DARK Classics in Chemical Neuroscience: Bucinnazine.

Bucinnazine (1-butyryl-4-cinnamylpiperazine) is a synthetic opioid recently discovered in heroin seized samples in the U.S and in Europe. It was first synthesized in the late 1960s and has been used for the treatment of cancer-associated chronic pain in China for many years. Bucinnazine is one of the most potent compounds among the series of piperazines, which also include other relevant compounds, such as MT-45, AD-1211, and 2-methyl-AP-237, a methylated derivative of bucinnazine. Bucinnazine is considered a μ-selective opioid, binding primarily to the μ-opioid receptor. However, bucinnazine

Assisted reproductive technologies and metabolic syndrome complications: medico-legal reappraisal.

In the last 40 years, the number of elderly patients that require Assisted Reproductive Technologies (ART) has risen enormously, especially after heterolougus fertilization techniques have become available. In recent years, the incidence of peripartum cardiomyopathy (PPCM) has substantially grown, as a consequence of the combined effect of increased maternal age, consequent high prevalence of hypertension and metabolic syndrome (MS). That cohort of women may be exposed to a greater number of cardiac, obstetric and anesthesio-logical complications, therefore the incidence of medico-legal issues

What Adverse Events and Injuries Are Cited in Anesthesia Malpractice Claims for Nonspine Orthopaedic Surgery?

BACKGROUND: Malpractice claims that arise during the perioperative care of patients receiving orthopaedic procedures will frequently involve both orthopaedic surgeons and anesthesiologists. The Anesthesia Closed Claims database contains anesthesia malpractice claim data that can be used to investigate patient safety events arising during the care of orthopaedic patients and can provide insight into the medicolegal liability shared by the two specialties. QUESTIONS/PURPOSES: (1) How do orthopaedic anesthetic malpractice claims differ from other anesthesia claims with regard to patient and case

Authority Without Infrastructure: A Researcher's Experience With Specimen Access Barriers in Sudden Unexpected Infant Death Research.

An infant death that remains unexplained after a comprehensive medicolegal investigation does not establish the absence of an underlying biological mechanism. Investigation of a specific research hypothesis may nevertheless be limited when suitable specimens or preservation records are unavailable. This editorial describes non-systematic exploratory outreach undertaken during development of a proposed National Institutes of Health (NIH) R21 study and considers relevant legal provisions in Florida and California. The outreach identified concerns involving staffing, workflow, authorization, liab

Preventable yet persistent: silicosis and gaps in public health policy - a narrative review.

Silicosis remains a critical, 100% preventable occupational lung disease that continues to persist as a global public health crisis in the 21st century. Despite decades of industrial safety knowledge, the disease is currently seeing a modern resurgence, particularly in the engineered stone industry, where high-intensity silica exposure leads to rapid, fatal disease variants. This review synthesizes current global and national evidence to highlight the staggering burden of silicosis, with a specific focus on India, where over 50 million workers are projected to be at risk by 2026. Epidemiologic

Navigating Chinese-led economic integration: a comparative study of commercial dispute resolution and public policy exceptions in Cambodia, Pakistan and Thailand.

The rapid expansion of Chinese-led economic integration under the Belt and Road Initiative (BRI) has intensified pressure on domestic dispute resolution systems across emerging Asian economies. While international commercial arbitration offers a neutral risk-management mechanism, its effectiveness relies entirely on enforcement by national courts under the 1958 New York Convention. This article investigates the escalating friction between transnational arbitral norms and host-state sovereignty through a comparative doctrinal analysis of Cambodia, Pakistan, and Thailand. Analyzing landmark disp

Allegations of Failure to Obtain Informed Consent in Otolaryngology: Evidenced-Based Recommendations for Sinus Surgeons.

BACKGROUND: Informed consent requires preoperative discussion of surgical risks, complications, and alternative treatment options. Allegations of incomplete informed consent are common in the field of otolaryngology. OBJECTIVES: Analyze outcomes and case variables in cases of alleged informed consent failure involving otolaryngologists. METHODS: A legal research database containing state and federal case records from across the United States was retrospectively reviewed for malpractice claims involving informed consent and otolaryngology. RESULTS: Among the 128 informed consent cases identi

Restraints and seclusion in psychiatry: striking a balance between protection and coercion. Critical overview of international regulations and rulings.

Restraint and seclusion (R&S) measures in psychiatric settings are applied worldwide, despite poor scientific evidence to back up their effectiveness. The medical, ethical and medico-legal implications of coercive interventions are broad-ranging and multifaceted. The review aims to shed a light on the most relevant and meaningful standards that have been laid out by international treaties, supranational institutions (United Nations, Council of Europe, World Health Organization), scientific institutions (American Medical Association, Australian Department of Health), legislative bodies and cour

Physical Therapists' Views and Experiences of Pregnancy-Related Low Back Pain and the Role of Acupuncture: Qualitative Exploration.

BACKGROUND: Low back pain is often accepted as a "normal" part of pregnancy. Despite research suggesting that quality of life for women who are pregnant is adversely affected, most are advised to self-manage. Although the use of acupuncture for the management of persistent nonspecific low back pain has been recommended in recent UK national guidelines, its use in the management of pregnancy-related low back pain remains limited. OBJECTIVES: This study aimed to explore the perceptions and experiences of physical therapists involved in treating women who are pregnant and have low back pain with

Examination of factors determining fault in two-vehicle motorcycle crashes.

Motorcycle crashes frequently involve a combination of high-risk behaviors by the motorcyclist or the other crash-involved driver. Such behaviors may include riding or driving without appropriate licensure or while under the influence of alcohol, as well as deciding not to use a safety device such as a helmet or safety belt. Given that these factors frequently occur in combination with one another, it is difficult to untangle the specific effects of individual factors leading up to the crash outcome. This study assesses how various rider-, driver-, and other crash-specific factors contribute t

[On the psychiatric examination of fire-raisers].

By far not every fire-raiser suffers from so-called pyromania, i.e. a pathological urge to raise fire. It is indispensable to differentiate between extrinsic and intrinsic motivation, whenever a person is accused of arson. The former aims at external gain, such as the sum insured, while the latter tends towards manipulation of internal states. The leading motive may be frustration, fascination, or in rare cases even sexual arousal. The concept of pyromania, as established by French psychiatrist Esquirol, is not a validated one in all parts; today's diagnostic manuals take "pyromania" rather as

Clinical, ethical, and medical legal considerations on emergency contraception.

PURPOSE OF INVESTIGATION: To evaluate how many women required the so-called "emergency contraception" at our outpatient service and what the actual role is of this kind of pharmacological administration in interfering with ovulation and pregnancy, paying particular attention to the ethical and medico-legal aspects of this subject. METHODS: During the period from 1 December 1998 to 30 November 2003, emergency contraception was prescribed to a total of 1,160 women. With regard to the contraceptives used, in most cases (1,132, 97.6%) a combined oral estrogen-progestogen pill (ethinyloestradiol 0

Legal regulation mechanisms in the control of medicines.

In 2002, a case was brought against the Secretary of State--sued on behalf of the Committee for the Safety of Medicine (CSM)--by a mother who in May 1986 gave aspirin to her 6-year-old daughter who was suffering from chickenpox (Smith vs. Secretary of State for Health, [2002]). The child deteriorated and following admission to hospital she was diagnosed as suffering from Reye's syndrome. As a consequence, the girl was left with a serious neurological handicap and was totally dependent on others for her care. In June 1986, the government issued a general public warning advising parents not to g

[Traumatic lesions of the pregnant uterus and its contents: medico-legal aspects].

Pregnant women are not free of the risk of trauma and any abnormality of pregnancy or the child is easily attributed to an accident occurring during the preceding days, weeks or ... months. This study reviews the traumatic complication of pregnancy and methods which can be used to attribute pathological events to the accident deemed responsible. Particularly stressed is the essential need for a meticulous and accurate approach when drafting the initial certificate, the only document available to an expert medical witness called upon to assist the judicial process in case of litigation.

Foregoing prehospital care: should ambulance staff always resuscitate?

Approximately 400,000 people die outside US hospitals or chronic care facilities each year. While there has been some recent movement towards initiating procedures for prehospital Do Not Resuscitate (DNR) orders, the most common situation in the US is that emergency medical systems (EMS) personnel are not authorized to pronounce patients dead, but are required to attempt resuscitation with all of the modalities at their disposal in virtually all patients. It is unfair and probably unrealistic for EMS personnel to have to make a determination of the validity of a non-standard prehospital DNR or

Situational status and moral incoherence in UK bio-law.

UK bio-law is praised for its pragmatic governance of ethically contested practices, especially embryo research, where the post-Warnock settlement regulates conduct through licensing, oversight, and the 14-day or primitive streak rule without resolving moral-status disputes. This article argues that this pragmatism exposes a structural incoherence, partly narrowed but not resolved by abortion-law reform. Embryo research law treats the embryo as a procedurally governed object, while abortion law has historically operated through a criminal-exception model under the Offences Against the Person A

Current status and future directions of AI in prostate cancer detection on MRI: a special report from the ESUR prostate MRI working group authors.

This report from the ESUR Prostate MRI Working Group assesses the current role of artificial intelligence (AI) in MRI-based detection of prostate cancer. While deep learning tools demonstrate high technical accuracy, the report emphasizes a significant gap between research achievements and actual clinical application. Key statements include promoting a "human-in-the-loop" approach, where AI supports rather than replaces radiological expertise, to reduce risks such as automation bias and legal liability. There is an urgent need for prospective validation across multiple vendors and for implemen

Defensive medicine practices as a result of malpractice claims and workplace physical violence: a cross-sectional study from Egypt.

Defensive medicine refers to practices motivated mainly by legal rather than medical purposes. Increased healthcare costs, overutilization of medical services, and potential harm to patients from unnecessary procedures are among its drawbacks. We performed this study to assess the prevalence of defensive medicine practices in Egypt and their associations with experiencing malpractice claims and workplace physical violence. We investigated 1797 physicians (68.1% male), with an average age of 36.8 ± 9.1 years, practicing in Egypt between January 14th and February 23rd, 2023. SPSS was used for st
