Effect of hyperthermic intraperitoneal chemotherapy in combination with cytoreductive surgery on the prognosis of patients with colorectal cancer peritoneal metastasis: a systematic review and meta-analysis.

BACKGROUND: Peritoneal metastasis often occurs in patients with colorectal cancer peritoneal metastasis, and the prognosis is poor. A large body of evidence highlights the beneficial effects of cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) on survival, but to date, there is little consensus on the optimal treatment strategy for patients with colorectal cancer peritoneal metastasis. The purpose of this study is to evaluate the impact of CRS + HIPEC on survival and provide reference for the treatment of patients with colorectal cancer peritoneal metastasis. M…

Drainage of a Subphrenic Abscess Followed by Two-Stage Gastrectomy and Adjuvant Hyperthermic Intraperitoneal Chemotherapy (HIPEC) for Perforated Gastric Carcinoma: A Case Report.

BACKGROUND Perforated gastric cancer accounts for less than 1% of patients who present with an acute abdomen and for up to 16% of all gastric perforations. A two-stage laparoscopic procedure may be the therapeutic strategy of choice in selected patients, and adjuvant hyperthermic intraperitoneal chemotherapy (HIPEC) can reduce the incidence of peritoneal recurrence. A rare case of subphrenic abscess and gastric perforation due to carcinoma of the gastric fundus, followed by two-stage gastrectomy and adjuvant HIPEC is presented. CASE REPORT A 65-year old man presented with a left subphrenic abs…

Significance of diaphragmatic resections and thoracic chemoperfusion on outcomes of peritoneal surface disease treated with cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC).

BACKGROUND: Diaphragmatic resection (DR) during CRS/HIPEC exposes the thoracic cavity to direct contamination from the peritoneal cavity. The effect of thoracic chemoperfusion in combination with HIPEC in these patients is unknown. METHODS: A prospective database of 1,077 procedures was analyzed. Type of malignancy, thoracic perfusion, resection status, comorbidities, morbidity, mortality, and overall survival were reviewed. RESULTS: DR was a component of 102 CRS/HIPEC procedures performed for 57 (55.9 %) appendiceal and 22 (21.6 %) colon primary lesions. DR was associated with higher volume

Morbidity and Mortality of Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy in Patients Aged 70 Years and Older: A Two-Center 10-Year Experience.

BACKGROUND AND OBJECTIVES: Cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) is established for peritoneal surface malignancies, but its use in older patients is debated. We assessed CRS-HIPEC morbidity and mortality in patients aged ≥ 70 years using a cohort of younger patients as a comparator. METHODS: We retrospectively reviewed consecutive patients aged ≥ 70 and < 70 undergoing CRS-HIPEC for peritoneal carcinomatosis at two centers (2017-2026). Peritoneal carcinomatosis index (PCI), completeness of cytoreduction (CC), complications (Clavien-Dindo), and mor…

Cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy and intraoperative radiation therapy in the management of gastric cancer: a 10-year single center experience.

BACKGROUND: The rationale behind the use of HIPEC involves targeted elimination of microscopic peritoneal metastasis, a common route for GCa dissemination, thereby improving the overall survival and reducing recurrences. Moreover, the reasoning behind the use of IORT is enhanced loco-regional control and, therefore, reducing recurrence rates. METHODS: From February 2013 to June 2023, all GCa patients who underwent HIPEC plus IORT during surgery were included in this study. Median overall survival (OS) and disease-free (DFS) survival were used to evaluate the efficacy of this treatment strateg

The Effect of Prior Nondefinitive Surgery on Perioperative Outcomes and Survival in Mucinous Appendix Cancer Patients Undergoing Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy.

Introduction: Peritoneal metastases (PM) from mucinous appendix cancer (MAC) are common and often mismanaged. While cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) is standard, many patients undergo nondefinitive surgery. We compared outcomes in MAC patients undergoing CRS/HIPEC after limited vs excessive prior surgeries. Methods: We conducted a retrospective analysis of MAC patients (1998-2023) from a single-center prospective database who underwent CRS/HIPEC. Surgical history was evaluated using the prior surgical score (PSS). Perioperative characteristics wer…

KELIM score and BRCA status are unreliable to predict the completeness of cytoreduction during interval debulking surgery with hyperthermic intraperitoneal chemotherapy for ovarian cancers.

OBJECTIVE: The gold standard for treating primary advanced ovarian, primary peritoneal, and fallopian tubal cancers (OC) is optimal debulking surgery plus adjuvant chemotherapy. Neoadjuvant chemotherapy followed by optimal interval debulking surgery (NACT/IDS) with hyperthermic intraperitoneal chemotherapy (HIPEC) provides better survival outcomes than without HIPEC in current literature. The modeled CA-125 elimination rate constant k (KLEM) score reflects the response of NACT before IDS. A BRCA mutation may indicate a better response to chemotherapy and improved outcomes in OC. However, the c…

Society of Onco-Anaesthesia and Perioperative Care consensus guidelines for perioperative management of patients for cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS-HIPEC).

Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS-HIPEC) for primary peritoneal malignancies or peritoneal spread of malignant neoplasm is being done at many centres worldwide. Perioperative management is challenging with varied haemodynamic and temperature instabilities, and the literature is scarce in many aspects of its perioperative management. There is a need to have coalition of the existing evidence and experts' consensus opinion for better perioperative management. The purpose of this consensus practice guideline is to provide consensus for best practice pattern …
