Review article: investigation and management of internal fistulae in Crohn's disease.

BACKGROUND: Crohn's disease is an inflammatory, penetrating intestinal disease associated with fistula formation. Fistulae in Crohn's disease can be classified into external and internal fistulae. Internal fistulae form between the gastrointestinal tract and another internal organ and include enteroenteric, enterocolic, enterovesical and rectovaginal fistulae. They are associated with significant morbidity and a decreased quality of life. AIM: To review the classification, diagnosis, medical and surgical management of internal fistulae in Crohn's disease, and propose a treatment algorithm. M

Laparoscopic management of cholecystocolic fistula.

INTRODUCTION: Cholecystocolic fistula is a rare complication of gallbladder disease. Its clinical presentation is variable and nonspecific, and the diagnosis is made, mostly, incidentally during intraoperative maneuver. Cholecystectomy with closure of the fistula is considered the treatment of choice for the condition, with an increasingly reproducible tendency to the use of laparoscopy. AIM: To describe the laparoscopic approach for cholecystocolic fistula and ratify its feasibility even with the unavailability of more specific instruments. TECHNIQUE: After dissection of the communication a

Uretero-appendiceal fistula.

We report a case of uretero-appendiceal fistula. A 63-year-old woman was admitted to our hospital with high grade fever and loss of 5 kg of body weight over 6 months. Pyonephrosis was diagnosed. A retrograde ureterogram and barium enema showed a connection between cecum and ureter. Right nephroureterectomy and right hemicolectomy were carried out. The final diagnosis was uretero-appendiceal fistula.

Sigmoid-urachal-cutaneous fistula in an adult male.

An infected urachal cyst is an uncommon finding in adults. We report the first case of a sigmoid-urachal-cutaneous fistula that resulted from rupture of an infected urachal cyst in an adult male. Definitive management consisted of resection of the urachus with a bladder cuff, along with removal of the affected bowel segments and bowel anastomosis.

Oral intake of poppy seed: a reliable and simple method for diagnosing vesico-enteric fistula.

PURPOSE: In up to 50% of patients with vesico-enteric fistula conventional diagnostic methods fail. We report a simple and low cost diagnostic method that involves the oral intake of poppy seed. MATERIALS AND METHODS: From 1994 to 1999, 17 patients in whom vesico-enteric fistula was suspected underwent the poppy seed test. We administered 250 gm. poppy seed orally in the evening and the urinary excretion of poppy seed was evaluated during the next 2 days. RESULTS: Of the 17 patients 11 excreted the black seeds in the urine for 2 days after oral intake of the seeds. The diagnosis was supporte

Surgical management and treatment of sepsis associated with gastrointestinal fistulas.

The development of sepsis associated with a GI fistula can be a catastrophic complication of any surgical procedure in the vicinity of the abdominal cavity. The predominant sites of infection directly associated with GI fistulas are in the surgical wound and within the abdominal cavity. Some patients present with florid signs of sepsis, whereas others may have minimal signs of infection. CT scanning is the main diagnostic method for intra-abdominal collections. Often, it also provides a means of treatment by percutaneous placement of catheters. Patients who develop extensive cellulitis or necr

Tandem colovaginoscopy in the diagnosis of colovaginal fistula.

The diagnosis of colovaginal fistula with the traditional methods of contrast enema, sigmoidoscopy, and vaginal speculum examination can be problematic. Precise identification of the pathologic anatomy underlying the colovaginal fistula can be obtained by simultaneous flexible fiberoptic vaginoscopy and colonscopy.
