Effectiveness of mastoid process percussion for identification of the vertebral artery ostium on Doppler studies.

OBJECTIVES: The pretransverse or first segment of the vertebral artery may be confused with adjacent branches of the proximal subclavian artery during Doppler assessment. This study investigated the effectiveness of mastoid process percussion, the "mastoid tap" maneuver, for identification of the vertebral artery ostium. METHODS: Fifty patients presenting consecutively for carotid sonography were recruited. Doppler waveforms were collected at the vertebral artery ostia, thyrocervical trunks, and proximal subclavian arteries while the mastoid tap maneuver was performed. The outcome indicator w…

Epithelioid hemangioendothelioma of the mastoid: resection for recurrence and adjuvant radiation with 8-year followup.

Epithelioid hemangioendothelioma (EH) is a rare, vascular neoplasm that can affect any age group and has been reported previously in sites including bone, liver, lung, breast, and brain. We describe a case of EH located in the mastoid, which appears to be the first report of EH in this site. The patient was 62 years old when they presented with dizziness and nausea. A suboccipital surgical approach was utilized to resect the tumor. After 18-month followup, the patient was symptom-free; however, imaging demonstrated a recurrence and the patient was taken back to the operating room for a resecti…

Molecular confirmation of transmission route of Staphylococcus intermedius in mastoid cavity infection from dog saliva.

We report a case of infection of a mastoid cavity after mastoidectomy had been performed for chronic otitis media with cholesteatoma. The infection was caused by Staphylococcus intermedius after a pet dog had licked the patient's ears. Bacterial strains from the dog's saliva and the otorrhea in the patient were confirmed to be identical by pulsed-field gel electrophoresis analysis. The possibility of an oral transmission route of S. intermedius from pets to humans should be noted.

Pneumocephalus caused by fistulas of the mastoid air cells treated with a temporoparietal fascial flap.

Using a temporoparietal fascial flap and hydroxyapatite ceramics, we treated a patient for complications after a neurosurgical operation for glossopharyngeal neuralgia. These consisted of pneumocephalus and cerebrospinal fluid rhinorrhea resulting from fistulas of mastoid air cells associated with a subcutaneous dead space. By means of the temporoparietal fascial flap, we were able to fill the dead space and reinforce the repaired dural and mastoid lesion as well. Hydroxyapatite ceramics were also useful for closing the mastoid air cell fistulas.

[Comparison of two different recording sites in electrocochleography: promontory and external mastoid position (author's transl)].

One possible evaluation of the functional unit: hair-cell/afferent neurit is the recording of an action-potential (AP) from the acoustic nerve. The authors report their experience in recording the AP from an external position on the mastoid planum and from a transtympanic position on the promontory. The promontory site is prefered because of the higher electrical response amplitude and because of its relatively reduced sensitivity against myogenic potentials. Characteristic findings in normal ears and in some pathological conditions are to be demonstrated.

A Unique Case of Osteoid Osteoma Within the Mastoid Air Cell: Insights From CT Imaging.

Osteoid osteomas are benign, solitary bone-forming tumors associated with progressively worsening pain. The exact etiology remains unclear, although it is speculated to result from neoplasia, direct trauma, or an inflammatory process. We present a rare case of a 53-year-old male who presented to the ED with a one-year history of worsening right ear pain and no significant medical or surgical history. On physical examination, the right mastoid area was tense and swollen, without signs of inflammation. Following an enhanced and non-enhanced CT, a diagnosis of an osteoid osteoma was established. …

Reevaluating the Routine Use of Mastoid Pressure Dressings in Otologic Surgery: A Randomized Controlled Trial.

OBJECTIVE: The use of mastoid pressure dressings (MPDs) after otologic surgery has been a standard practice for decades, believed to minimize complications such as hematoma and wound infection. However, the efficacy of MPDs in improving patient outcomes has been questioned. This study aimed to evaluate the necessity and impact of MPDs on postoperative pain, complications, and overall patient comfort. METHOD: In this prospective, randomized, controlled trial, 200 patients undergoing major ear surgeries were randomly assigned to receive either a MPD group or a non-MPD (NMPD) group. Postoperativ…
