For patients with HIV/AIDS, highly active antiretroviral therapy (HAART) is currently the only effective therapy for progressive multifocal leukoencephalopathy (PML), a viral-induced demyelinating disease caused by polyomavirus JC. Immune reconstitution inflammatory syndrome (IRIS) following initiation of HAART can cause paradoxical clinical deterioration in patients with established PML. BACKGROUND: We report 2 cases in which ultrasound biomicroscopy (UBM) was used to diagnose Ahmed valve obstruction by the iris.
METHODS: Two patients underwent unilateral placement of Ahmed implants, one into the anterior chamber and the other into the ciliary sulcus.
RESULTS: Intraocular pressures increased in the early postoperative period. In each case, a clear view of the aqueous drainage tube was precluded. Occlusion of the end of the tube by iris tissue was demonstrated by UBM. An iridectomy over the tip of the tube instituted filtration in each case.

The possibility of an acute increase in intraocular pressure (IOP) following laser iridotomy is well known. However, little attention has been paid to the hypotony after laser irradiation of the iris. We performed this study to evaluate the effect of laser irradiation of the iris on the IOP in pigmented rabbits. Pigmentation is reported to affect cisplatin-induced ototoxicity in adult humans. The hearing loss is worse in people with brown irises, than in those with blue irises. We assessed the hypothesis that cisplatin-treated children with dark irises suffer more deterioration in auditory thresholds than do those with less pigmentation. The content of lactic acid and proteins in the aqueous humour of eyes with neovascularization of the iris (NVI) of various etiology was determined. The results were analysed for any possible correlation with the angiogenesis observed in such eyes. Aqueous humour from patients with immature senile cataracts served as the control. This study examined malignant melanoma of the iris in Denmark over a 25-year period on the basis of the files of the Eye Pathology Institute. From 1961 to 1985 a total of 80 cases were found (41 males, 39 females; median age 53 years, range 8-83). This means an average of 3.2 cases/year with an average population of 4.9 million. Iris specimens obtained during the peripheral iridectomy in 12 cases (18 eyes) of primary angle closure glaucoma were studied histologically. The iris specimens taken during the senile cataract extraction in otherwise normal eyes were used as controls. The remarkable differences of the two groups are the changes of the quality and quantity of the mitochondria in the cells of iris tissue. The changes in primary glaucoma, especially in the intermittent stage of the angle closure glaucoma, were found in the mitochondria of the cells.
