AIMS: International recommendations stipulate not performing screening skin tests to a drug in the absence of a clinical history consistent with that specific drug allergy. Nevertheless, two publications showed that a positive history of non-anaesthetic drug allergy was the only predictive factor for a positive skin test when screening for allergy to anaesthetic drugs was done. BACKGROUND: Corticosteroids, which are anti-inflammatory and immunosuppressive agents used for the treatment of various diseases including allergic disorders, can induce immediate and delayed hypersensitivity reactions. Although these reactions are not common, due to the wide usage of corticosteroid medications, corticosteroid hypersensitivity reactions are clinically important.
OBJECTIVE: In this review, we summarise the prevalence, pathogenetic mechanism, clinical manifestations, risk factors, diagnostic and therapeutic approach for corticosteroid-induced hypersensitivity reactions.

We have evaluated the in vitro leukocyte histamine release tests for the diagnosis of allergy to muscle relaxant drugs in 40 patients (Group A) and a control group of 44 subjects with negative leukocyte histamine release (Group B). Non-IgE dependent histamine release, expressed as a percentage of the total blood histamine, was 3.94% +/- 0.49 in Group B. The upper limit of positivity was estimated to be 5% (mean + 2 SD). The sixth case of an allergic contact gingivostomatitis to gold crowns previously misdiagnosed as erosive lichen planus is reported. Patch tests to gold chloride (0.5%) and 0.05 cm3 intradermal gold sodium thiomalate (Myochrysine 50 mg/cm3) were positive at 48-72 h. Histopathology of the allergic contact gingivitis revealed a pseudolymphomatous inflammatory dermal reaction with a dense lymphohistiocytic and plasma cell infiltrate. The precision of two intracutaneous skin test techniques was investigated by testing three grass pollen-sensitive patients. Weal and flare histamine dose-responses were measured in 30 normal subjects and 15 patients with chronic idiopathic urticaria. The log dose-response curves were shifted to the left corresponding to an increase in histamine potency of 2.5 for weal and 3.1 for flare. An enhanced response to histamine may be important in chronic idiopathic urticaria and comparison of potency of antihistamines in different subjects may be unreliable where initial weal response is abnormal.

Allergy to human seminal fluid is a rare but potentially life-threatening disorder. Acute symptoms range from local reactions to generalized symptoms. A 30-year-old woman experienced vulvovaginal itching, erythema and swelling as well as generalized urticaria and angioedema of the neck and face after unprotected sexual intercourse. A scratch test with undiluted seminal fluid and the detection of specific IgE antibodies revealed a type I allergy to seminal fluid. Risk factors and therapeutic options are discussed.

Apparently conflicting guinea pig sensitisation data on diacrylates and dimethacrylates, published by 3 groups of authors, are analysed in terms of the RAI model. This model, correlating sensitisation score with a combination of induction dose, chemical reactivity and lipophilicity, enables the major discrepancies to be rationalised. The data provide a striking example of the overload effect, in that some of the diacrylates have failed to show sensitisation when tested at high induction doses, but are revealed as strong sensitisers when tested by other authors at lower induction doses.
