Primary leiomyosarcoma of the bronchus is an extremely rare malignant neoplasm. A 29-year-old man presented with persisting cough and sputum over 1 month. Unenhanced and contrast-enhanced chest CT showed a nodular lesion in the main bronchus of the left lower lobe and multiple enlarged lymph nodes in the mediastinum. Preoperative staging F-FDG PET/CT scan displayed a hypermetabolic lesion of the known nodule and mild FDG uptake of mediastinal lymph nodes. The histological examination confirmed bronchial leiomyosarcoma and reactive hyperplasia in lymph nodes.

Primary leiomyosarcoma of the orbit is extremely rare. Here we report the case of a 32-year-old woman who presented with proptosis and diplopia of the left eye. MRI examination showed a solid mass in the left orbit with invasion to adjacent tissue, and the lesion had intense FDG uptake with SUVmax of 18.7 on F-FDG PET/CT. Primary orbit leiomyosarcoma was diagnosed by surgery-pathology. F-FDG PET/CT has an advantage in excluding secondary or metastatic orbit malignancy.

Primary spermatic cord leiomyosarcoma is a rare malignancy. We describe FDG PET/CT findings in a case of primary high-grade leiomyosarcoma of the spermatic cord. It appeared as a soft tissue mass with heterogeneous hyperenhancement on MRI and focal activity with SUV max of 4.8 on FDG PET/CT. This case indicates that spermatic cord leiomyosarcoma, although rare, should be included in the list of differential diagnoses of hypermetabolic spermatic cord lesions.

PURPOSE: Leiomyosarcoma (LMS) is an aggressive tumor with a high metastatic rate that rarely metastasizes to the periocular region.
METHODS: A 50-year-old male with a previous two-year history of primary stage IV LMS presented with metastatic retroperitoneal LMS, which was initially incorrectly described as an eyelid chalazion refractory to medical management. An excisional biopsy sent to pathology revealed metastatic retroperitoneum LMS. There was resolution of ocular irritation following biopsy, and an oncology referral was made.
