Surgical and medical treatment of secondary hyperparathyroidism in patients on continuous dialysis.

BACKGROUND: Secondary hyperparathyroidsm (2HPT) is a common complication in hemodialysis patients associated with morbidity and sometimes mortality. The majority of patients with 2HPT can be managed by medical treatment. However, medical treatment does not always provide control of parathyroid disorder. Some patients require surgical treatment, parathyroidectomy (PTx). Successful surgical treatment often results in dramatic drop of the parathyroid hormone (PTH) levels, relieves the patients from clinical symptoms, and reduces mortality. However, at present we do not have reliable evidences sho

[Aortic valve replacement in elderly patients].

Aortic valve procedures represent the second most frequent surgical intervention in cardiac surgery. Due to the demographic developments there is a shift into the seventh to ninth decade of age. For patients beyond 65 years biological valve prostheses are recommended. In these patients xenografts are showing only a slow degeneration. Furthermore, especially these patients are threatened by complications caused by cumarine therapy. Modern valve-sparing procedures might also be used in old patients with an insurable risk. Mortality is slightly increased in old patients compared to younger patien

[Management of the compartment syndrome in hemophilic patients].

Diffuse bleeding in the muscles or localized hematomas are illness-specific factors increasing the possibility of a posttraumatic compartment syndrome in hemophiliac patients. Sufficient substitution of coagulation factor is the first step after trauma. The main principals of surgical treatment are:--a generous fasciotomy with a sufficient skin incision to allow adequate hemostasis under direct visualization. Localized hematomas should be evacuated;--fractures must be definitively stabilized as soon as possible. Therefore, internal fixation should be performed primarily whenever possible;--pri

Gastrointestinal complications of gastroschisis.

The mortality of infants with gastroschisis has been reduced markedly in the last decade with the application of new surgical techniques, improved metabolic monitoring, and total parenteral nutrition. The late complications of repaired gastroschisis are now emerging. In this series of 30 infants with gastroschisis (20% mortality) significant gastroesophageal reflux was identified in seven of 10 infants with the appropriate barium study. The clinical symptom complexes of dysmotility, a recognized problem in gastroschisis, and gastroesophageal reflux can be easily confused. In addition, 11 episo

Direct Economic Burden of Cubital Tunnel Surgery in the United States: Total Payments and Components of Cost.

PURPOSE: Despite its widespread prevalence, the cost of cubital tunnel syndrome (CuTS) in the United States to patients and insurers is not well understood. The purpose of this study was to quantify the direct payments associated with operative treatment of CuTS. We hypothesized that CuTS represents a substantial cost to the payer in facility fees, surgeon fees and other expenses. METHODS: Utilizing the MarketScan database of insured patients (commercial and Medicaid), we identified a cohort of 41,777 patients aged 18-64 years with surgically treated CuTS from 2006 to 2018. We estimated the m

Digital Versus In-Person Physical Therapy in Adults With Musculoskeletal Conditions: Retrospective Matched-Cohort Analysis of Surgery and Low-Value Surgical Rates.

BACKGROUND: Musculoskeletal (MSK) disorders are leading causes of disability worldwide, with clinical guidelines recommending physical therapy-based interventions. Digital MSK programs offer an alternative to address logistical and socioeconomic barriers to regular in-person care. However, evidence comparing surgical use between digital and in-person physical therapy remains limited, particularly for low-value procedures. OBJECTIVE: This study aimed to evaluate the 12-month incidence of MSK surgery and low-value surgical procedures among participants initiating a multimodal Digital Care Progr

Socioeconomic Disparities in Secondary Procedures for Pediatric Distal Radius Fractures.

BACKGROUND: Repeat reduction and surgical fixation are often performed in children with distal radius fractures failing initial closed reduction (CR). While socioeconomic disparities have been described in adult orthopaedic conditions, less is known about the pediatric population. The purpose of this investigation was to determine risk factors, if any, for differences in secondary procedure rates in children with distal radius fractures. METHODS: Of 2,072 patients in a multicenter prospective cohort of pediatric distal radius fractures, 270 had radiographic loss of reduction (LOR) following i

Postoperative direct health care costs of lumbar discectomy are reduced with the use of a novel annular closure device in high-risk patients.

BACKGROUND CONTEXT: Lumbar discectomy is largely successful surgical procedure; however, reherniation rates in patients with large annular defects are as high as 27%. The expense associated with a revision surgery places significant burden on the healthcare system. PURPOSE: To compare the direct health care costs through 5 years follow-up of conventional discectomy (Control) with those of discectomy supplemented by an adjunctive annular closure device (ACD) in high-risk patients with large annular defects. STUDY DESIGN: This was a cost-effectiveness study. METHODS: All-cause index level reo

[Current Status and Future Prospects of Minimally Invasive and Function-Preserving Surgery].

The keywords of surgical treatment in the 21st century are "minimally invasive" and "function preserved". "Laryngeal function" is important in the head and neck region. The goal of laryngeal function preservation surgery is to be able to talk on "telephone" (vocalization function), to be able to "eat out" (chewing/swallowing function), "bath to shoulder" (nasal respiration function). Minimally invasive surgery and function-preserving surgery for head and neck cancer have been developed mainly for transoral surgery. In Japan, oral endoscopic surgery is the main surgical method. Transoral videol

Pediatric myringoplasty: Prognostic factors in surgical outcome and hearing threshold recovery.

CONCLUSIONS: This study shows as decisive factors the history of tympanostomy tube insertion and surgical approach. The closure of the perforation observed at the 6th post-operative month is almost definitive, without finding significant differences ahead. The myringoplasty can be performed simultaneously in bilateral perforations and in a day-case basis with same success rates. Astatistically significant audiometric improvement area is described, after myringoplasty, undescribed previously in literature. OBJECTIVES: The aim of this study was to evaluate prognostic factors in surgical outcome

Bridging the Gap: Implementation and Uptake of AUDIT-C Alcohol Screening for Inpatient Surgery.

OBJECTIVE: To evaluate the feasibility of Alcohol Use Disorders Identification Test-Consumption (AUDIT-C) screening among surgical inpatients following hospital-wide implementation of a quality improvement initiative. BACKGROUND: Risky alcohol use increases postoperative complications, yet alcohol screening for surgery remains inconsistently applied. As a brief, validated screening tool, the AUDIT-C may improve risk assessment and guide timely intervention, but its adoption in surgical settings is understudied. METHODS: We retrospectively analyzed electronic health record data from inpatient

Area Deprivation Index (ADI) as a Predictor of Surgical Complications in Patients Undergoing Mastectomy.

INTRODUCTION: Socioeconomic disparities significantly influence health outcomes, particularly in surgical care. The Area Deprivation Index (ADI), a comprehensive measure of neighborhood-level socioeconomic status, is increasingly recognized as a predictor of clinical outcomes. This study examines the association between ADI and postoperative complications in patients undergoing mastectomy, hypothesizing that higher ADI scores correlate with worse outcomes. METHODS: A retrospective cohort analysis was conducted using the ACS-NSQIP database to evaluate 1141 female patients who underwent mastect

Factors associated with short-term hospital readmission rates for breast cancer patients in Western Australia: an observational study.

BACKGROUND: Unplanned hospital readmissions after surgical treatment for breast cancer are an indicator of morbidity. We explore the relationship between the rate of unplanned hospital readmissions within 42 days of initial treatment and various factors, including tumor size and histology, lymph node involvement, type of surgical treatment, mastectomy, or breast-conserving surgery, and patient demographics. METHODS: Linked Western Australian cancer mortality and hospital morbidity data were used in the assessment of readmissions within a period of 42 days after initial surgical treatment for

Natural rubber latex-related occupational asthma: association with interventions and glove changes over time.

BACKGROUND: Exposure to natural rubber latex (NRL) has been recognized as a cause of occupational asthma (OA), especially among health care workers (HCWs) associated with use of gloves. Little or no population-based data are available which chart the changes over time in the number of cases of OA as this problem was increasingly recognized and then interventions instituted. METHODS: We obtained the numbers of allowed asthma claims with exposure to latex by year in the province of Ontario, Canada from the Ontario Workplace Safety and Insurance Board; details of the cases from the largest teach
