacial department regarding a fractured mandible. Airway compromise was suspected as a result of neck swelling, stridor, haemoptysis and surgical emphysema. Severe laryngeal injuries were diagnosed, including a fractured hyoid. His treatment and outcomes are summarized, with discussion of aetiology, symptoms, diagnosis and treatment of laryngeal injuries, including urgent airway management, if necessary. The risks and complications of delayed diagnosis and treatment are discussed, particularly if symptoms are delayed or the injury is ma

Chemoradiation to the submental muscles alters hyoid movement during swallowing in a rat model.

Hyolaryngeal dysfunction is a commonly reported swallowing problem after chemoradiation treatment for head and neck cancer. The displacement of the hyolaryngeal complex during swallowing protects the airway and assists in opening the upper esophageal sphincter. Activation of the submental muscles, specifically the mylohyoid and geniohyoid muscles, is thought to facilitate movement of the hyoid. The purpose of this study was to determine if targeted radiation to the submental muscles given concurrently with chemotherapy alters hyolaryngeal displacement 1 mo after treatment. We hypothesized that

Latent changes in the pharyngeal stage of swallowing in non-aspirating older adults.

PURPOSE: The characteristic changes in the swallowing mechanism with aging are collectively termed presbyphagia. Although several studies ha … eal wall contact, duration and dimension of upper esophageal sphincter opening (UES-O), and maximal hyoid bone displacement between the age groups. RESULTS: Among the older groups, the oldest-old showed significantly longer PDT than younger-old adults, and the UES-O tended to be wider in the former. However, no other remarkable differences were found betwee …

Temporal characteristics of laryngeal penetration and aspiration in stroke patients.

BACKGROUND: Despite the importance of understanding penetration-aspiration (PA) in patients with stroke, the pathophysiology of PA remains unclear. OBJECTIVES: This study aimed to investigate the temporal characteristics of PA in post-stroke patients in terms of the timing of the PA event and hyolaryngeal incoordination. METHODS: Fifty-eight swallows (38 stroke patients), showing PA when swallowing a thin liquid, were included. The timing of PA was classified kinematically as before or during the swallow. The movement sequence of vertical laryngeal elevation, horizontal hyoid excursion, and

Hyo-neck lift evolution: Neck lift with fixation of the platysma to the deep cervical fascia.

UNLABELLED: The aim of any neck lift obviously includes the restoration of an acute cervicomandibular angle. The hyo neck lift, first published in April 2016, did proposed a new technique of neck lift with a sub-cutaneous neck dissection and suture of the platysma to the hyoid through this sub-cutaneous approach. To enhance results and efficiency, a major change of the hyo neck lift technique is herein proposed with a vertical anterior sub-platysmal and sub-platysmal fat opening and dissection. This anterior dissection ensures a precise and efficient fixation of the platysma to the deep cervic

New insight into the anatomy of the hyolingual apparatus of Alligator mississippiensis and implications for reconstructing feeding in extinct archosaurs.

Anatomical studies of the cranium of crocodilians motivated by an interest in its function in feeding largely focused on bite force, the jaw apparatus and associated muscles innervated by the trigeminal nerve. However, the ossified and cartilaginous elements of the hyoid and the associated hyolingual muscles, innervated by the facial, hypoglossal and glossopharyngeal nerves, received much less attention. Crocodilians are known to retain what are ancestrally the 'Rhythmic Hyobranchial Behaviors' such as buccal oscillation, but show diminished freedom and movement for the hyobranchial apparatus

Effects of maxillomandibular advancement on the upper airway and surrounding structures in patients with obstructive sleep apnoea: a systematic review.

Obstructive sleep apnoea is ideally treated by continuous positive airway pressure, but other options are needed because its clinical effectiveness is limited by poor acceptance and tolerance, which results in suboptimal compliance. Patients often prefer operation, with maxillomandibular advancement (MMA) being the most effective approach. In this systematic review we have assessed its effects on the upper airway and surrounding structures in patients with obstructive sleep apnoea. After a structured search of electronic databases and hand searching, we retrieved 104 publications. After applic

[Articulatory compensation after supracricoid partial laryngectomy with cricohyoidoepiglottopexy].

OBJECTIVES: The consequences of the modification of the glottis and the shortening of the vocal tract after supracricoid partial laryngectomy (SCPL) with cricohyoidoepiglottopexy (CHEP) were investigated prospectively on ten patients. An acoustic analysis of the transfer function of the vocal tract was performed by measuring the formant frequencies of the [a] and [i] vowels. The articulation compensatory mechanisms of the vocal tract were observed with cinefluoroscopy in order to evaluate the phonation and articulation constraints. PATIENTS AND METHODS: Ten male patients were recorded before

Hyoepiglottic ligament in supraglottic cancer.

The hyoepiglottic ligament (HL) is a connective tissue structure that serves as the roof of both the paraglottic and the preepiglottic spaces and thereby anatomically separates the supraglottic larynx from the tongue base. Whole mount serially sectioned larynges with supraglottic cancer were reviewed to help clarify cephalad spread of cancer in this region. The whole mount slides were analyzed from 70 laryngectomy specimens that were resected for supraglottic cancer. The HL was breached by cancer in 13 specimens, and all of these displayed clinical and histopathologic invasion of the preepiglo

Changes in tongue morphology predict responses in pharyngeal patency to selective hypoglossal nerve stimulation.

STUDY OBJECTIVES: The major goal of the study was to determine whether changes in tongue morphology under selective hypoglossal nerve therapy for obstructive sleep apnea were associated with alterations in airway patency during sleep when specific portions of the hypoglossal nerve were stimulated. METHODS: This case series was conducted at the Johns Hopkins Sleep Disorders Center at Johns Hopkins Bayview Medical Center. Twelve patients with apnea implanted with a multichannel targeted hypoglossal nerve-stimulating system underwent midsagittal ultrasound tongue imaging during wakefulness. Chan

Simplified four-step retropharyngeal approach for the upper cervical spine: technical note.

PURPOSE: In this paper, we propose a simplified four-step retropharyngeal approach, whose aim is getting straight to the upper cervical spine minimizing complications. METHODS: While the classical retropharyngeal approach includes about 11 steps, ours is a four-step approach: patient positioning, skin-platysma incision, hyoid bone superolateral dissection and retropharyngeal blunt exposure. We avoid several steps of the classical anterior retropharyngeal approach, particularly dissection of submandibular gland, facial veins, external carotid artery and thyroid artery, bellies of the digastric…
