Trajectories of antidepressant dispensing among privately insured transgender people in the United States.

Psychiatric medication utilization studies rarely identify transgender and gender diverse (TGD) people, masking TGD health inequities. This study assessed antidepressant prescription fill patterns and their predictors among a privately insured TGD sample with newly dispensed anti-depressants after a recent depression or anxiety diagnosis from Merative MarketScan commercial insurance claims from 2007 to 2021. We measured trajectories of antidepressants utilization over 6-months using group-based trajectory modeling (GBTM), then identified demographic and clinical predictors of medication utiliz…

Willingness to pay for health insurance in Italy: evidence from a survey on consumers' behaviour.

This paper examines the willingness to pay (WTP) for duplicative private health insurance in Italy, a country with universal healthcare system characterized by broad public coverage alongside substantial waiting times and relatively high out-of-pocket expenditure. Using nationally representative data from the Italian Survey of Consumer Expectations (ISCE), we elicit individual WTP through a structured bidding mechanism for insurance offering faster access to surgeries, outpatient procedures, and diagnostic examinations already covered by the public system. Willingness to pay increases with inc…

Insurers' Acquisition Of Ambulatory Surgery Centers Was Not Associated With Changes In Performance, 2013-21.

Vertical integration between insurers and providers is increasingly common in US health care markets, yet there is little evidence on its impacts. This study examined the association between UnitedHealthcare's acquisition of ambulatory surgery centers (ASCs) and performance and pricing for arthroscopy and colonoscopy procedures. Using 100 percent Medicare fee-for-service claims data from the period 2013-21, we applied a difference-in-differences approach to assess changes in ASC size, procedure volume, and quality of care, as well as patient mix. We analyzed 2023 data from the Transparency in

Health Insurance and Trauma Outcomes: The Impact of Coverage on Survival and Recovery in Craniomaxillofacial Injuries.

Introduction Health disparities, particularly those related to insurance status, significantly impact trauma outcomes. Craniomaxillofacial (CMF) trauma patients without insurance often experience worse outcomes due to limited access to care and delayed treatment. This study examines the effects of insurance coverage on outcomes such as length of stay (LOS), survival, and surgical site infections (SSIs) among CMF trauma patients in Puerto Rico. Methods An institutional review board-approved retrospective analysis was conducted on 1,165 CMF trauma patients treated at the Puerto Rico Medical Cent…

State Prior Authorization Prohibitions and Buprenorphine Retention Among Privately Insured Patients.

IMPORTANCE: Buprenorphine is the most commonly prescribed medication for opioid use disorder, but rates of retention in treatment remain low. Prior authorization (PA) has been identified as a barrier to the administration of buprenorphine, and patients may need multiple PAs to continue treatment; thus, many states have passed laws to prohibit the use of PA for buprenorphine in private insurance. OBJECTIVE: To examine the association between PA prohibitions and buprenorphine treatment retention overall and by branded vs generic drug status. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectio…

MIST versus INSURE: Superior outcomes in surfactant administration for preterm neonates with RDS.

INTRODUCTION: Minimally invasive surfactant therapy (MIST) is increasingly adopted in neonatal care as an alternative to the INSURE (Intubation-Surfactant-Extubation) method for managing respiratory distress syndrome (RDS) in preterm infants. MIST reduces intubation-related complications and facilitates surfactant administration without invasive ventilation. This study aimed to compare the complications and outcomes of surfactant delivery via MIST versus INSURE in preterm infants with RDS. METHODS: In this retrospective study, we analyzed data from 513 preterm infants (gestational age: 28-34

Beyond insurance coverage: the impact of Medicaid expansion on colorectal cancer surgery in the Midwest.

PURPOSE: The Affordable Care Act's Medicaid expansion increased insurance coverage nationally; its impact on colorectal cancer surgery access and outcomes remains unclear, particularly in rural regions with structural barriers. This study evaluated whether Medicaid Expansion was associated with changes in colorectal cancer surgery utilization, access to elective surgery, and short-term outcomes within 8 Midwestern states. METHODS: A population-based difference-in-differences analysis used State Inpatient Databases from 2010 to 2022, including 4 expansion and 4 late or nonexpansion states. Adu…

Age and Endometrial Cancer Survival Disparities Among Black and White Insured Patients.

IMPORTANCE: Black patients with endometrial cancer have nearly twice the mortality of White patients with endometrial cancer, but it is unclear if the disparity differs by age, independent of insurance status. OBJECTIVE: To compare, in a large cohort of insured patients, the disparity between Black and White individuals in survival for patients aged younger than 65 years vs those age 65 years or older and to assess the association of age with excess mortality among Black patients. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study looked at comparative data from a community-b…

Population-Based vs Health System and Insurer Records: Significant Underdiagnosis of PCOS.

CONTEXT: Polycystic ovary syndrome (PCOS) is one of the most prevalent cardio-metabolic disorders in women. However, PCOS diagnosis is often missed or delayed, which may have serious physical and mental health consequences. The extent of underdiagnosis remains unknown. OBJECTIVE: This work aimed to estimate the extent to which PCOS is underdiagnosed. METHODS: We systematically searched PubMed and Embase through July 2023 for studies reporting PCOS prevalence in the general population and the Health System or Insurer Records (HSIR). RESULTS: We identified 6583 records, of which we included 7

An artificial insurance framework for a hydrogen-based microgrid to detect the advanced cyberattack model.

Microgrid systems have evolved based on renewable energies including wind, solar, and hydrogen to make the satisfaction of loads far from the main grid more flexible and controllable using both island- and grid-connected modes. Albeit microgrids can gain beneficial results in cost and energy schedules once operating in grid-connected mode, such systems are vulnerable to malicious attacks from the viewpoint of cybersecurity. With this in mind, this paper explores a novel advanced attack model named the false transferred data injection (FTDI) attack aiming to manipulatively alter the power flowi

The Influence of Structural Factors and Telemedicine on Missed Appointments Among Medicaid-Insured Patients.

BACKGROUND: Missed clinic appointments disproportionately affect Medicaid-insured patients and residents of socioeconomically deprived neighborhoods. The role of the recent telemedicine expansion in reducing these disparities is unclear. We analyzed the relationship between census tract (CT) poverty level, residential segregation, missed appointments, and the role of telemedicine. METHODS: This retrospective cohort study used electronic health records (EHR) data merged with population datasets and restricted to adult patients (≥ 18 years) with completed or missed internal or family medicine o

Nurse Practitioner Practice Regulations and Psychotropic Medication Dispensing for Medicaid-Insured Adults.

This study assessed the impact of Maryland's Nurse Practitioner Full Practice Authority Act of 2015 on psychotropic medication dispensing for Medicaid-insured adults treated by nurse practitioners (NPs) and physicians. A piecewise regression analysis of 168,509 Medicaid enrollees found that after this policy implementation, individuals were more likely to receive newly dispensed psychotropic medications from psychiatric-mental health NPs and from other NPs without a psychiatric specialty and were less likely to receive these medications from psychiatrists and from other medical doctors. Indivi

The Clinical Significance of the Manchester Colour Wheel in a Sample of People Treated for Insured Injuries.

Background/Objectives: The Manchester Colour Wheel (MCW) was developed as an alternative way of assessing health status, mood and treatment outcomes. There has been a dearth of research on this alternative assessment approach. The present study examines the sensitivity of the MCW to pain, psychological factors and recovery status in 1098 people with insured injuries treated in an interdisciplinary clinic. Methods: A deidentified data set of clients treated in a multidisciplinary clinic was conveyed to the researchers, containing results of MCW and injury-specific psychometric tests at intake,

Increasing prevalence of cirrhosis among insured adults in the United States, 2012-2018.

BACKGROUND: Liver cirrhosis is a chronic disease that is known as a "silent killer" and its true prevalence is difficult to describe. It is imperative to accurately characterize the prevalence of cirrhosis because of its increasing healthcare burden. METHODS: In this retrospective cohort study, trends in cirrhosis prevalence were evaluated using administrative data from one of the largest national health insurance providers in the US. (2011-2018). Enrolled adult (≥18-years-old) patients with cirrhosis defined by ICD-9 and ICD-10 were included in the study. The primary outcome measured in the …
