Higher everyday functioning, lower quality of life, and home care are associated with persisting symptoms after a COVID-19 infection in German care recipients.

INTRODUCTION: To date, risk factors for persisting symptoms after a COVID-19 infection have not been investigated in people needing care or support. Prior meta-analyses identified age, obesity, and female sex as risk factors for persisting symptoms after a COVID-19 infection in the general population. METHODS: This study is part of the Bavarian ambulatory COVID-19 monitor. Data were collected from ambulatory patients needing care/support and a past COVID-19 infection. Different exposure measures (age, sex, body-mass index, income, packyears, smoker status, relationship status, type of care, c

"It's the fear of the unknown" clinicians' perceptions on paediatric compassionate transport to preferred place of death: A qualitative descriptive study.

BACKGROUND: Compassionate transport is used to transfer dying neonates and children from hospital wards to their preferred place for death, such as home. For families, choice of where their child will die is important and meaningful. Yet the child's physiological instability, care complexity, and the lack of comprehensive guidelines impede wider use of compassionate transport. OBJECTIVES: The aim of this study was to identify clinicians' perceptions of enablers and barriers to transferring children receiving life-sustaining therapies from the paediatric or neonatal intensive care unit to thei

Exploring barriers and facilitators to shared decision-making for older adults in East Asian health systems: a scoping review.

BACKGROUND AND OBJECTIVES: Shared decision-making (SDM) is a core component of patient-centered care, yet evidence on SDM among older adults is largely derived from Western healthcare systems. Given sociocultural differences in East Asian contexts, particularly regarding family involvement, the transferability of Western SDM models remains uncertain. This study aimed to identify barriers and facilitators to SDM for older adults in East Asian healthcare systems across stakeholder groups. METHODS: A scoping review was conducted following the Arksey and O'Malley framework and PRISMA-ScR guidelin

Advance Care Planning and Care Coordination for People With Parkinson's Disease and Their Family Caregivers-Study Protocol for a Multicentre, Randomized Controlled Trial.

Background: Parkinson's disease (PD) is a progressive neurodegenerative disease with motor- and non-motor symptoms. When the disease progresses, symptom burden increases. Consequently, additional care demands develop, the complexity of treatment increases, and the patient's quality of life is progressively threatened. To address these challenges, there is growing awareness of the potential benefits of palliative care for people with PD. This includes communication about end-of-life issues, such as Advance Care Planning (ACP), which helps to elicit patient's needs and preferences on issues rela

Provision of palliative and end-of-life care by community health nurses: A scoping review on the challenges and potential solutions.

BACKGROUND: The demand for home-based palliative care is increasing, driven by ageing populations and a rise in multiple long-term conditions. Community health nurses are pivotal to enabling people to be cared for and die at home, yet their contributions to palliative and end-of-life care remain under-recognised and poorly understood. Previous international reviews have focused on specific themes but in-depth synthesis of evidence from one national health system has not been undertaken. AIM: This scoping review aimed to identify, map and synthesise evidence within a single national health sys

Towards a Better Understanding of the Lived Experience of Carers of People With Bipolar Disorder: A Mixed-Methods Study.

BACKGROUND: Caring for someone with Bipolar disorder (BD) is unique, and the severity and complexity of symptoms can overwhelm informal carers such as family, friends, and partners. AIM: This study employed a concurrent mixed-methods model to investigate the lived experience of informal carers caring for those with BD in Australia. METHODS: Participants (n = 46) answered an online survey containing standardised measures of quality of life and coping mechanisms as well as open-ended questions about their caring role challenges and support. RESULTS: Carers reported mental health problems as t

Integrating end-of-life care into intensive care practice: Outcomes following Gold Standards Framework accreditation.

In 2023, the Intensive Care Unit at the Dudley Group NHS Foundation Trust became the first in the UK to achieve Gold Standards Framework (GSF) accreditation. This study evaluates the impact of GSF implementation on end-of-life care delivery and associated metrics. Coding of end-of-life care increased from 10.7% to 16.1% and the proportion of deaths recognised as GSF Amber/Red rose from 14.5% to 58.1% with an increased number of referrals to Specialist Palliative Care. GSF accreditation was associated with sustained improvements in key end-of-life care metrics in intensive care, enhancing colla

Exercise therapy and self-management support for individuals with multimorbidity: a randomized and controlled trial.

Despite increasing individual and societal burden, evidence for effective management strategies of multimorbidity is missing. Exercise therapy and self-management support are promising interventions, but their effect has not been evaluated. We hypothesized that exercise therapy and self-management support were superior to usual care alone in improving health-related quality of life (HRQoL) in individuals with multimorbidity. In this pragmatic multicenter, assessor-blinded randomized controlled trial (MOBILIZE), we enrolled 228 adult patients with two or more selected long-term conditions that

Measuring End-of-Life Care: Development of a National Questionnaire for Bereaved Family Members.

BACKGROUND: Evaluating end-of-life care quality remains a central challenge in health systems facing population ageing and complex chronic disease trajectories. Given the clinical challenges of direct data collection at the end-of-life, bereaved family members act as critical proxies, providing data on the care delivered in the final months of life. However, most available instruments lack the contextual sensitivity required for national-level epidemiological surveillance. OBJECTIVES: This study aims to develop and establish the face validity of a national questionnaire to evaluate the health

Cost-Effectiveness of Virtual Health Coaching for Caregivers of Patients With Heart Failure: Results From a Randomized Controlled Trial.

BACKGROUND: Informal caregivers play an important role in heart failure care. Health coaching can effectively help informal caregivers adopt healthy lifestyles to prevent and control diseases. However, the cost-effectiveness of interventions that support them remains unclear. We compare the cost-effectiveness of a virtual health coaching intervention (Virtual Caregiver Coach for You) designed to support caregivers in managing their stress by promoting self-care practices versus health information alone for informal caregivers of patients with heart failure. METHODS: In this randomized control

Impact of palliative care at end-of-life Covid-19 patients - a small-scale pioneering experience.

BACKGROUND: In March 2020, the outbreak caused by the SARS-CoV-2 virus was declared a pandemic, resulting in numerous fatalities worldwide. To effectively combat the virus, it would be beneficial to involve professionals who specialize in symptom control for advanced illnesses, working closely with other specialties throughout the illness process. This approach can help manage a range of symptoms, from mild to severe and potentially life-threatening. No studies have been conducted in Portugal to analyse the intervention of Palliative Medicine at the end of life of Covid-19 patients and how it

Supportive and palliative care of adults with respiratory problems experiencing structural vulnerability from homelessness, prison or other criminal justice system involvement.

PURPOSE OF REVIEW: This review seeks to identify the current prevalence of potentially life-limiting respiratory conditions among those who have experienced homelessness, incarceration or had criminal justice involvement, and current developments in, and barriers to, delivery of supportive and palliative respiratory care to these populations. These structurally vulnerable populations are known to be growing, their health behaviours more risky, and their morbidity and mortality higher, with evidence of accelerated ageing. RECENT FINDINGS: Most studies identified investigated prevalence of resp

Illness Experiences in Young and Middle-Aged Patients With Dilated Cardiomyopathy on Chinese Social Media: Qualitative Study.

BACKGROUND: Dilated cardiomyopathy (DCM), characterized by ventricular dilation and systolic dysfunction, has a 10-year survival rate of 25%. The number of young and middle-aged patients with DCM is increasing, with progressive physical limitations and psychosocial distress substantially impairing quality of life. However, illness experiences in this population remain underexplored, particularly in non-Western contexts. Social media platforms such as Zhihu and Weibo offer a novel avenue for exploring patient narratives and peer support. OBJECTIVE: A qualitative descriptive study was conducted
