A case of achalasia presented with cardiopulmonary arrest.

Achalasia is a rare disorder characterised by obstruction of the distal oesophagus and subsequent dilation of the proximal oesophagus. Patients generally complain of gastrointestinal symptoms; however, pulmonary symptoms and complications may also occur. A 35-year-old woman was brought to our emergency service complaining of sudden-onset dyspnea that started 15 minutes earlier during dinner. She suffered a cardiopulmonary arrest due to aspiration 5 minutes after being admitted to the emergency room and was intubated. Thoracic computed tomography examination showed that her oesophagus was fille

The esophageal obturator airway. A critique.

The esophageal obturator airway (EOA) has been used in cardiopulmonary resuscitation since 1973. The device is inserted into the esophagus and isolates the stomach from the airways by an occlusive balloon located below the tracheal bifurcation. It aims to prevent gastrointestinal inflation and regurgitation and to provide ventilation. A critique and literature review point out the speed and ease of insertion (5 s), short training period, and efficacy as it relates to blood gases and tidal volumes, which are comparable with those obtained with endotracheal intubation. Complications, including e

Neonatal Catheter and Tube Placement and Radiographic Assessment Statistics in Relation to Important Anatomic Landmarks.

OBJECTIVE: Neonatal catheters and tubes are commonly used for monitoring and support for intensive care and must be correctly positioned to avoid complications. Position assessment is routinely done by radiography. The objective of this study is to characterize neonatal catheter and tube placement in terms of the proportion of those devices that are malpositioned. STUDY DESIGN: Using an institutional dataset of 723 chest/abdominal radiographs of neonatal intensive care unit (ICU) patients (all within 60 days of birth), we assessed the proportion of catheters that are malpositioned. Many radio

An evaluation of the I-gel supraglottic airway in 70 pediatric patients.

The I-gel is a supraglottic airway device which is commonly used in adult patients undergoing general anesthesia. In this study, we evaluated the speed of insertion, adequacy of ventilation, leak pressure, gastric tube insertion, and problems when using the I-gel in children. We included 70 patients aged between 1 and 16 years old with ASA physical status classification I or II, undergoing elective surgery requiring general anesthesia, for which use of a supraglottic airway would be appropriate. The overall insertion success rate was 96 % with a median insertion time of 25 (18-34) [7-100] s. S

Capnometry and air insufflation for assessing initial placement of gastric tubes.

BACKGROUND: Nurses are often responsible for placement of large-bore gastric tubes. Tube misplacement into the lungs is a potential complication with serious sequelae. The reliability of common bedside methods for differentiating between pulmonary and gastric placement has not been acceptable. OBJECTIVE: To compare the accuracy of capnometry (colorimetric indicator of end-tidal carbon dioxide) and air insufflation/auscultation with the accuracy of radiography in detecting the location of gastric tubes. METHODS: A prospective convenience sample of insertions of Salem sump gastric tubes was st

Nasointestinal tube for decompression or enteroclysis: experience with 150 patients.

The initial clinical experience with the use of a triple lumen long tube designed for gastrointestinal decompression and enteroclysis is reported in 150 patients. Based on clinical observations, this tube is effective in suctioning retained gastric and intestinal fluid but requires frequent irrigation of the sump port for effective decompression of distended small bowel. In all patients with a preexisting nasogastric tube, the replacement by the decompression/enteroclysis tube was considered more comfortable by the patients. Successful placement of the tube in the jejunum was achieved in 147 o

The effect of stellate ganglion block on postoperative recovery quality in peri-menopausal women undergoing gynecological laparoscopic surgery.

OBJECTIVE: This study aims to observe whether right-sided stellate ganglion block (SGB) before surgery can improve early postoperative recovery quality in peri-menopausal women undergoing elective gynecological laparoscopic surgery. METHODS: Ninety-four peri-menopausal women, who meet the inclusion criteria, scheduled for gynecological laparoscopic surgery were selected (Trial Registration: ChiCTR2200057907, March 21, 2022). They were randomly assigned into two groups (Group S and Group C). Group S received right-sided SGB under ultrasound guidance, combined with tracheal intubation and gener

Delivery room management of extremely low birth weight infants: spontaneous breathing or intubation?

OBJECTIVE: To study the effect of two different delivery room (DR) policies on the rate of endotracheal intubation and mechanical ventilation (EI/MV) and short term morbidity in extremely low birth weight infants (ELBWI; <1000 g, >/=24 weeks). METHODS: Retrospective cohort study of 123 inborn ELBWIs born in 1994 and in 1996. DR policies have changed. Until 1994, ELBWIs were intubated immediately after delivery when presenting the slightest signs of respiratory distress or asphyxia after initial resuscitation using a face mask and a handbag. During 1995, the guidelines for respiratory support

Left atrio-esophageal fistula of a possibly iatrogenic aetiology.

The study presents an exceptionally rare case of an esophago-left atrial fistula, which was diagnosed during a forensic post-mortem examination. Due to complex nature of the disease and many attempts to cure the patient, the authors did not manage to identify the aetiology of the fistula. It was only implied that the fistula might have been a distant complication of intraoperative endocardial ablation or it might have appeared as a consequence of perforation of the esophageal wall or left atrial wall of the enlarged heart with the end of an intubation tube or nasogastric tube.

Type IV laryngo-tracheo-esophageal cleft with CPAM in a preterm twin- a case report.

BACKGROUND: Laryngo-tracheo-esophageal cleft (LTEC) is a rare congenital anomaly caused by incomplete separation of the foregut. Type IV defects are exceptionally rare and typically carry high mortality and morbidity despite surgical interventions. CASE PRESENTATION: We report a preterm neonate born at 34 + 1 weeks' gestation from a monochorionic diamniotic twin pregnancy. Antenatal ultrasound detected a large microcystic congenital pulmonary airway malformation (CPAM) with mediastinal shift, absent gastric bubble, and suspected esophageal pouch. Immediately upon birth, bedside ultrasound ide

Retrograde esophageal intubation.

Intrathoracic esophageal injury can be treated using conventional surgical techniques, including buttressed closure, T-tube drainage, resection, and exclusion with diversion. If esophageal replacement is necessary, stomach, left or right colon, and jejunum are commonly used conduits. Problems arise when esophageal drainage and diversion is necessary in patients with prior gastrectomy. This problem compounds when planning esophageal reconstruction in those with previous colectomy because preserving esophageal length is crucial. We developed the technique of retrograde esophageal intubation, and

Neutropenic colitis during standard dose combination chemotherapy with nedaplatin and irinotecan for testicular cancer.

A 54-year-old man received combination chemotherapy with nedaplatin and irinotecan as salvage chemotherapy for refractory non-seminomatous testicular cancer. The patient developed abdominal pain and high fever on Day 21 after the initiation of chemotherapy. Computed tomography revealed thickening of the terminal ileum wall and paralytic ileus. The patient recovered with intensive supportive management including broad-spectrum antibiotics, bowel rest with gastric intubation and intravenous gamma-globulin. Neutropenic colitis has been thought to be a serious gastrointestinal complication associa

Safety and utility of direct laryngoscopy and bronchoscopy in patients hospitalized with croup.

Acute croup is a common admitting diagnosis for pediatric patients. If a patient is not responding to medical management for presumed croup, the otolaryngology team is occasionally consulted for direct laryngoscopy and bronchoscopy (DLB) to rule out tracheitis or another airway pathology. We conducted a study to determine if inpatient DLB in acute croup is safe and efficacious and to correlate preoperative vital signs with intraoperative findings. We reviewed the charts of 521 patients with an admitting diagnosis of acute tracheitis, acute laryngotracheitis, or croup. Of this group, 18 patient

Arteriobiliary Fistula and Post-traumatic Pseudoaneurysm in the Right Hepatic Artery.

Hemobilia is a rare but potentially life-threatening cause of upper gastrointestinal bleeding, defined as hemorrhage into the biliary tree resulting from an abnormal communication between the hepatic vasculature and the biliary system. It is most commonly associated with hepatobiliary surgical interventions and vascular injuries. We report the case of a female in her 20s with no relevant medical history, who underwent an apparently uncomplicated laparoscopic cholecystectomy in September 2023 and subsequently presented in February 2024 with recurrent, intermittent episodes of hematemesis and he

Endoscopic Nd-YAG laser therapy for palliation of upper gastrointestinal malignancy.

Endoscopic laser therapy with the neodymium-yttrium-aluminium-garnet (Nd-YAG) laser has been shown to provide good palliation of upper gastrointestinal obstruction caused by malignancy, and to be associated with a low morbidity and a low mortality rate. Fifty patients with inoperable upper gastrointestinal malignancy have been treated with this method: 22 had oesophageal carcinoma, 16 adenocarcinoma at the cardio-oesophageal junction, two carcinoma of the antrum and 10 recurrent tumours at the site of previous anastomoses. The main symptoms were dysphagia in 40 and vomiting in seven; three oth

A case report of foreign body appendicitis caused by tongue piercing ingestion.

INTRODUCTION: Foreign body ingestion is an uncommon clinical problem in healthy adults. Furthermore, it is even less common for an ingested foreign body to cause any obstructive symptoms within the gastrointestinal tract. PRESENTATION OF CASE: Here, we describe an unusual case of acute appendicitis induced by a tongue piercing that was ingested by a 32-year-old woman with a recent history of endotracheal intubation. Abdominal X-ray revealed metallic foreign bodies in the right lower quadrant. The foreign bodies remained in place on serial X-rays despite bowel preparation and they were not vis

A novel reconstructive technique for pylorus-preserving pancreaticoduodenectomy: avoidance of early postoperative gastric stasis.

In 30 patients a reconstructive technique was used after pylorus-preserving pancreaticoduodenectomy in which the anastomoses were constructed in the sequence: duodenojejunal, hepaticojejunal (8-10 cm distal) and finally duct-to-mucosa pancreaticojejunal to a separate Roux loop. Indications for surgery included periampullary tumours, (n = 13), carcinoma of the head of the pancreas (n = 10) and chronic pancreatitis (n = 4). No patient required prolonged (> 7 days) nasogastric intubation for primary gastroparesis in the early postoperative period. Postoperative morbidity (17% overall) delayed rec

Nasogastric feeding tube placement in patients with esophageal cancer: application of ultrathin transnasal endoscopy.

BACKGROUND: Patients with esophageal cancer often present with dysphagia and malnutrition. Obstructive symptoms may improve after radiotherapy and chemotherapy. Nutrition support via a nasogastric tube (NG) or gastrostomy is very important during treatment. The newly developed ultrathin endoscope (Olympus, GIF-N230, outer diameter: 6 mm) has a smaller diameter than the standard endoscope and can be introduced into the esophagus via the nasal cavity. This article reports on the use of an ultrathin endoscope for NG placement for patients with esophageal cancer who presented with dysphagia and fa
