Contemporary reconstruction of the mandible.

Reconstruction of the mandible has evolved significantly over the last 40years. Early attempts were often disfiguring and wrought with compl … ptimizing tongue mobility are important to post-operative oral function. Persistent problems in oro-mandibular reconstruction relate to the effects of radiation treatment on the native tissue and include xerostomia, dysgeusia, osteoradionecrosis and trismus. These problems continue to plague the oral cancer patient despite the significant advances that allow a far more com …

Wnt/OPG signaling axis drives ectopic mandibular regeneration: Mechanism mediated by tibial periosteal stem cell sheets.

Mandibular reconstruction presents substantial clinical challenges due to the jaw's complex anatomical and functional requirements. This study investigated gene expression differences between tibial and mandibular periosteum to understand their distinct osteogenic properties and evaluate the therapeutic potential of tibial periosteum-derived stem cell (PSC) sheets for mandibular defect repair.Transcriptomic analysis revealed differential expression patterns: tibial periosteum demonstrated higher OPG expression, while mandibular periosteum exhibited increased RUNX2 and BMP-2 expression. KEGG en…

Novel approach to the study of jaw kinematics in an alloplastic TMJ reconstruction.

Total alloplastic temporomandibular joint (TMJ) reconstruction is a reliable treatment modality in patients with severely diseased TMJ with good clinical behaviour. TMJ mandibular function after alloplastic reconstruction has scarcely been analysed as a biomechanical parameter and investigation has generally been limited to interincisal measurements without deeper insight into joint kinematics. Dynamic stereometry to assess condylar movements relative to the fossa was performed at the 5 year follow-up of a patient who underwent condylar resection of the right TMJ followed by total alloplastic

Implant Rehabilitation After a Partial Resection of the Mandible Resulting from Submandibular Gland Malignancy-A Case Report.

Adenoid cystic carcinoma of the submandibular gland is a relatively rare malignancy of the oral and maxillofacial region. It is an insidious disease characterized by regional spread, perineural invasion, and potential distant metastases. Because of the aggressive nature of this lesion, it is often impossible to completely excise the tumor, necessitating combination therapy of surgery and radiation. The consequences of surgical ablation coupled with radiation treatment can result in complex considerations in the dental care of these patients. This case report outlines a patient who has recovere

Repairing Cranial Defect With Autogenous Mandibular Outer Plate.

BACKGROUND: Autogenous bone is the best material in cranioplasty because of biological advantages. Previously, skull, rib, ilium, and fibula have been used in the reconstruction of cranial defects. However, the application of autologous mandibular outer plate in the treatment of skull defect is rarely reported. This study evaluated the application of the autogenous mandibular outer plate in the reconstruction of cranial defect. METHODS: Eleven patients who underwent cranioplasty with mandibular outer plate were recruited. Three-dimensional tomography data were collected to calculate the preop

Digital Model-Assisted Prophylactic Stabilization for Mandibular Preservation in Recurrent Juvenile Ossifying Fibroma.

A 22-year-old woman with recurrent psammomatoid-type juvenile ossifying fibroma of the anterior mandible was treated using a mandibular preservation strategy combining local tumor resection with digital model-assisted prophylactic stabilization. Cone-beam computed tomography showed a well-demarcated expansile lesion with marked cortical thinning, indicating that resection alone could leave the residual mandible mechanically vulnerable. Preoperative imaging data were converted into a 1:1 stereolithographic mandibular model for reconstruction plate precontouring, screw-position planning, and est

Mandibular rehabilitation: From the Andy Gump deformity to jaw-in-a-day.

The mandible is a critical structure of the lower facial skeleton which plays an important role in several vital functions. Segmental resection of the mandible is at times required in patients with advanced oral cavity malignancies, primary mandibular tumors, and radiation or medication induced osteonecrosis. Mandibulectomy can significantly decrease quality of life, and thus mandibular reconstruction is an important aspect of the operative plan. Mandibular reconstruction is challenging due to the complex three-dimensional anatomy of the mandible, and the precision required to restore dental o…

Gorham disease of the mandible.

Gorham disease, or massive osteolysis, is a rare condition of unknown etiology. The disease is characterized by spontaneous progressive osteolysis of 1 or more skeletal bones. The mandible is the most commonly involved bone in the maxillofacial region. This article reports a case of Gorham disease with mandibular involvement in a 46-year-old male patient with a 7-year follow-up. In this case, we performed lower right mandibular osteotomy and reconstruction with a phased titanium plate. Postoperative follow-up showed continued mandibular bone loss that was progressing to the contralateral mandi

Visor Flap-Assisted Segmental Mandibulectomy and Immediate Free Fibula Reconstruction for Extensive Plexiform Ameloblastoma of the Anterior Mandible: A Case Report.

Ameloblastoma is a benign but locally aggressive odontogenic tumour commonly affecting the mandible. This case report describes the surgical management of mandibular ameloblastoma in a 65-year-old male with a gradually enlarging anterior mandibular swelling for two years. Clinical and radiological evaluation revealed an expansile lesion in the symphyseal region, and histopathology confirmed plexiform ameloblastoma. Considering the lesion's extent, segmental mandibulectomy was performed using a visor flap and pull-through approach. Immediate reconstruction was achieved with a pre-bent 2.5-mm ti

Intraosseous mandibular schwannoma managed via submandibular approach: a case report with a review of previously published cases.

A 40-year-old female presented with right mandibular swelling. A panoramic radiograph showed a unilocular radiolucency from the mandibular angle to tooth #46. Biopsy confirmed a schwannoma. Surgical resection was performed via a submandibular approach with a reconstruction plate. Teeth 46 and 47 were extracted. Surgery was complication-free, and histopathology confirmed the tumor's benign nature. The patient was discharged on the second postoperative day. At the 1-year follow-up, she had no paresthesia, normal mouth opening, and full mandibular motion. The reconstruction plate was intact. This

Limitations of Osteocutaneous Radial Flap Despite Satisfactory Outcomes in Anterior Mandible Reconstruction With Dental Implants.

The management of oral malignancy necessitates a comprehensive approach focusing on disease eradication and patient quality of life. Surgery remains pivotal, although extensive resection can lead to aesthetic and functional challenges. Reconstruction, often with osteocutaneous radial forearm free flaps (OCRFFF), is crucial for restoring form and function. Prophylactic plating aims to prevent donor site complications, although plate selection is critical. Despite OCRFFF's efficacy in anterior mandible reconstruction, concerns persist regarding donor site morbidity and adequacy for subsequent de
