Temporomandibular joint dysfunction syndrome: a close association with systemic joint laxity (the hypermobile joint syndrome).

Sixty-two patients admitted for elective reconstructive surgery of the temporomandibular joint (TMJ) and eight seen as outpatients with a chief complaint of TMJ dysfunction during the same time interval were evaluated for possible etiologic factors contributing to the disease. All hospitalized patients had severe, end-stage degenerative changes within the TMJ, whereas outpatients had less severe disease and did not require surgery. TMJ dysfunction in some patients was said to be a result of established causes including bruxism, malocclusion, and trauma. No patient in this series had evidence o

Editorial Commentary: Cartilage Restoration Is Joint Preservation, and Joint Preservation Requires Treating the Whole Joint.

Focal articular cartilage lesions of the knee rarely occur in isolation. Malalignment, meniscal deficiency, ligamentous instability, and patellofemoral maltracking frequently coexist, creating the mechanical environment in which cartilage restoration either succeeds or fails. In our view, the most important conceptual shift in cartilage surgery over the past decade has been the reframing of concomitant procedures (osteotomy, meniscal allograft transplantation, ligament reconstruction, and tibial tubercle osteotomy) from add-ons into graft-protecting interventions that address the drivers of th

Provocative sacroiliac joint maneuvers and sacroiliac joint block are unreliable for diagnosing sacroiliac joint pain.

Mapping studies of pain elicited by injections into the sacroiliac joints (SIJs) suggest that sacroiliac joint syndrome (SIJS) may manifest as low back pain, sciatica, or trochanteric pain. Neither patient-reported symptoms nor provocative SIJ maneuvers are sensitive or specific for SIJS when SIJ block is used as the diagnostic gold standard. This has led to increasing diagnostic use of SIJ block, a procedure in which an anesthetic is injected into the joint under arthrographic guidance. However, several arguments cast doubt on the validity of SIJ block as a diagnostic gold standard. Thus, the…

Joint coordinate system for biomechanical analysis of the sacroiliac joint.

Sacroiliac joint (SIJ) biomechanics have been described in both in vitro and in vivo studies. A standard for joint coordinate systems has been created by the International Society of Biomechanics for most of the joints in the human body. However, a standardized joint coordinate system for sacroiliac joint motion analysis is currently still lacking. This impedes the comparison across studies and hinders communication among scientists and clinicians. As SIJ motion is reported to be quite limited, a proper standardization and reproducibility of this procedure is essential for the interpretation o…

Joint-preservation surgery for pediatric osteosarcoma of the knee joint.

The multi-disciplinary approach involving imaging, multi-agent chemotherapy, meticulous surgical procedures, and careful postoperative care has facilitated an increase in the use of limb-sparing surgery for pediatric osteosarcoma. Osteosarcoma usually occurs around the metaphysis of the distal femur or proximal tibia and needs wide excision with the adjacent joint and replacement by a megaprosthesis. The recent advancement in imaging modalities and surgical techniques supports joint-preservation surgery (JPS), involving the preservation of the adjacent epiphysis, for select patients following

Temporomandibular joint reconstruction with total alloplastic joint replacement.

This paper is a preliminary paper which presents the early findings of an ongoing prospective trial on the use of the TMJ Concepts and Biome … eplacements using either the TMJ Concepts system or the Biomet Lorenz joint system. Two patients (3 joints) had the TMJ Concepts system and five patients (9 joints) had the Biomet Lorenz system. Although still early, the results were generally pleasing, with the longest replacement having been in position for three years and the most recent six months. T …

Joint replacement in Malawi: establishment of a National Joint Registry.

In Africa the amount of joint replacement surgery is increasing, but the indications for operation and the age of the patients are considerably different from those in the developed world. New centres with variable standards of care and training of the surgeons are performing these procedures and it is important that a proper audit of this work is undertaken. In Malawi, we have pioneered a Registry which includes all joint replacements that have been carried out in the country. The data gathered include the age, gender, indication for operation, the prosthesis used, the surgical approach, the

Stability of the distal radioulnar joint contributed by the joint capsule.

PURPOSE: The distal radioulnar joint (DRUJ) capsule is assumed to be an important stabilizer of the DRUJ. There are few published data regarding the capsule and its specific contribution to stability of the DRUJ. METHODS: We analyzed the contribution of the joint capsule to DRUJ stability in a biomechanical study consisting of collection of force/displacement data from 16 adult human cadaveric upper extremities. Each specimen was subjected to anteroposterior translation testing of the radius relative to the ulna in 3 positions of forearm rotation (neutral, 60 degrees pronation, 60 degrees sup

Position of the knee joint after total joint arthroplasty.

A method for assessing knee joint position after surgery using the preoperative long-leg radiograph and the postoperative knee radiograph is described. Assessment of the formula has shown a near perfect correlation between the calculated position on the long-leg radiograph compared with the measured position for 44 knees. Three hundred eighteen knees after total joint arthroplasty were retrospectively reviewed and the postoperative position was determined. The preoperative position of the mechanical axis was 14.5 +/- 37.3 mm medial to the knee joint center. Using the standing knee radiograph t

Radioulnar joint fusion for distal radioulnar joint instability.

Each one of this small group of patients illustrates a rare indication for the radioulnar joint fusion procedure in distal radioulnar joint instability. The case histories tell of a last ditch effort to salvage function in an extremity crippled by painful radioulnar instability after excision of the distal ulna. The fusions healed slowly and two required repeat surgery to achieve union. Today we would routinely add iliac bone graft to the fusion area to hasten healing. Rarely indicated, this is a salvage procedure that is done after failure of other procedures geared to preserve the rotation o

Joint-on-a-Chip: The Future of Preclinical Modelling of Joint Diseases.

Joint diseases, including osteoarthritis (OA) and rheumatoid arthritis (RA), are leading causes of disability worldwide and pose a major socioeconomic burden. Effective treatments remain limited, partly due to difficulties in understanding and replicating complex joint structures as well as the dynamic interactions among multiple tissues and immune components. Traditional in vitro and animal models are restricted in their ability to accurately simulate the human joint microenvironment, limiting their predictive value for drug discovery and therapeutic development. Joint-on-a-Chip (JOC) is emer

Thumb Interphalangeal Joint Replacement as an Alternative to Joint Fusion.

Arthrodesis is the generally accepted treatment for symptomatic osteoarthritis of the interphalangeal (IP) joint of the thumb. For 7 thumbs in 5 patients, an IP joint replacement was offered as a pain alleviating and motion conserving treatment option for osteoarthritis. In these cases, an offlabel use of the SR-PIP prosthesis was performed. All patients did well postoperatively with reasonable pain-free range of motion of the interphalangeal joint except one patient who required further surgeries for recurring joint instability. Radiologically, though, an osteolysis around the stems that coul
