Steroid-responsive encephalopathy is a general term for diseases that are characterized by diffuse brain injury and respond well to corticosteroids or immunosuppressive agents, including Hashimoto's encephalopathy (HE), limbic encephalitis (LE), systemic lupus erythematosus encephalopathy (SLEE), antineutrophil cytoplasmic antibodies (ANCA)-associated systemic vasculitis encephalopathy (AASV), viral encephalitis (VE), and primary central nervous system lymphoma (PCNSL). Epilepsy and status epilepticus are the main manifestations of steroid-responsive encephalopathy. The spectrum of "autoimmune…

Varieties of autoantibodies are known to relate to autoimmune neurological disorders as the diagnostic and therapeutic markers. Some of them affected directly to the pathomechanisms of neurological diseases. Recently several autoantibodies with such roles have been reported showing the common characters as recognizing cell surface antigens. Among them, anti-aquaporin 4 antibody (AQP4-Ab) in neuromyelitis optica (NMO) and anti-NMDA receptor antibody (NMDAR-Ab) in non-herpetic limbic encephalitis are drawn considerable attention. The features of NMO with AQP4-Ab are as higher age at onset, extre…

BACKGROUND AND OBJECTIVES: Seronegative autoimmune encephalitis (AE), defined by an appropriate clinical phenotype in the absence of known n … ied prognostic markers.
RESULTS: Of 224 patients (median age 49 years, 50.9% male), 72 (32.1%) had limbic encephalitis (LE) and 152 (67.9%) had antibody-negative probable AE (ANPRA). Good outcome (mRS score ≤2) was more common in LE (49/72, 68.1%) than in ANPRA (76/154, 50.0%) (p < 0.05). Delayed immunotherapy was associated with an increased risk of poor outcome. A …

OBJECTIVE: This study aimed to explore the clinical profiles of Omani patients with true-positive autoimmune encephalitis (AE) autoantibodies and compare them with those having false-positive autoantibodies. The incidence of AE is rising due to growing awareness and the detection of new autoantibodies. Coinciding with this rise are false positive autoantibodies without clinical correlates.
METHODS: The medical records of all patients who tested positive for AE antibodies in Sultan Qaboos University Hospital, Muscat, Oman, from May 2016 to December 2022 were reviewed. Cases were verified by th

HSV type 1 (HSV-1) is one of the leading etiologies of sporadic viral encephalitis. Early antiviral intervention is crucial to the survival of herpes simplex encephalitis patients; however, many survivors suffer from long-term neurologic deficits. It is currently understood that HSV-1 establishes a latent infection within sensory peripheral neurons throughout the life of the host. However, the tissue residence of latent virus, other than in sensory neurons, and the potential pathogenic consequences of latency remain enigmatic. In the current study, we characterized the lytic and latent infecti

BACKGROUND: Hashimoto's encephalitis (HE) is a rare neurological complication of Hashimoto's thyroiditis (HT), while limbic encephalitis (LE) is an autoimmune inflammatory disorder frequently associated with anti-neuronal antibodies. The glutamate receptor α-amino-3-hydroxy-5-methyl-4-isoxazole-propionic acid receptor (AMPAR) is important for synaptic transmission, memory, and learning. The etiology of HE remains unclear. We present a case of HE with antibodies to AMPAR2 both in the serum and cerebrospinal fluid.

AIM: To examine the clinical characteristics, laboratory tests, imaging and electroencephalography presentation, treatment, and prognosis of anti-leucine-rich glioma-inactivated 1 (anti-LGI1) encephalitis and improve the awareness of this disease.
METHOD: We performed a retrospective analysis of the clinical data of 41 patients who were diagnosed with anti-LGI1 encephalitis. Their clinical characteristics, laboratory tests, and imaging and electroencephalography data were collected, and the treatment results and prognosis were evaluated. The modified Rankin Scale (mRS) was used to evaluate ne

Rapid eye movement (REM) sleep behaviour disorder (RBD) precedes or accompanies many neurodegenerative disorders. In synucleinopathies, including dementia with Lewy bodies, Parkinson's disease and multiple system atrophy, the prevalence of RBD varies from 19% to almost 77% in different reports. In tauopathies, including Alzheimer's disease, corticobasal degeneration, progressive supranuclear palsy and frontotemporal dementia, the prevalence is rare, ranging from 0% to 27%. RBD has however also been described in amyotrophic lateral sclerosis, limbic encephalitis, Guillain-Barré syndrome, Touret…

BACKGROUND: Non-occlusive mesenteric ischemia (NOMI) is a rare and severe pathological condition that can cause intestinal necrosis without mechanical obstruction of the mesenteric artery. NOMI often develops during the treatment of severe disease in elderly patients and mostly occurs in the intestine supplied by the superior mesenteric artery (SMA). We experienced a 12-year-old patient with NOMI that was segmentally localized in the ascending colon and rectum during encephalitis treatment.
CASE PRESENTATION: A 12-year-old boy was hospitalized with limbic encephalitis. On day 41 after admissi…

Paraneoplastic neurological syndromes are conditions that manifest as the remote effects of cancer. These are very rare, occurring in 1/10000 patients with a malignancy, and include Lambert-Eaton myasthenic syndrome, limbic encephalitis, subacute cerebellar ataxia, opsoclonus-myoclonus, Stiff-Person Syndrome, retinopathies, chronic gastrointestinal pseudo-obstruction and sensory neuropathy. This report describes a case of 41-year-old man who presented with elements of multiple paraneoplastic syndromes, including chronic gastrointestinal pseudo-obstruction, myasthenia gravis-Lambert-Eaton overl…

A case of paraneoplastic syndrome diagnosed post mortem in a 68-year-old man with a single submandibular lymphatic metastasis of testicular seminoma is presented. The syndrome developed 3 years after orchidectomy and clinically combined the features of limbic and bulbar encephalitis together with signs of cerebellar degeneration. Histological examination revealed marked loss of cerebellar Purkinje and granular cells and to a lesser extent of cerebral cortical neurons. Additionally axonal injury (spheroids) and multifocal demyelination with sparse lymphocytic infiltrations was observed. Immunoh

Feline hippocampal necrosis (FHN) is a histopathological diagnosis first identified ∼25 years ago in domestic cats, and characterised by marked neuronal necrosis largely restricted to the hippocampus. FHN is often conflated with an overlapping condition, hippocampal sclerosis (HS). FHN does not represent a singular neurological condition, but rather a common endpoint of several disease entities including neoplasia, vascular disease and autoimmunity. The latter of these has been of particular interest in recent years with the discovery of limbic encephalitis and FHN associated with autoantibodi…

Paraneoplastic neurologic syndromes are cancer-associated, immune-mediated neurologic manifestations that may involve any part of the nervous system. They usually present with characteristic neurologic features and should be considered in high-risk phenotypes such as limbic encephalitis, encephalomyelitis, rapidly progressive cerebellar syndrome, opsoclonus-myoclonus, sensory neuronopathy, enteric neuropathy, and Lambert-Eaton myasthenic syndrome. The diagnosis is made by antibody positivity in the serum or cerebrospinal fluid, in the presence of an appropriate clinical phenotype. Findings on …

We report a human case of viral encephalitis caused by pseudorabies virus (PRV) in China. A 43-year-old man with no previous medical history presented with high-grade fever, headache and tonic-clonic seizures as well as coma. Plain computer tomography (CT) brain imaging showed hypo-density in the bilateral basal ganglia, bilateral occipital lobe, bilateral limbic lobe, and left thalamic. Next-generation sequencing (NGS) confirmed the presence of PRV in cerebral spinal fluid (CSF). Regular polymerase chain reaction (PCR) was applied to confirm the presence of PRV in the CSF and blood. Paraneoplastic neurologic syndrome (PNS) is an uncommon manifestation of cancer that is not caused by the tumor or metastasis. Trigeminal neuralgia (TN) is an initial symptom of this disease, but it has rarely been reported in the literature. Here, we report the case of a 76-year-old woman who presented with classic TN, followed by limbic encephalitis due to an underlying ovarian intestinal-type mucinous borderline tumor, with the presence of anti-Hu antibodies. She recovered quickly after removal of the tumor and was essentially free of symptoms 2 weeks after surgery. Because PNS precedes the…

The classic paraneoplastic neurological syndromes include Lambert-Eaton myasthenic syndrome, limbic encephalitis, sensory neuronopathy, intestinal pseudo-obstruction, subacute cerebellar degeneration, encephalomyelitis, and dermatomyositis. Approximately ten onconeural antibodies that recognize cancer and the nervous system have been described in paraneoplastic neurological syndromes. These antibodies appear to be important diagnostic tools, even though they may not always be present. Deciding whether a given neurological picture is definitely or possibly paraneoplastic depends on the clinical…
