A Dimer, but Not Monomer, of Tobramycin Potentiates Ceftolozane against Multidrug-Resistant and Extensively Drug-Resistant Pseudomonas aeruginosa and Delays Resistance Development.

Ceftolozane-tazobactam is a potent β-lactam/β-lactamase inhibitor combination approved for the treatment of complicated intraabdominal and complicated urinary tract infections and, more recently, the treatment of hospital-acquired and ventilator-associated bacterial pneumonia. Although the activities of ceftolozane are not enhanced by tazobactam against Pseudomonas aeruginosa, it remains the most potent antipseudomonal agent approved to date. Emerging data worldwide has included reports of microbiological failure in patients with serious bacterial infections caused by multidrug-resistant (MDR)

Activity of ceftolozane-tazobactam and comparators against Pseudomonas aeruginosa from patients in different risk strata - SMART United States 2016-2017.

OBJECTIVES: Infections caused by Pseudomonas aeruginosa are often difficult to treat. Knowledge of the risk of infection with resistant P. aeruginosa would allow more discriminatory prescribing of broad-spectrum antimicrobials. Using clinical isolates collected as part of the Study for Monitoring Antimicrobial Resistance Trends (SMART), we examined the activity of commonly used β-lactams, levofloxacin, and ceftolozane-tazobactam (C/T), an antipseudomonal cephalosporin/β-lactamase inhibitor approved in the United States and over 60 countries worldwide, against P. aeruginosa isolates from patien

[Liver transplant in childhood: our experience].

From december 1984 to december of 1991, 12 children underwent on orthotopic liver transplantation (OLT): 6 had extrahepatic biliary atresia (EHBA), 2 had Byler disease, 2 hepatocellular carcinoma (HCC), 1 Alagille Syndrome and 1 had a hyperacute Wilson disease. The children, transplanted for the most part a broad, return for observation 3 months after OLT. A patient with hyperacute Wilson's Disease had 2 emergency OLTs and died of sepsis (due to Aspergillus); another with EHBA, operated for hepatoportoenterostomy, without result, died after OLT because of a ruptured aortic aneurysm. Perforated Stercoral Ulceration of Transverse Colon in the Setting of Multiple Comorbidities: A Case Report.

Stercoral colitis is an inflammatory condition resulting from the long-term collection of fecal matter that may lead to colonic ulceration and perforation, consequently representing a surgical emergency. Herein, we present the case of a 65-year-old female who developed stercoral ulceration of the transverse colon, an uncommon location of occurrence. The patient had multiple comorbid conditions and risk factors for stercoral colitis that included a history of constipation, major depressive disorder, hypothyroidism, and a partial colonic resection due to an upper rectal perforation secondary to

A scarless technique of umbilical hernia repair in the adult population.

BACKGROUND: Umbilical hernias are common in adults during their 30s-60s. Most umbilical hernias result in increased intraabdominal pressure and prompt repair is advised. The technique described here involves dissection of the hernia sac from the subcutaneous tissue with a horizontal fascial closure in a tension-free procedure. METHODS: Records of 57 patients treated for umbilical hernia in a 7-year period were reviewed retrospectively. Data were collected regarding sex, age, follow up, and complications. RESULTS: All patients were treated successfully. Postoperatively, eight complications we

Abdominal ultrasound in acute schistosomiasis mansoni.

Reports on abdominal ultrasound studies in patients with acute schistosomiasis are still scarce and limited data are available on structural changes of the liver parenchyma in this stage of the disease. 26 patients with acute schistosomiasis mansoni were submitted to clinical and ultrasound examination. For ultrasound comparison, each acute patient was paired by age, gender, weight and height to a non-infected individual. Ultrasound showed a non-specific homogeneous size increase of the liver, and spleen in all acute patients, and easily identified intraabdominal lymph nodes in the periportal

[A new hereditary cause of portal vein thrombosis: the abnormal resistance to activated protein C by the Arg 506-->Gln mutation of the gene of factor V].

We report 2 cases of portal vein thrombosis associated with a single point mutation in the factor V gene that replaces arginine in residue 506 with glutamine. This mutation induces abnormal resistance to anticoagulant activity of activated protein C and increases the risk of deep vein thrombosis. Both patients had a personal and familial history of deep vein thrombosis. Intraabdominal neoplasia or infection, myeloproliferative disorder, antiphospholipid syndrome, paroxysmal nocturnal hemoglobinuria and coagulation inhibitor deficiency (antithrombin, proteins C and S) were excluded by exhaustiv

Effect of aspoxicillin on anaerobic bacteria.

Aspoxicillin (ASPC), a semisynthetic penicillin has a broad spectrum of antibacterial activities against Gram-positive and Gram-negative anaerobic bacteria. Its in vitro antibacterial activity was less than those of cefoxitin against Peptostreptococcus and Veillonella, but was significantly high against Bacteroides fragilis, one of the most clinically important anaerobe. The therapeutic and/or protective effect of ASPC in experimental subcutaneous abscess or experimental intraabdominal mixed infection due to beta-lactamase producing B. fragilis and non-producing Escherichia coli were much stro

Impact of abdominal drainage time and bed rest duration on postoperative recovery in patients undergoing laparoscopic appendectomy: a single-center retrospective study.

BACKGROUND: The optimal duration of abdominal drainage and bed rest after laparoscopic appendectomy (LA) remains uncertain. This study investigated whether these modifiable factors are associated with residual intra-abdominal infection (RII) and prolonged length of hospital stay (LOS). OBJECTIVE: To retrospectively analyze the risk factors for RII and prolonged LOS following LA and clarify the impact of abdominal drainage time and bed rest duration on postoperative recovery. METHODS: This retrospective study included 478 adults who underwent LA for acute appendicitis (2017-2020). Patients we

Moxifloxacin plus Cordyceps polysaccharide ameliorate intestinal barrier damage due to abdominal infection via anti-inflammation and immune regulation under simulated microgravity.

BACKGROUND: Currently, there is limited research on the impact of abdominal infection on intestinal damage under microgravity conditions. Cordyceps polysaccharide (CPS), the main active ingredient of Cordyceps, has demonstrated various pharmacological effects, including anti-inflammatory, antioxidant, and immunomodulatory properties. Moxifloxacin (MXF) is a fourth-generation quinolone antibiotic that is believed to have a dual regulatory effect on immune system activation and suppression. Our objective was to investigate the effects of MXF plus CPS on the intestinal barrier damage due to abdom

Bacteriology of Spontaneous Intra-Abdominal Abscess in Patients with Crohn Disease in China: Risk of Extended-Spectrum Beta-Lactamase-Producing Bacteria.

BACKGROUND: Crohn disease (CD) has its own features and the etiology is believed to be related to the gastrointestinal flora, which are the cause of most intra-abdominal infections. However, few experiences have been reported regarding the specific bacteriology of spontaneous intra-abdominal abscess (IAA) in CD patients and related risk factors. In the present study, we aim to investigate the bacteriology of IAA in CD and the associated risk factors. METHODS: Retrospective study on 97 CD patients suffering from IAA was performed from January 2007 to December 2012. Microbiologic information of

Microbiological spectrum of the intraperitoneal surface after elective right-sided colon cancer: are there differences in the peritoneal contamination after performing a stapled or a handsewn anastomosis?

BACKGROUND: Although colonic surgery is performed with strict aseptic measures, some contamination is nearly impossible to avoid. In stapled anastomosis, the hole opened in the colon is minimum, just necessary for introducing the parts of the mechanical devices. In handsewn anastomosis, the colonic lumen is more exposed to the peritoneum, despite colonic occlusion with clamps while the suture is performed. PATIENTS AND METHODS: A prospective, randomized study was performed between October 2009 and June 2011. Inclusion criteria were a diagnosis of right-sided colon cancer and having undergone

Antimicrobial susceptibility of pathogens isolated from patients with complicated intra-abdominal infections at five medical centers in Taiwan that continuously participated in the Study for Monitoring Antimicrobial Resistance Trends (SMART) from 2006 to 2010.

The Study for Monitoring Antimicrobial Resistance Trends (SMART) is a worldwide surveillance program designed to longitudinally monitor the in vitro activity of antimicrobial agents against pathogens that cause intra-abdominal infections (IAIs). In this study, trends in antimicrobial resistance during the period 2006 to 2010 were analyzed at five tertiary-care hospitals in Taiwan. Enterobacteriaceae accounted for the majority (80.9%) of the 2417 Gram-negative isolates, and the two most common species were Escherichia coli (38.8%) and Klebsiella pneumoniae (23.5%). The rates of susceptibility o
