Inotrope use among extremely preterm infants in Canadian neonatal intensive care units: variation and outcomes.

OBJECTIVE: To compare neonatal outcomes between infants who received inotropes and those who did not, and identify variation in inotrope use. STUDY DESIGN: Retrospective review of data from neonates < 29 weeks gestation collected by the Canadian Neonatal Network during 2003 to 2010. After controlling for confounders and maternal/infant characteristics, rates of mortality and major morbidity were compared between those who received inotropes on days 1 and 3 of admission and those who did not. Rate of inotrope use was compared between sites. RESULTS: Inotropes were administered to 772 (10%) of

Depression and anxiety symptoms in mothers of newborns hospitalized on the neonatal intensive care unit.

BACKGROUND: Mothers of infants hospitalized in the neonatal intensive care unit (NICU) are at risk for clinically significant levels of depression and anxiety symptoms; however, the maternal/infant characteristics that predict risk have been difficult to determine. Previous studies have conceptualized depression and anxiety symptoms separately, ignoring their comorbidity. Moreover, risk factors for these symptoms have not been assessed together in one study sample. OBJECTIVES: The primary aim of this study was to determine whether a diagnostic classification approach or a common factor model

Spread of NDM-1-producing Enterobacteriaceae in a neonatal intensive care unit in Istanbul, Turkey.

Twenty-two consecutive carbapenem-resistant enterobacterial isolates were recovered from patients hospitalized between January and April 2013 in different units at a university hospital in Istanbul, Turkey. These were Klebsiella pneumoniae isolates producing the carbapenemases OXA-48, NDM-1, and KPC-2, Enterobacter cloacae isolates producing NDM-1, and Escherichia coli isolates producing OXA-48. Most of the OXA-48-producing K. pneumoniae and all the NDM-1-producing E. cloacae were clonally related. The NDM-1-producing E. cloacae isolates recovered from a single neonatal intensive care unit cor

Trends of Staphylococcus aureus bloodstream infections in a neonatal intensive care unit from 2000-2009.

BACKGROUND: Invasive methicillin-resistant Staphylococcus aureus (MRSA) and methicillin-sensitive Staphylococcus aureus (MSSA) infections are major causes of numerous neonatal intensive care unit (NICU) outbreaks. There have been increasing reports of MRSA outbreaks in various neonatal intensive care units (NICUs) over the last decade. Our objective was to review the experience of Staphylococcus aureus sepsis in our NICU in the last decade and describe the trends in the incidence of Staphylococcus aureus blood stream infections from 2000 to 2009. METHODS: A retrospective perinatal database re

Infection prevention practices in neonatal intensive care units reporting to the national healthcare safety network.

BACKGROUND: Patients in the neonatal intensive care unit (NICU) are at high risk for healthcare-associated infections. Variability in reported infection rates among NICUs exists, possibly related to differences in prevention strategies. A better understanding of current prevention practices may help identify prevention gaps and areas for further research. METHODS: We surveyed infection control staff in NICUs reporting to the National Healthcare Safety Network (NHSN) to assess strategies used to prevent methicillin-resistant Staphylococcus aureus (MRSA) transmission and central line-associated

[Management of late-onset sepsis due to coagulase-negative staphylococci in neonatal intensive care units].

Coagulase-negative staphylococci (CoNS) are the most frequent cause of late-onset sepsis (LOS) in neonatal intensive care units (NICUs). Staphylococcus epidermidis is usually considered the most prevalent CoNS in this setting. However, recent reports have identified Staphylococcus capitis, another CoNS, as an emerging cause of bacteremia in NICU wards. S. capitis is the main cause of LOS in several NICUs in France, whereas this species is rarely found in adult patients from the same hospitals. S. capitis isolates from NICU infants share several striking features: they all belong to the same pu

Risk factors and clinical analysis for invasive fungal infection in neonatal intensive care unit patients.

OBJECTIVE: In this study, we seek to determine independent risk factors of invasive fungal infection (IFI) in neonatal infants. STUDY DESIGN: The medical charts of 5135 neonatal intensive care unit admissions in the past 7 years between January 2004 and December 2010 were reviewed and 45 neonates were found with IFI. Two controls, matched by gestational age, birth weight category, admission date, ward, hospital stay, and admission age, were selected for each case. RESULTS: Candida parapsilosis was the leading causative pathogen of IFI and was isolated in 33.3% of the patients. The mortality

The epidemic of multiple gestations and neonatal intensive care unit use: the cost of irresponsibility.

OBJECTIVES: To determine the proportion of infants admitted to our neonatal intensive care unit (NICU) from multiple gestations resulting from artificial reproductive technology (ART), the complications experienced and interventions required by these infants, and the estimated effect of a mandatory policy of single embryo transfer on admissions and complication rates in our hospital and across Canada. STUDY DESIGN: We conducted a review of a prospectively maintained database and of hospital records and calculated excess complications compared with either universal single embryo transfer or a

Mucosal surveillance cultures in predicting Gram-negative late-onset sepsis in neonatal intensive care units.

This study aimed to examine the spectrum and time course of gut and nasopharyngeal colonization with Gram-negative micro-organisms, and to define the value of surveillance cultures in predicting late-onset sepsis in neonates admitted to neonatal intensive care units. Nasopharyngeal and rectal swabs were collected on admission and twice weekly thereafter in 278 neonates admitted within the first 72 h of life with risk factors of early-onset sepsis. Sterile body fluid cultures were obtained on admission and subsequently as clinically indicated. Approximately half of the rectal (693/1250, 55%) an

Neonatal intensive-care unit admission of infants with very low birth weight --- 19 States, 2006.

Neonatal mortality is disproportionately common among infants with very low birth weight (VLBW) (<1,500 g [3.3 lbs]). In 2006, the mortality rate among infants with VLBW was 240.4 per 1,000 live births. Because neonatal intensive care has been shown to reduce mortality among infants with VLBW, current standards call for neonatal intensive-care for all infants with VLBW; however, the proportion of infants with VLBW who are admitted to a neonatal intensive care unit (NICU) is not known, nor are the predictors for NICU admission. To estimate the prevalence of admission to NICUs among infants with

Trends in the rates of cerebral palsy associated with neonatal intensive care of preterm children.

Progressive changes in perinatal and neonatal intensive care of preterm infants since the late 1960s have led to an increase in survival and had an effect on the rates of neonatal morbidity, including brain injury, chronic lung disease, and sepsis. These have influenced the rates of neurodevelopmental impairment, including cerebral palsy. There was initially an increase in neonatal morbidity and rates of cerebral palsy associated with the increased survival of extremely low birth weight and low gestation infants. However, since the late 1990s and especially since the year 2000, the rates of ne

An outbreak of Serratia marcescens septicaemia in neonatal intensive care unit in Gaza City, Palestine.

We performed a case-control study of Serratia marcescens incidence in the neonatal intensive care unit of a governmental Gaza Strip hospital from January to December 2005. S. marcescens was detected in the blood of 159 confirmed nosocomial cases of septicaemia, 70 (44%) neonates died due to S. marcescens infection and 89 recovered. The main clinical symptoms were hypothermia 38%; jaundice 42%; Apgar score 4 at 1 min in 29% of neonates; and Apgar score 5 at 5 min in 5%. Risk factors significantly associated with S. marcescens infection were birthweight <1,500 g (OR: 1.7; P=0.026); <37 weeks ges

An architecture for multi-dimensional temporal abstraction and its application to support neonatal intensive care.

Temporal abstraction (TA) provides the means to instil domain knowledge into data analysis processes and allows transformation of low level numeric data to high level qualitative narratives. TA mechanisms have been primarily applied to uni-dimensional data sources equating to single patients in the clinical context. This paper presents a framework for multi-dimensional TA (MDTA) enabling analysis of data emanating from numerous patients to detect multiple conditions within the environment of neonatal intensive care. Patient agents which perform temporal reasoning upon patient data streams are

Cumulative effective doses delivered by radiographs to preterm infants in a neonatal intensive care unit.

OBJECTIVE: We sought to determine the number and distribution of radiographs and the cumulative effective radiograph doses (cED) received by a population of preterm infants (PIs) hospitalized in an NICU. STUDY DESIGN: We reviewed the files of all preterm infants (gestational age: <34 weeks) who were admitted to an NICU during an 18-month period and were discharged alive. A generalized additive model was used to study the relationship between cED and patient characteristics. RESULTS: Four hundred fifty files were analyzed. The median gestational age was 30.1 weeks (range: 24.1-33.9 weeks), an

Hope in the neonatal intensive care nursery: values, ethics, and the injury of continued existence.

Metabolic dysfunction-associated steatohepatitis (MASH) is not solely a disorder of hepatocellular lipid accumulation, but a multicellular disease driven by coordinated metabolic stress, sterile inflammation, fibrogenesis, and niche remodeling. Recent therapeutic progress with the provisional approval of resmetirom and semaglutide has validated MASH as a tractable clinical target. However, many experimental agents have shown limited or inconsistent efficacy, particularly for regression of hepatic fibrosis or cirrhosis, reflecting the biological heterogeneity and dynamic cellular architecture o

Extrauterine growth restriction in preterm infants: importance of optimizing nutrition in neonatal intensive care units.

Extrauterine growth restriction in preterm infants secondary to suboptimal nutrition is a major problem in neonatal intensive care units. Evidence is emerging that early growth deficits have long-term adverse effects, including short stature and poor neurodevelopmental outcomes. The parenteral route of feeding is essential to maintain nutritional integrity before successful transition to the enteral route of feeding is achieved. Nevertheless, early initiation of enteral feeding in sub-nutritional trophic quantity is vital for promoting gut motility and bile secretion, inducing lactase activity

A simultaneous outbreak of Serratia marcescens and Klebsiella pneumoniae in a neonatal intensive care unit.

We describe two concurrent outbreaks of Serratia marcescens and Klebsiella pneumoniae in a neonatal intensive care unit (NICU). Over a 16-month period, a total of 27 infants were either colonized (N=14) or infected (N=13). There were 15 cases of S. marcescens and 11 cases of K. pneumoniae. Both micro-organisms were involved in one fatal case. Seven preterm babies developed septicaemia, two had bacteraemia, three had respiratory infections and one had purulent conjunctivitis. The S. marcescens and K. pneumoniae isolates were investigated by three molecular methods: enterobacterial repetitive in
