Evaluating the early impact of Medicaid expansion on trends in diagnosis and treatment of benign gallbladder disease in Kentucky.

BACKGROUND: In January 2014, Kentucky expanded Medicaid coverage in an effort to improve access to healthcare. This study evaluated the early impact of Medicaid expansion on diagnosis and treatment of benign gallbladder disease in Kentucky. METHODS: Administrative claims data were queried for patients undergoing cholecystectomy for benign gallbladder disease between 2011 and 2015. Demographic, procedure, and outcome variables from 2011 to 2013 (PRE) and 2014-2015 (POST) were compared. RESULTS: After Medicaid expansion, patients were more likely to have their operation performed as an outpati…

The association between newborn screening analytes and childhood autism in a Texas Medicaid population, 2010-2012.

Autism (or autism spectrum disorder [ASD]) is an often disabling childhood neurologic condition of mostly unknown cause. It is commonly diagnosed at 3 or 4 years of age. We explored whether there was an association of any analytes measured by newborn screening tests with a later diagnosis of ASD. A database was compiled of 3-5 year-old patients with any ASD diagnosis in the Texas Medicaid system in 2010-2012. Two controls (without any ASD diagnosis) were matched to each case by infant sex and birth year/month. All study subjects were linked to their 2007-2009 birth and newborn screening labora…

Are Medicaid Coverage Gaps Associated with Higher Health Care Resource Utilization and Costs in Patients with Schizophrenia?

The objective was to assess the association of Medicaid coverage gaps with health care resource utilization (HRU) and costs of patients with schizophrenia. Patients with schizophrenia were identified from the Medicaid database. The beginning of the first eligible gap was defined as the index date. Per-patient per-month (PPPM) HRU and costs before versus after a gap were assessed, and the association between gap duration and PPPM HRU and costs was examined up to 12 months post index. Together with 95% confidence intervals, HRU differences were reported in rate ratios (RRs), and cost differences…

Impact of a medical home model on costs and utilization among comorbid HIV-positive Medicaid patients.

OBJECTIVES: The Pennsylvania Chronic Care Initiative (CCI) was a statewide patient-centered medical home (PCMH) initiative implemented from 2008 to 2011. This study examined whether the CCI affected utilization and costs for HIV-positive Medicaid patients with both medical and behavioral health comorbidities. STUDY DESIGN: Nonrandomized comparison of 302 HIV-positive Medicaid patients treated in 137 CCI practices and 2577 HIV-positive Medicaid patients treated elsewhere. METHODS: All patients had chronic medical conditions (diabetes, chronic obstructive pulmonary disease, asthma, or congesti…

Continuity of medication management in Medicaid patients with chronic comorbid conditions: An examination by mental health status.

OBJECTIVE: Patients with serious mental illness (SMI) often have comorbid cardiometabolic conditions (CMCs) that may increase the number of prescribers involved in treatment. This study examined whether patients with SMI (depression and schizophrenia) and comorbid CMCs experience greater discontinuity of prescribing than patients with CMCs alone. METHODS: 2009 Medicaid data were used to compare number and types of prescribers (primary care, cardiometabolic, psychiatric, other) in individuals with 1-3 CMCs (diabetes, hypertension, dyslipidemia) alone (n=76.451); with CMC and schizophrenia (n=6…

Leveraging The Affordable Care Act To Enroll Justice-Involved Populations In Medicaid: State And Local Efforts.

The Affordable Care Act provides an unprecedented opportunity to enroll criminal justice-involved populations in health insurance, particularly Medicaid. As a result, many state and county corrections departments have launched programs that incorporate Medicaid enrollment in discharge planning. Our study characterizes the national landscape of programs enrolling criminal justice-involved populations in Medicaid as of January 2015. We provide an overview of sixty-four programs operating in jails, prisons, or community probation and parole systems that enroll individuals during detention, incarc…

Antiretroviral Therapy Use During Pregnancy and the Risk of Small for Gestational Age Birth in a Medicaid Population.

BACKGROUND: Several studies have assessed the association between antiretroviral (ARV) therapy use during pregnancy and small for gestational age (SGA), but the evidence remains incompletely elucidated. METHODS: We linked data from Tennessee Medicaid files and vital records to evaluate pregnancies among human immunodeficiency virus (HIV)-infected women who delivered between 1994 and 2009. Maternal HIV status was defined based on diagnosis codes, ARV prescriptions and laboratory codes for CD4 count or HIV RNA assays. ARV use was identified from pharmacy claims. Risk of SGA (defined as birth we…

The direct medical costs of Huntington's disease by stage. A retrospective commercial and Medicaid claims data analysis.

OBJECTIVE: This study quantified the direct healthcare costs and major cost drivers among patients with Huntington's disease (HD), by disease stage in commercial and Medicaid databases. METHODS: This retrospective database analysis used healthcare utilization/cost data for HD patients (ICD-9-CM 333.4) from Thomson Reuters' MarketScan Commercial and Medicaid 2002-2009 databases. Patients were classified by disease stage (Early/Middle/Late) by a hierarchical assessment of markers of disease severity, confirmed by literature review and key opinion leader input. Costs were measured over the follo…

Changes in antipsychotic medication use after implementation of a Medicaid mental health carve-out in the US.

BACKGROUND: Many Medicaid programmes now offer behavioural healthcare through managed care organizations. Medicaid programmes are concerned about carve-outs because the use of non-included services may rise, limiting the efficiencies anticipated with the implementation of managed care. There also exist concerns that patients with serious mental illness may receive reduced care through managed care and consequently have poorer outcomes. OBJECTIVE: This study examined prescription drug utilization among Medicaid recipients with the implementation of a mental health carve-out plan in Florida. In…

The evidence doesn't justify steps by state Medicaid programs to restrict opioid addiction treatment with buprenorphine.

Many state Medicaid programs restrict access to buprenorphine, a prescription medication that relieves withdrawal symptoms for people addicted to heroin or other opiates. The reason is that officials fear that the drug is costlier or less safe than other therapies such as methadone. To find out if this is true, we compared spending, the use of services related to drug-use relapses, and mortality for 33,923 Massachusetts Medicaid beneficiaries receiving either buprenorphine, methadone, drug-free treatment, or no treatment during the period 2003-07. Buprenorphine appears to have significantly ex…

Hormonal contraception use and pregnancy in adolescents with sickle cell disease: analysis of Michigan Medicaid claims.

BACKGROUND: Access to effective family planning is of great importance for women with sickle cell disease (SCD) due to the increased frequency of pregnancy complications. However, little is known regarding use of hormonal contraception, complications of contraception, and pregnancy in young women with SCD. STUDY DESIGN: We abstracted diagnostic codes, procedure codes and pharmacy claims from women with SCD, aged 13-21 years, enrolled in Michigan Medicaid between 1/1/2000 and 12/31/2003 with ≥ 9 months of continuous eligibility. RESULTS: We identified 250 women with SCD with an average period…

An evaluation of a Medicaid expansion for cancer care: The Breast and Cervical Cancer Prevention and Treatment Act of 2000.

The National Breast and Cervical Cancer Early Detection Program is a multifaceted, federal program that provides breast and cervical cancer screening and diagnostic services to low-income women, but does not cover the costs of treatment. This study used a fixed-effects, longitudinal time-series research design (from 1995 to 2005) to evaluate the impact of a Medicaid expansion aimed at covering treatment for program clients, enacted via the Breast and Cervical Cancer Prevention and Treatment Act of 2000. In summary, the Treatment Act of 2000 had some positive impacts, including a 12.8% decrease…

Transition probabilities and predictors of adherence in a California Medicaid population using antihypertensive and lipid-lowering medications.

OBJECTIVES: To determine adherence rates, transition probabilities, and factors associated with transition from higher to lower adherence in antihypertensive (AH) and lipid-lowering (LL) medications. METHODS: California Medicaid data (1995-2003) were used to identify hypertensive patients with prescriptions for both AH and LL medications. Proportion of days covered (PDC) was used to define three adherence classifications: fully adherent (FA, PDC >or= 0.8), partially adherent (PA, 0.2 <or= PDC < 0.8), and nonadherent (NA, PDC < 0.2). Annual transition matrices documented the probability of adh…

State Medicaid Body Mass Index Requirements for Solid Organ Transplant Coverage: A 50-State Policy Review.

INTRODUCTION: Body mass index (BMI) has long served as a screening tool for assessing procedural risk and is used as an exclusion criterion for insurance coverage. Variation in state Medicaid policies, specifically pre-transplant BMI requirements, may affect transplant eligibility and limit access to transplantation. No published literature summarizes these policies. We conducted a 50-state review of Medicaid policies to identify pre-transplant BMI requirements across solid organ transplantation (heart, lung, liver, kidney, pancreas, intestine). METHODS: Public records requests were submitted…
