A biochemical, histologic and morphometric evaluation of spontaneous, diet-induced (thoracic aorta) and injury-induced (iliac-femoral) atherosclerotic lesions was performed in rabbits maintained on varying levels of dietary cholesterol. Rabbits were meal-fed a 3% peanut oil, 3% coconut oil diet containing 0%, 0.1%, 0.25%, 0.5%, 1.0%, 1.5% or 2.0% cholesterol for 9 weeks. Plasma total cholesterol exposure (area under cholesterol-time curve (TC-AUC)) increased diet-dependently over the course of the study. VLDL and LDL cholesterol (VLDL-C, LDL-C) comprised 41% and 55%, respectively, of the plasm

This report presents the second portion of the morphologic studies on chronic, diet-induced hypercholesterolemia in nonhuman primates (Macaca nemestrina) examined sequentially between 5 and 13 months. A direct relationship was observed between the rate of cholesterol increase, the level and duration of hypercholesterolemia, and the changes in the artery wall that led to the formation of fatty streaks and their conversion to fibrous plaques. A loss of endothelial continuity was first observed in the iliac arteries between 3 and 4 months of atherogenic diet and appears to be a critical step in t…

Cholesterol balance studies were carried out twice in a young male patient with homozygous familial hypercholesterolemia. At 13 mo, cholesterol balance in this patient averaged 31.3 mg/kg per d, and bile acid excretion was 12.0 mg/kg per d; at 3 yr, results were similar, 27.3 and 15.5 mg/kg per d for cholesterol balance and bile acids, respectively. A normal boy of 3 yr was also studied for comparison with the second study in our patient. Cholesterol balance and bile acid outputs in the normal child were 11.5 and 3.3 mg/kg per d, respectively. Thus, in comparison with the normal child, the pat…

Dyslipidemia, characterized by an excessive amount of lipids in the bloodstream, is a significant risk factor for metabolic disorders and cardiovascular diseases (CVDs). Maternal supraphysiological hypercholesterolemia (MSPH) is associated with increased maternal levels of total cholesterol (TC) and low-density lipoprotein (LDL). This condition has been linked to negative consequences on the fetoplacental vasculature, including increased atherosclerosis development in the fetal aorta and later in children and adolescents. This study aims to determine whether the high cholesterol levels associa…

Excessive hypercholesterolemia in pregnancy is a risk factor for the development of preeclampsia, but its ensuing impact on offspring cardiovascular health remains not fully understood. In the present study, Sprague Dawley rats were fed a control or high cholesterol diet (HCD) from gestational days 6-20 (term=22 days). Female and male offspring were aged to adulthood (4 months old), and in vivo and ex vivo cardiovascular function were assessed. In female offspring, body weight gain was greater in the HCD group compared with controls after 2 months of age. Blood pressure, echocardiographic para…

BACKGROUND: Heterozygous familial hypercholesterolemia (HeFH) is a common autosomal dominant disorder, and adults with HeFH can have other cardiometabolic risk factors that increase the risk of events. Recent studies have reported relatively low rates of these risk factors among children with HeFH.
OBJECTIVE: Compare cardiometabolic risk factors among children with and without HeFH presenting to a pediatric lipid clinic.
METHODS: We conducted a retrospective chart review study of subjects who presented to the pediatric lipid clinic in a single institution between January 1, 2011, and October…

BACKGROUND: Familial hypercholesterolemia (FH) is a monogenic disorder associated with premature coronary artery disease. The Dutch Lipid Clinic Network (DLCN) and Familial Hypercholesterolemia Case Ascertainment Tool (FAMCAT) are tools used to identify individuals with possible FH. Although FAMCAT performs well in primary care, its accuracy for genetically confirmed FH in tertiary care settings remains unclear. This study assessed the performance of FAMCAT in an Australian tertiary lipid clinic.
OBJECTIVE: This study assessed the performance of FAMCAT in identifying patients with genetically…

Familial hypercholesterolemia (FH) constitutes the most common inherited lipid disorder caused by mutations in any of the genes involved in the metabolism of low-density lipoprotein (LDL), including the LDL receptor (LDLR), Apolipoprotein B (APOB), or Proprotein Convertase Subtilisin/Kexin Type 9 (PCSK9). FH causes increased LDL-cholesterol levels, leading to an increased risk for premature atherosclerotic cardiovascular disease. This comprehensive review aims to discuss the progress of FH management, from classic statin therapy to relatively new therapies such as PCSK9 inhibitors and emerging…

Homozygous familial hypercholesterolemia (HoFH) is a rare inherited metabolic disorder. Meanwhile, HoFH is characterized by extremely high plasma levels of low-density lipoprotein cholesterol (LDL-C) from birth, alongside xanthomas and premature atherosclerotic cardiovascular diseases (ASCVDs). Traditional drugs such as statins have difficulty maintaining serum lipids at an ideal level. Here, we report the case of a 12-year-old child with HoFH who underwent liver transplantation. The goal of lipid reduction could not be achieved in this patient by any other means, and the patient had also expe…

OBJECTIVE: Familial hypercholesterolemia (FH) is a genetic disorder characterized by elevated plasma cholesterol and an increased risk of early-onset coronary artery disease (CAD). FH has a complex genetic basis. Advances in molecular genetic techniques have deepened our understanding of FH, which is critical for accurate classification, CAD risk assessment, and optimized treatment strategies. This study aimed to evaluate the monogenic etiologies of FH and polygenic background as a possible cause of clinical FH phenotype and their relationship to clinical outcomes in a Turkish cohort.

Homozygous familial hypercholesterolaemia (HoFH) is a severe form of FH in which inheritance of two defective or null mutations in genes associated with metabolism of low-density lipoprotein cholesterol (LDL-C) results in extremely high LDL-C, premature atherosclerotic cardiovascular disease (ASCVD) and mortality. Treatment of HoFH comprises a multi-modal approach of statins, ezetimibe, lipoprotein apheresis; and inhibitors of proprotein convertase subtilisin/kexin type, angiopoietin-like protein 3 (ANGPTL3) and microsomal triglyceride transfer protein. These treatments are generally costly, a

BACKGROUND: Patients with chronic kidney disease (CKD) are more prone to cardiovascular disease (CVD) risks, including CVD-derived sudden death. Additionally, patients with CKD also develop lipid metabolism abnormalities. Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors (PCSK9i) are drugs capable of reducing CVD risk in patients with CKD, but their efficacy is scarcely assessed in transplant patients.
CASE PRESENTATION: Here, we report a case of a 74-year-old man undergoing nephrology follow-up for a cadaver donor kidney transplant. The patient described an atorvastatin allerg
