Total and near-total lower lip reconstruction: 20 years experience.

BACKGROUND: Reconstruction of total and near-total lower lip defects presents a formidable challenge for the reconstructive plastic surgeon. Many methods have been described, and each has its own advantages and disadvantages. The aim of this article is to discuss the selection of techniques and report our experience of total or near-total lower lip reconstruction. MATERIAL AND METHODS: Over a 20-year period from January 1993 to December 2013, a total of 87 patients underwent total or near-total lower lip reconstruction. Bilateral Yu's flaps were used in 61 patients, double mental neurovascula

Resilience and positive mental health in persons with spinal cord injury and their informal caregivers: A dyadic study.

BACKGROUND: Despite daily challenges, persons with spinal cord injury (PwSCI) successfully mobilize individual and relational resources and attain good mental health. Psychosocial resources were investigated as components of resilience, defined as satisfactory adaptation to adversity. While the association between resilience and positive mental health, conceptualized as psychological, emotional, and social well-being, was widely observed in individuals, it remains unexplored within dyads. This study was thus aimed to investigate this association at both individual and relational levels among P

"My home is (now) at peace": Evaluating the relevance, acceptability and potential scalability of a guided self-help intervention for male refugees in Uganda.

In humanitarian crises, male refugees face significant mental health challenges, including high rates of depression and alcohol misuse. However, access to adequate mental health services is limited. In this study, we evaluate a combined intervention that addresses both mental health and alcohol misuse among male refugees in Uganda, focusing on its relevance, acceptability, and potential for scalability. We conducted a qualitative study, using process evaluation data, to evaluate a combined guided self-help intervention for mental health and alcohol use among male refugees in Uganda's Rhino and

Sexual and gender minority adverse childhood experiences (SGM-ACEs), perceived social support, and adult mental health.

BACKGROUND: Building on decades of research into the long-term developmental impacts of adverse childhood experiences (ACEs), researchers have called for expanding the ACEs framework to include experiences specific to minoritized identities. Recent empirical research has led to the development of a measure of sexual and gender minority adverse childhood experiences (SGM-ACEs). Within the SGM-specific ACEs framework, research on the long-term impact to adult mental health and the role of perceived social support are not well studied. OBJECTIVE: The purpose of this paper was two-fold. First, ex

A system analysis of the mental health services in Norway and its availability to women with female genital mutilation.

BACKGROUND: This article explores mental health services in Norway and their availability for women subjected to female genital mutilation/cutting (FGM/C). The article focus on the system of communication and referrals from the perspective of health workers, and aims to identify bottlenecks in the system, what and where they are to be found, and analyze how different mental health services deal with Sub Saharan African (SSA) women in general, but in particular with respect to FGM/C. METHOD: The study was conducted in Oslo, Norway, using a qualitative fieldwork research design, with the use of

Reliability of clinical judgment for evaluation of informed consent in mental health settings and the validation of the Evaluation of Informed Consent to Treatment (EICT) scale.

INTRODUCTION: The competence assessment to give informed consent in the legal and healthcare settings is often performed merely through clinical judgment. Given the acknowledged limited reliability of clinician-based evaluation in the mental health sector, particularly for the assessment of competence to consent, our objective was to ascertain the dependability of clinical judgment when evaluating the ability of schizophrenia patients to make choices about their health. METHODS: The potential convergence between clinical evaluation and scores from a new standardized assessment (the "Evaluatio

Teachers' Response to Stress, Anxiety and Depression During COVID-19 Lockdown: What Have We Learned From the Pandemic?

BACKGROUND: As a result of national lockdown due to the COVID-19 outbreak, teachers were forced to suspend their classes and replace them with online teaching and home schooling. Additional stressors such as competing family responsibility have increased their worries and mental health problems. The aim of this study was to determine the impact of COVID-19-related worries on teachers' emotional symptoms, considering the mediating role of several protective factors. METHODS: A total of 614 Chilean teachers (94.60% women) participated in this study using a cross-sectional design and incidental

Homeless people and health: a qualitative enquiry into their practices and perceptions.

BACKGROUND: Homeless people experience poorer health and shorter life expectancies than the UK average, but their health needs and expectations have received little attention in research. This study aims to understand homeless people's health perceptions and experiences. METHODS: Semi-structured interviews took place with 28 homeless people in Gloucestershire. The transcripts were thematically analysed drawing out key themes. RESULTS: Homelessness was often rooted in early trauma or an unstable family unit. Participants described poor support on leaving prison, termination of a cohabiting re

General disease factor: evidence of a unifying dimension across mental and physical illness in children and adolescents.

BACKGROUND: Understanding the relationship between mental and physical health conditions is crucial for developing comprehensive healthcare strategies. The putative existence of a general disease factor (d-factor) that underlies the vulnerability to both physical and mental conditions could have important implications for our approach to health assessment and treatment. OBJECTIVE: To investigate the presence and characteristics of a general d-factor in children and adolescents. METHODS: This Swedish registry-based cross-sectional study included children and adolescents born between 1996 and

Short-term and long-term influences of family arguments and gender difference on developing psychological well-being in Taiwanese adolescents.

Adolescent mental health is crucial for social competence and accomplishment in later life. The World Health Organization (WHO) estimates that approximately 20% of adolescents suffer from psychological symptoms. However, improving family risk and school environments can largely promote adolescent mental health. A longitudinal survey was conducted to investigate adolescent psychological well-being (PWB) status and associated factors in adolescents 15-20 years of age. Family and school context variables were interviewed and recorded. A total of 2896 participants were included from high, middle,

Treatment over Objection Versus Withdrawal of Dialysis in Cases of Severe Mental Illness.

AbstractPatients with severe mental illness are more likely to develop kidney disease and its complications compared to patients without severe mental illness. Once patients with mental illness develop end-stage kidney disease, supporting adherence is important but can be difficult in moments of acute psychosis when the patient objects to dialysis, particularly when considering the lack of dialysis-compatible psychiatric hospitals. In this case, a woman with a history of bipolar disease and end-stage kidney disease has a manic episode after discontinuing her antipsychotic medications and refus

The difficulties of secondary prophylaxis of cervical cancer in women suffering from paranoid schizophrenia - a case study.

Cervical cancer constitutes 5.32% of all malignant neoplasm cases, it is the sixth most common condition of the cancer type and it is an important problem because of its medical, epidemiological and social implications. The aim of primary prophylaxis is to reduce the number of new cases, while secondary prophylaxis is to provide early diagnoses and treatment of cancer cases. The aim of this work is to present the case of 55-year-old woman treated with chronic paranoid schizophrenia whose gynecologist refused to collect biological material for cytological evaluation. The patient was diagnosed w

Initial neonatal resuscitation: skill retention after the implementation of the novel 24/7 HybridLab® learning system.

BACKGROUND: Ensuring long-term retention of the acquired practical skills is one of the major aims of a medical school. This depends on the application of training techniques and their combinations. In order to standardize the teaching process, to acquire and maintain a broad array of technical, professional, and interpersonal skills and competencies, and to improve the retention of practical skills, we developed a new training technique - the HybridLab® learning method. It consists of an e-learning platform, hands-on simulation, carefully elaborated learning algorithms (DRAKON), peer-to-peer

Neuropsychiatry: Towards a Philosophy of Praxis.

Neuropsychiatry is a specialized clinical, academic and scientific discipline with its field located in the borderland territory between neurology and psychiatry. In this article, we approach the theoretical definition of neuropsychiatry, and in order to address the practical aspects of the discipline, we describe the profile of a neuropsychiatric liaison service in the setting of a large hospital for neurological diseases in a middle-income country. An audit of consecutive in-patients requiring neuropsychiatric assessment at the National Institute of Neurology and Neurosurgery of Mexico is re

Incapacity of the Mind Secondary to Medication Misuse as a Not Criminally Responsible Defense.

The manifestations of disorders of the mind may play a role in the occurrence of criminal behavior. In the majority of the cases, the presence of a psychiatric disorder is cited as the reason that an individual was not fully aware of his behavior. However, other conditions, such as seizure disorders or hypoglycemia, have also been linked to an inability to understand the nature and consequences of one's actions. On occasion, these situations can be explained by a state of automatism that may be described as insane or noninsane. In this article, we describe the case of a 77-year-old man, suffer

Ethical concerns in caring for elderly patients with cognitive limitations: a capacity-adjusted shared decision-making approach.

Mrs. A is a pleasant seventy-seven-year-old widow with an increasingly symptomatic right knee that has markedly limited her activities in the past year. Mrs. A's daughter, who lives in town, urged her to seek treatment. History, physical examination, and radiographs confirmed the diagnosis of end-stage knee arthritis. Dr. Z, the orthopaedic surgeon, presented total knee arthroplasty as a potential treatment option and provided detailed information on the surgery and recovery. Mrs. A indicated that if Dr. Z thinks that total knee arthroplasty is a good idea, she would agree to have the surgery.

Decision-making cognition in neurodegenerative diseases.

A large proportion of human social neuroscience research has focused on the issue of decision-making. Impaired decision-making is a symptomatic feature of a number of neurodegenerative diseases, but the nature of these decision-making deficits depends on the particular disease. Thus, examining the qualitative differences in decision-making impairments associated with different neurodegenerative diseases could provide valuable information regarding the underlying neural basis of decision-making. Nevertheless, few comparative reports of decision-making across patient groups exist. In this Review

A conceptual model and assessment template for capacity evaluation in adult guardianship.

PURPOSE: We develop a conceptual model and associated assessment template that is usable across state jurisdictions for evaluating the independent-living capacity of older adults in guardianship proceedings. DESIGN AND METHODS: We used an iterative process in which legal provisions for guardianship and prevailing clinical practices for capacity assessment were integrated, through expert group consensus and external review by legal and health care professionals, to form a conceptual model and template. RESULTS: The model and template provide a structure for conducting and documenting a capaci

[The project of a law concerning patients' rights at the end of life].

A bill about Patients' rights and the end of life was adopted unanimously by the National Assembly on November 30, 2004. Article 1 provides that the physician is not obligated to continue treatment "when the latter appears futile, disproportional and has no effect other than artificially maintaining the patient's life". Article 2 recognizes that painkillers administered at high doses to terminally ill patients may as a side effect "shorten the patient's life". Conscious patients and the families of unconscious patients must be so informed, and the discussion must be mentioned in the patient's
