Patient overcrowding in hospital wards as a predictor of diagnosis-specific mental disorders among staff: a 2-year prospective cohort study.

OBJECTIVE: Hospital ward patient overcrowding has been hypothesized to increase psychiatric morbidity among staff, but it is unknown whether the association is specific to depressive disorders. This study examined whether patient overcrowding in hospital wards predicts diagnosis-specific mental disorders among staff. METHOD: A 2-year prospective cohort study was conducted, in which the extent of hospital ward overcrowding was determined using routinely recorded patient bed occupancy rates between 2003 and 2004 and linked to sickness absence for 5,166 nurses and physicians in 203 somatic illne

Preventive service utilization among adults with cystic fibrosis covered by private insurance is comparable to the general population.

BACKGROUND: As the life expectancy of the cystic fibrosis (CF) population is lengthening with modulator therapies, diligent age-appropriate screening and preventive care are increasingly vital for long-term health and wellbeing. METHODS: We performed a retrospective analysis comparing rates of receiving age- and sex-appropriate preventive services by commercially insured adult people with CF (PwCF) and adults without CF from the general population (GP) via the Truven Health MarketScan database (2012-2018). RESULTS: We captured 25,369 adults with CF and 488,534 adults from the GP in the Unite

Dual-Site Pain in the Low Back and Knee as a Risk Factor for Dementia: A Population-Based Cohort Study.

PURPOSE: Chronic pain is a major public health concern and has been linked to cognitive decline. However, the effect of pain occurring simultaneously in two musculoskeletal sites on dementia risk remains unclear. In this study, we aimed to examine whether dual-site pain (DSP; in this study defined as cooccurring chronic low back and knee pain) is associated with an increased risk of dementia in community-dwelling older adults. PATIENTS AND METHODS: We analyzed data from 2060 participants aged ≥65 years in the Locomotive Syndrome and Health Outcomes in the Aizu Cohort Study. DSP was defined as

Unresolved legal and ethical issues in research of adults with severe traumatic brain injury: analysis of an ongoing protocol.

This paper synthesizes federal and state laws and bioethics literature with observations from an ongoing research protocol to identify, define, and clarify the unresolved legal and ethical issues regarding research involving adults with traumatic brain injury (TBI). Solutions that protect rights and minimize unnecessary impediments to valuable clinical and scientific inquiry are also illustrated using the same protocol. Research was performed at intensive care, inpatient rehabilitation, and long-term acute chronic hospitals. Our research protocol identified five areas of law impacting adults w

Age at Breast Cancer Diagnosis and Short-Term Postoperative Outcomes for Women Treated in a Universal Health System.

IntroductionSurgery is an essential component of breast cancer treatment. Surgical complications may impact receipt of adjuvant therapy and long-term outcomes. Differences in insurance coverage and access to care by patient age in the general U.S. population may affect breast cancer diagnosis and surgery and thus age comparisons in research. We compared postoperative outcomes following breast cancer surgery between age groups in the U.S. Military Health System, which provides beneficiaries access to comprehensive care across the lifespan.MethodsWe identified women aged ≥18 years with stage I-I

Impact of Medicaid expansion on the multimodal treatment of biliary tract cancer.

INTRODUCTION: The impact of Medicaid expansion (ME) on the treatment of patients with cancer remains controversial, especially individuals requiring complex multidisciplinary care. We sought to evaluate the impact of Medicaid expansion (ME) on receipt of multimodal care, including surgical resection, for Stage I-III biliary tract cancer (BTC). METHODS: Patients diagnosed with BTC between 40 and 65 years of age were identified from the National Cancer Database and divided into pre- (2008-2012) and post- (2015-2018) ME cohorts. Difference-in-difference (DID) analysis was used to determine the i

Prevalence of Psychological Frailty in Japan: NCGG-SGS as a Japanese National Cohort Study.

There has been less research conducted on the psychological aspects of frailty than on the physical and cognitive characteristics of frailty. Thus, we aimed to define psychological frailty, clarify its prevalence, and investigate the relationship between psychological frailty and lifestyle activity or disability incidence in older adults in Japan. The participants in our study were 4126 older adults (average age 71.7 years) enrolled in the National Center for Geriatrics and Gerontology-i87uStudy of Geriatric Syndromes (NCGG-SGS). We characterized physical frailty of the following as ≥ 3: slow

[Costs of a guideline-based treatment of patients with chronic hepatitis C in the era of interferon-free treatment].

The treatment of chronic hepatitis C has considerably changed with the introduction of recent direct acting antivirals. These antivirals have sustained virologic response (SVR) rates above 90 % as well as reduced toxicity and treatment duration. Therefore, current German guidelines recommend these interferon-free regimens as first-choice treatment. Nevertheless, recent developments were accompanied by a significant increase in treatment costs, which led to extensive discussions on reasonable pharmaceutical prices. The aim of the current study was to analyze the average treatment costs and cost

Outpatient benzodiazepine utilization in Croatia: drug use or misuse.

Background Benzodiazepines are commonly prescribed medications, especially among elderly, despite known risks and guidelines focused on short term usage. There is an increased trend of benzodiazepine consumption in Croatia. Consumption of anxiolytics in 2015 and 2016 in Croatia can almost entirely be ascribed to benzodiazepines, with diazepam being the most commonly prescribed drug, followed by alprazolam. Objective The aim of this study was to examine benzodiazepine utilization habits among the Croatian population. Setting This study was conducted on the national level, based on digital presc

COVID-19 infections in German long-term care facilities: a descriptive three-level analysis using claims and infection statistics data from October 2020 to March 2021.

BACKGROUND: Although many studies have investigated COVID-19 outbreaks in long-term care facilities (LTCFs), evidence that combines multiple clustered levels is scarce. We aimed to describe individual, LTCF, and regional-level factors associated with COVID-19 infections. METHODS: We conducted a nationwide study using insurance claims data from Germany between 1st October 2020 and 31st March 2021. The sample comprised 284,186 residents over 60 years in 9,869 LTCFs across all of Germany's 400 districts. We used multilevel logistic regression to model associations between individual, LTCF, and d

Regional variations in morbidity and mortality among neonates with intraventricular hemorrhage: a national database analysis.

BACKGROUND: Intraventricular hemorrhage (IVH) often affects newborns of low gestational age and low birth weight, requires critical care for neonates, and is linked to long-term neurodevelopmental outcomes. Assessing regional differences in the U.S. in care for neonatal IVH and subsequent outcomes can shed light on ways to mitigate socioeconomic disparities. METHODS: Using the 2016-2019 National Inpatient Sample (NIS), patients with a primary diagnosis of IVH were identified using ICD-10-CM codes. A retrospective cohort study was conducted with patients stratified by hospital region. Demograp

Prehabilitation in older patients prior to elective cardiac procedures (PRECOVERY): study protocol of a multicenter randomized controlled trial.

BACKGROUND: Previous studies have demonstrated the efficacy of rehabilitation after a cardiovascular procedure. Especially older and multimorbid patients benefit from rehabilitation after a cardiac procedure. Prehabilitation prior to cardiac procedures may also have positive effects on patients' pre- and postoperative outcomes. Results of a current meta-analysis show that prehabilitation prior to cardiac procedures can improve perioperative outcomes and alleviate adverse effects. Germany currently lacks a structured cardiac prehabilitation program for older patients, which is coordinated acros
