Annular Salzmann degeneration: Avoiding perturbations and pitfalls in phacoemulsification surgery.

UNLABELLED: We highlight potential problems and pitfalls in cases of Salzmann nodular degeneration by reporting the case of an 84-year-old man with reduced visual acuity due to particularly extensive bilateral Salzmann nodules and moderate cataracts. Large annular Salzmann lesions were removed by superficial keratectomy in each eye. The reproducibility and accuracy of keratometry and biometry improved significantly, and uneventful phacoemulsification cataract surgery was performed 4 weeks after the keratectomy. The phacoemulsification procedures were routine other than the development of prono…

Corneal iron lines after refractive keratoplasty.

Twenty-three of 55 patients who underwent refractive keratoplasty for the correction of aphakia were found to have a pigmented corneal epithelial line similar to the Fleischer ring. In 21 of 23 cases, the pigmented ring was adjacent to the superficial keratectomy scar that resulted from the keratorefractive operation. In two cases, the pigmented line was located central to the lamellar keratectomy scar. Histopathologic study demonstrated that the line was due to iron in the form of ferritin particles deposited within basal corneal epithelial cells. We believe that the steep anterior corneal cu…

LASIK and surface ablation in corneal dystrophies.

Corneal dystrophies are a rare group of hereditary disorders, that are bilateral, non-inflammatory, and progressive. Clinically, they can be classified based on the anatomic layer of the cornea affected. Refractive surgery and phototherapeutic keratectomy (PTK) can be performed with caution in patients with certain corneal dystrophies, but should be avoided in others. For epithelial basement membrane dystrophy, photorefractive keratectomy (PRK) is the procedure of choice for treatment of refractive error, and PTK may be performed for the treatment of recurrent erosions or irregular astigmatism…

Diagnosis and management of postrefractive surgery ectasia.

Postrefractive surgery ectasia is a serious, sight-threatening complication seen after the following procedures: laser in situ keratomileusis, photorefractive keratectomy, small-incision lenticule extraction, radial keratotomy, and/or arcuate keratotomy. Specific risk factors may include age, corneal thickness, degree of refractive error, corneal topographic changes including irregular astigmatism, percent tissue ablation, and residual stromal bed. Biomarkers may be a new option to help indicate who is at greatest risk for ectasia. Visual aids including spectacles or contacts lenses are often …
