Divergence paralysis caused by acute midbrain infarction.

A 41-year-old woman with idiopathic thrombocytopenic purpura and dyslipidemia abruptly developed vertigo, truncal ataxia and divergence paralysis. Cranial magnetic resonance imaging demonstrated the presence of infarction in the left superior paramedian mesencephalic artery involving the vicinity of the periaqueductal gray matter. The symptoms rapidly resolved under the administration of anti-platelet agents. The precise location of the hypothetical divergence center of the ocular motor system remains unclear because the lesions responsible for divergence paralysis are rarely identified on neu

Rosette-forming glioneuronal tumors: a report of two cases.

Rosette-forming glioneuronal tumor, a mixed glial and neuronal tumor, is a relatively new entity in tumors of the central nervous system, included in 2007 classification published by World Health Organization (WHO). It was initially described to occur in and around the fourth ventricle; however, recent case series have reported other locations also. Their occurrence in supratentorial and spinal locations has recently been reported. We report two cases of rosette-forming glioneuronal tumors, one in the midbrain and one in a suprasellar location, and review the literature.

[Collier's sign in Miller Fisher syndrome].

Collier's sign is well known as unilateral or bilateral eyelid retraction due to midbrain lesions. This sign is usually caused by infarction, tumor, multiple sclerosis, neuro-degenerative disease, or encephalitis. We report a case of Miller Fisher syndrome (MFS) which demonstrated Collier's sign. A 54-year-old man developed ophthalmoplegia, ataxia, and areflexia two weeks after common cold-like symptoms. At the same time, bilateral upper eyelid retraction (Collier's sign) was remarkably observed. Serum anti-GQ1b antibody was positive. Albumino-cytologic dissociation was seen at two weeks after

Superior divisional third cranial nerve paresis: clinical and anatomical observations of 2 unique cases.

BACKGROUND: Within the midbrain, the third nerve nucleus is composed of a complex of subnuclei. The fascicular portion of the nerve courses through the red nucleus and exists in the midbrain just medial to the cerebral peduncle. The cisternal portion of the nerve is a single structure that divides into a superior branch and an inferior branch in the region of the cavernous sinus and superior orbital fissure. OBJECTIVE: To describe 2 patients with superior divisional third cranial nerve paresis resulting from a lesion involving the cisternal portion of the nerve prior to its anatomical bifurca

The thalamus and midbrain in Reye syndrome.

A previously healthy 5 1/2-year-old male had Reye syndrome. He presented in coma with apnea 1 week after a viral infection and following 2 days of vomiting and progressive obtundation. He was in coma with dystonic posturing and intact brainstem function. Laboratory evaluation revealed initial hypoglycemia, and markedly elevated liver enzymes, prolonged clotting times, and elevated ammonia levels. No underlying metabolic disorder was present, and the patient completely recovered. On a modified diffusion-weighted image magnetic resonance imaging scan, restriction of diffusion in the thalamus and

[A case report of bilateral cerebral peduncular infarction manifesting tetraparesis and pseudobalbur palsy].

A 63-year-old female had presented with right hemiparesis and slight dysarthria. MRI had showed the infarction of left pons and left peduncle. Three months later she further presented with left hemiparesis, severe dysarthria and swallowing disturbance. MRI showed bilateral cerebral peduncular infarction. And the angiogram showed the occlusion of basilar artery at the just distal portion of the superior cerebellar artery. We recognized our case as the infarction due to the basilar artery occlusion. The 16 cases of bilateral cerebral peduncular infarction were reported. In these reports, the sym

[Functional structure and cerebrovascular circulation of the vestibulo-ocular formations of the brain stem of the human cerebrum].

This work is dedicated to the study of anatomo-physiological peculiarities of the construction and cerebrovascular circulation of the vestibulo-ocular formations of the brain stem of the human cerebrum. The analysis of anatomo-physiological data enable to convince that the arches of the vertical and horizontal vestibulo-ocular reflexes include many structures of the brain stem of the human cerebrum, which are located along its longitude from the superior colliculi to caudal sections of brain stem. There is an existence of separate mechanisms of horizontal and vertical vestibulo-ocular interact

The relative embryotoxicity of 1,3-dichloro-2-propanol on primary chick embryonic cells.

1,3-Dichloro-2-propanol (1,3-DCP) is a chlorinated compound used in the fabrication of industrial products such as hard resins, celluloid or paints. It has also been detected in instant soups and soy sauce. 1,3-DCP has been associated with major necrosis of the liver in humans [Chem.-Bio. Interact. 80 (1991) 73]. In humans and laboratory animals, 1,3-DCP is metabolised to dichloroacetone (1,3-DCA) by cytochromes P450 2E1 and 1A2 [J. University Occup. Environ. Health 14 (1992) 13]. 1,3-DCA is a hepatotoxin. We suggest that 1,3-DCA could be embryotoxic at doses that do not cause adverse maternal

Immunocytochemical localization of the AMPA receptor subunits in the mesencephalic trigeminal nucleus of the rat.

The mesencephalic trigeminal nucleus is composed of large (35-50 microns) pseudo-unipolar neurons. Closely associated with them are small (< 20 microns) multipolar neurons. An unique peculiarity of the pseudo-unipolar perikarya is that they receive synaptic input from various sources, which sets them apart from the dorsal root and cranial nerves sensory ganglia neurons. Whereas glutamate is the best neurotransmitter candidate in pseudo-unipolar neurons, glutamatergic input into them has not yet been reported. AMPA glutamate receptors are implicated in fast excitatory glutamatergic synaptic tra

Pathology of symptomatic tremors.

Symptomatic tremors are labeled in the literature under different names including rubral tremor, midbrain tremor, thalamic tremor, myorhythmia, Holmes' tremor, cerebellar tremor, and goal-directed tremor. The most common tremor is a delayed-onset postural and action tremor with a low frequency of 3 Hz and a proximal distribution. Resting irregular tremor is sometimes present. Mild cerebellar dysmetria is often detected. The lesions are mainly located in the thalamus, the brain stem, and the cerebellum, with secondary interruption and degeneration of various pathways and olivary hypertrophy. Th

[Perimesencephalic subarachnoid hemorrhage].

Acute, non-traumatic subarachnoid haemorrhage is usually caused by a ruptured aneurysm. This is a serious condition involving high mortality. Perimesencephalic haemorrhage has recently been identified as a clinical subset of subarachnoid haemorrhage. A pattern of haemorrhage predominantly limited to perimesencephalic cisterns on CT is highly predictive of a normal angiogram, and is connected with an excellent prognosis. We describe two patients with perimesencephalic haemorrhage.

Absence of correlation between transient cranial hemorrhages and congenital malformations following neural crest ablation in chicks.

Ablation of premigratory cardiac neural crest has been used to produce and study extensively a model of abnormal cardiovascular dysmorphology. Previous and continuing research in this laboratory concerns different aspects of the involvement of cranial neural crest in the development of cranial, cervical and cardiac tissues in chick embryos. Recently, we detected the occurrence of transient cranial hemorrhages 24-48 hr after the ablation of selected segments of premigratory cranial neural crest. Since one possible mechanism of action for certain teratogens involves nonreparable damage to a prim

Serial MRI in listeria mesenrhombencephalitis: a case report.

Listeria mesenrhombencephalitis is a rare disorder, especially in the non-immunocompromised host. The predilection for brain stem involvement is unexplained. The anatomical correlation of brain stem signs with serial MRI over three months is shown in a 64 year old white man. The pathological lesions seemed to be a combination of micro-abscesses and associated oedema.

Connections of the posterior nucleus of the amygdala.

The connections of a relatively homogeneous band of neurons in the caudal amygdala have been examined with anterograde and retrograde axonal tracing methods in the rat. This region, called here the posterior nucleus of the amygdala (PA), corresponds in part to an area that has been referred to as the cortico-amygdaloid transition area, posterior part of the medial nucleus of the amygdala, amygdalo-hippocampal transition area, and posteromedial basal nucleus. Experiments with fluorogold and phaseolus vulgaris leucoagglutinin (PHAL) indicate that the major neuronal input to the PA arises in the

[The parasympathetic direct pathway from the midbrain to the ciliary muscle in cats and monkeys].

The visceral, namely the parasympathetic component of the oculomotor complex which directly innervates ciliary muscle was investigated using horseradish peroxidase (HRP) in three cats and three monkeys. A unilateral total iridectomy to abolish innervation to the iris was made in all animals. After 1 month to allow fiber degeneration in the iris, HRP was injected into the ciliary muscle of ipsilateral eye. Distribution of the labeled cells at the midbrain and at the ciliary ganglion was examined. Labeled cells were present in the ciliary ganglion and midbrain in all three cats and three monkeys

[Delayed finger opening caused by a midbrain lesion].

We report a 25 year-old woman with delayed finger opening caused by a midbrain lesion. Magnetic resonance imaging revealed an abnormal intensity area in the left midbrain in the vascular territory corresponding to the mesencephalic inferomedian branch and central posteromedian branch of posterior cerebral artery. Clinically, she suffered from Weber's syndrome, and in the course of recovery, she developed marked delayed finger opening. Her weakness was slight, this delayed finger opening was observed only after maximal grip, and she required about ten seconds to open her right fingers. She coul

The human brain at stages 18-20, including the choroid plexuses and the amygdaloid and septal nuclei.

The development of the human brain during the seventh embryonic week was studied in serial sections of 88 embryos, and graphic reconstructions were prepared. From stages 18 to 20 the cerebral hemispheres expand rapidly and become more and more distinct entities. The longitudinal fissure between them occupies approximately half of their rostrocaudal extent. In stage 20 they have progressed so far in organization that functional aspects (based on synapses in the primordial plexiform layer) are of importance. An advanced differentiation is also present in the amygdaloid body, which has at least f
