Impact of national reimbursement policy supporting test strips on glycemic control in people with insulin-treated diabetes: retrospective cohort study.

BACKGROUND: Self-monitoring blood glucose (SMBG) is an important factor for glycemic control in people with diabetes. Many countries have implemented policies to support SMBG for better glycemic control. The long-term impact of supporting the cost of SMBG strips for glycemic control in type 1 diabetes mellitus (T1DM) and insulin-treated type 2 DM (T2DM) have been determined in only a few studies. METHODS: People with T1DM and insulin-treated T2DM for whom the cost of blood glucose strips was and was not supported were included in the present study. Longitudinal changes in HbA1c were repeatedl

A Cohort Analysis of Postbariatric Panniculectomy--Current Trends in Surgeon Reimbursement.

BACKGROUND: The overall number of patients undergoing body contouring procedures after massive weight loss (MWL) has progressively increased over the past decade. The purpose of this study was to evaluate the charges and reimbursements for panniculectomy after MWL at a large academic institution in Massachusetts. METHODS: A retrospective review was performed and included all identifiable panniculectomy procedures performed at our institution between January 2008 and January 2014. The annual number of patients undergoing panniculectomy, the type of insurance coverage and reimbursement method o

Where Will Pathologic Hip Fractures Go in a Value-based Hip Fracture Bundle?

INTRODUCTION: There has been a burgeoning interest for implementing bundled payments for hip fractures being treated with hemiarthroplasty, percutaneous pinning, and/or open reduction and internal fixation. Concerns exist about how hip fracture bundles may impede access to care for patients who require more resources, such as those with pathologic/neoplastic fractures. METHODS: The 2011 to 2017 American College of Surgeons-National Surgical Quality Improvement Program database was queried to identify patients undergoing percutaneous pinning, hemiarthroplasty, plate/screw, and intramedullary n

Improving equitable collection and analysis of PROMIS Global Health data over time following spine surgery: characterizing survey nonresponse and missing data.

BACKGROUND CONTEXT: Patient-reported outcome measures (PROMs), the gold standard for outcome assessment in spine surgery, exhibit variability over time. Incomplete PROM collection, however, introduces nonresponse bias and limits the generalizability of time-based analyses of outcomes. PURPOSE: This study compared PROM-respondents and nonrespondents in spine surgery to characterize survey nonresponse and improve equitable patient representation in time series PROM analyses. STUDY DESIGN: Retrospective study. PATIENT SAMPLE: Patients undergoing surgery at a large, tertiary care center in the

Out-of-pocket payments, vertical equity and unmet medical needs in France: A national multicenter prospective study on lymphedema.

Out-of-pocket payments might threaten the vertical equity of financing and generate unmet medical needs. The main objective was to assess the vertical equity of outpatient out-of-pocket payments for lymphedema patients in France. Twenty-seven centres, among which 11 secondary care hospitals and 16 primary care practices participated in this prospective national multicenter study. We measured the lymphedema-specific outpatient out-of-pocket payments over 6 months. The vertical equity of out-of-pocket payments was examined using concentration curves, the Gini coefficient for income, the Kakwani

Persistence of major socio-economic inequalities in childhood measles-mumps-rubella vaccination coverage and timeliness under vaccination mandates, France, 2015 to 2024.

IntroductionSince the late 2000s, several major measles epidemics have occurred in Europe, including France. In 2017, the French Health Ministry extended from three to 11 the number of mandatory childhood vaccines required for preschool and primary school admission; these included the vaccine against measles, mumps and rubella (MMR).AimOur aim was to assess if this measure helped to improve MMR vaccine timeliness (VT) or reduce socioeconomic inequalities in MMR vaccine coverage.MethodsA nationwide study of three birth cohorts (2015, 2017, 2019) followed up 2.1 million children for 48 months to

Pharyngeal electrical stimulation for postextubation dysphagia after stroke: a randomized trial on hospitalization costs from a health insurance perspective.

Dysphagia is a frequent and serious complication after stroke, associated with pneumonia, malnutrition, prolonged intensive care unit stay, and higher healthcare costs. Pharyngeal electrical stimulation (PES) is a novel therapy that may support recovery from postextubation dysphagia. This study assessed the cost-effectiveness of PES in stroke patients from a health insurance perspective. We performed a secondary analysis of a randomized controlled trial including 60 stroke patients with postextubation dysphagia, randomized to PES (n = 30) or sham stimulation (n = 30). Sham treatment involved i

Prevalence and mortality of patients with multiple sclerosis in France in 2012: a study based on French health insurance data.

Data on the prevalence of multiple sclerosis (MS) in France are scarce. National and regional updated estimates are needed to better plan health policies. In this nationwide study, we provided estimates of the prevalence of MS in France in 2012 and mortality rate in 2013. MS cases were identified in the French national health insurance database (SNIIRAM-PMSI) using reimbursement data for disease-modifying treatment, long-term disease status for MS, disability pension for MS, and hospitalisation for MS (MS ICD-10 code: G35). We identified 99,123 MS cases, corresponding to an overall crude preva

Increased risk of hip fractures in patients with dementia: a nationwide population-based study.

BACKGROUND: Dementia has been associated with an increased risk of hip fracture. However, little research has been conducted on the impact of dementia on wrist or vertebral fracture development. The aim of this study was to investigate whether dementia is a risk factor for different types of fracture in Taiwan. METHODS: The study sample was drawn from Taiwan's National Health Insurance Research Database of reimbursement claims, and comprised 1408 patients who visited ambulatory care centers or were hospitalized with a diagnosis of dementia. The comparison group consisted of 7040 randomly sele

Cost Considerations in Penile Implantation Revision Surgery from a Global Perspective.

PURPOSE OF REVIEW: Penile prosthesis revision surgery represents a critical yet underexplored aspect of men’s health care. While primary penile prosthesis implantation offers a durable solution for erectile dysfunction unresponsive to medical therapy, complications such as infection, mechanical failure, and patient dissatisfaction often necessitate revision procedures. These surgeries are inherently more complex than primary implantations, requiring greater perioperative resources and incurring higher costs. This review synthesizes global evidence on the economic implications of penile prosthe

Cost-effectiveness analysis of parathyroidectomy for asymptomatic primary hyperparathyroidism.

BACKGROUND: Controversy exists concerning the best treatment for asymptomatic primary hyperparathyroidism (PHPT) when the National Institutes of Health consensus conference criteria for parathyroidectomy are not met. We hypothesized that parathyroidectomy would be more cost-effective than observation or pharmacologic therapy for these patients. METHODS: Cost-effectiveness analysis was performed comparing treatment strategies for asymptomatic PHPT. Treatment outcomes, their probabilities, and costs were identified on the basis of literature and cost database review. Outcomes were weighted by u

Policy and Payment Factors Influencing Adoption of Low-Barrier Buprenorphine Treatment.

IMPORTANCE: Treatment of opioid use disorder (OUD) with buprenorphine reduces overdose and all-cause mortality, yet access and retention remain limited. Much of the literature describing barriers to buprenorphine access and retention has focused on practitioner-level or patient-level barriers, but less is known about how potential policy and payment levers may influence prescribing behaviors and treatment practices from the practitioner perspective. OBJECTIVE: To explore perspectives among OUD treatment program clinicians and staff about how broader policy and payment structures influence ado

Personalized medicine: Function of CFTR variant p.Arg334Trp is rescued by currently available CFTR modulators.

Most of the 2,100 CFTR gene variants reported to date are still unknown in terms of their disease liability in Cystic Fibrosis (CF) and their molecular and cellular mechanism that leads to CFTR dysfunction. Since some rare variants may respond to currently approved modulators, characterizing their defect and response to these drugs is essential for effective treatment of people with CF (pwCF) not eligible for the current treatment. Here, we assessed how the rare variant, p.Arg334Trp, impacts on CFTR traffic and function and its response to existing CFTR modulators. To this end, we performed th
