Postoperative haemorrhage following cleft surgery: A national survey of practice in Great Britain and Ireland.

Postoperative haemorrhage following cleft palate and pharyngeal surgery is an uncommon but potentially life-threatening complication due to the risk of airway compromise. Within centralised cleft service models, patients may present outside specialist centres, making clear escalation pathways particularly important. We conducted a cross-sectional survey of cleft surgeons in Great Britain and Ireland to describe current approaches to classification and management of postoperative bleeding. Twenty-three current cleft surgeons representing 12 of 13 cleft services responded. Four respondents (17%)…

Optimising survivorship care in Hodgkin's lymphoma: Aligning practice with Ireland's AYA cancer framework.

BACKGROUND: With increasing numbers of cancer survivors living well beyond treatment, national and international policy strongly emphasises self-management as a core component of survivorship care. Hodgkin's Lymphoma (HL) is one of the most common cancers in Adolescents and Young Adults (AYAs) in Ireland, with five-year overall survival exceeding 95%. However, survivors face a tenfold increased risk of death due to recurrence and late effects, which may emerge up to 20 years post-treatment, often after structured follow-up ends. Supporting self-management is therefore critical to long term hea…

Preserving the antimicrobial arsenal: exploring alternatives to carbapenems in ESBL battles within the southeast of Ireland.

Introduction. Carbapenems are usually employed as first-line antimicrobials against bacteria harbouring extended-spectrum beta-lactamases (ESBLs). These enzymes confer resistance often to multiple classes of antimicrobials.Hypothesis/Gap Statement. This indiscriminate use of carbapenems and the inevitable development of carbapenem resistance have prompted the need for carbapenem-sparing strategies.Methodology. The non-carbapenem antimicrobial susceptibility patterns of 60 ESBL-producing Enterobacterales (ESBL-PE) isolates responsible for bloodstream infections, in 2022-2023 inclusive, processe

Bilateral Cleft Lip and Palate Care in the United Kingdom and Ireland: Is There Consensus on Surgical Management?

Since the centralization of cleft care in the United Kingdom and Ireland between 1998 and 2007, there is now greater opportunity for close communication between cleft surgeons and for comparison of surgical protocols. British and Irish surgeons were surveyed in both 2009 and 2019 to gather data regarding their surgical approaches to patients with bilateral cleft lip and palate (BCLP) and the use of presurgical orthopedics (PSO) in this cohort. We present this data as a snapshot of BCLP care in the United Kingdom and Ireland and analyze it to determine whether there is any consensus on the surg…

'Brought in Dead': Post-Mortem Glimpses of the Early 'Heroin Epidemic' in Ireland, 1971-1983.

This article explores the formation of Ireland's first 'hard drug' culture. To do this, it uses the coroners' reports on drug-related deaths in Dublin between 1971, when the first overdose by a regular user was recorded, and 1983, when the first Irish 'heroin epidemic' peaked. Through these reports, the article constructs a macro-view of the demographics involved in 'hard drug' use and the changing trends within the subculture. It contrasts this overview with the lived experience of the drug culture by developing a series of micro-histories of specific people who used drugs during this period,…

Investigation of an outbreak of novel hepatitis of unknown aetiology in children and adolescents, Ireland, 2021 to 2023.

An outbreak of severe acute hepatitis of unknown aetiology in children (HUAC) was reported by the United Kingdom (UK) in spring 2022. Within days, a corresponding increase was identified in Ireland. A multi-agency incident management team (IMT), led by the Health Protection Surveillance Centre (HPSC), established a national case definition, trawling questionnaire, testing protocol and communications plan. Between 1 October 2021 and 12 May 2023, 44 probable and three possible cases of HUAC were identified in Ireland with a median age of 3 years. Adeno-associated virus 2 (AAV2), detected in 18 o…

How are HIV services in the UK and Ireland managing care home residents living with HIV?

PURPOSE: The average age of people living with HIV in Europe is rising and with it the likelihood of entering long-term care. Little is known about the characteristics of people living with HIV in care homes and how their HIV care is delivered. METHODS: We conducted a multi-site, cross-sectional descriptive evaluation of care home residents receiving HIV care at seven centres in the UK and Ireland. Data included demographics, HIV history, comorbidities, medication use and HIV service contacts over the preceding 12 months. RESULTS: The sample included 62 residents with a median age of 61 year…

Surveillance for Leptospira sp. in Native and Invasive Shrews in Ireland Compared with Cohabitating Shrews in France.

Leptospirosis is a complex and often underestimated global bacterial disease that continues to be of zoonotic concern. It is difficult to diagnose and has an unclear pathogenesis. Several new species of the genus Leptospira have been discovered in recent years; the impact of these species on animal health is unknown. In 2013, Leptospira tipperaryensis was first identified in greater white-toothed shrews (GWTSs, Crocidura russula) in Ireland, where they are an invasive species that displaces the native pygmy shrew (Sorex minutus). This study investigated the prevalence of Leptospira spp., inclu…
