[Nail changes within the scope of reflex dystrophy].

We describe a 16-year-old boy with reflex sympathetic dystrophy after a fracture of the right hand. He had inflammation of the proximal nail folds and arrested nail growth of digits 3, 4 and 5 on the right hand. After 2 months lymphatic drainage treatment, the changes of the nails had disappeared.

Prenatal findings in chondrodysplasia punctata, tibia-metacarpal type.

Chondrodysplasia punctata, a skeletal dysplasia with craniofacial dysmorphism and joint contractures can occur with rhizomelia, mesomelia or both. The rhizomelic form is generally lethal, whereas one form of mesomelic chondrodysplasia punctata has been described that is associated with a presumably normal lifespan and intelligence. We describe a case of a fetus suspected prenatally of having rhizomelic chondrodysplasia punctata, who was subsequently diagnosed at 1.5 years of age to have the tibia-metacarpal form of chondrodysplasia punctata. The prenatal sonographic findings of second-trimeste

Growth plate injury of the hand and wrist in renal osteodystrophy.

We present two cases of growth plate injury of the hand and wrist in young patients with renal rickets and osteodystrophy but no history of antecedent trauma. The postulated mechanism of injury is subclinical biomechanical stress on the involved extremity superimposed on the already weakened metaphysis and growth plate. Thus, the findings may be viewed as an insufficiency injury to the metaphysis and growth plate. Physicians should be aware that, in the absence of unusual athletic activity, epiphysiolysis of the upper extremity is uncommon and requires a search for underlying metabolic bone di

Rigid fixation of phalangeal and metacarpal fractures.

Rational use of rigid fixation in hand surgery requires awareness of the advantages as well as the potential pitfalls of this relatively complex method of fracture management. A rational decision also necessitates familiarity with closed techniques of internal fixation which, in most cases, provide superior alternatives for the phalanges and metacarpals. The optimal application of screws, plates, and tension-band wires to small bone fractures can be ascertained only by critical assessment of large series of cases employing these devices and comparisons with similar series using other technique

Fracture-dislocations of all five carpometacarpal joints: a case report.

Injuries to all five carpometacarpal joints are rare. Only four cases have been reported in the English literature. A case of fracture-dislocations of all five carpometacarpal joints is presented. An excellent result has been obtained 2 years after open reduction and internal fixation. This case is the only one in the English literature so treated and with prolonged follow-up.

Nerve injuries associated with wrist trauma.

Although nerve injury rarely accompanies wrist trauma or disease, the long-term functional result frequently depends more on resolution of the nerve deficit and symptoms than on resolution of the basic orthopedic pathology. The origins of nerve injury associated with orthopedic problems are reviewed, and a rational approach to their management is described.

[The assessment of skeletal mineralization in children with urolithiasis].

The x-ray pictures have principal significance in the clinical estimation of morphology of the skeleton. Roentgenometrical investigations of second metacarpal bone allow to assess the value of PCA (percentage cortical area), which is adequate to the mineral contents of the bone structure. PCA value of 100 equals the density of 2,3 g/cm3. The standard values of PCA and density equivalents were established in a group of 600 healthy children. The study was based on a group of 50 sick children aged from 1 to 16 years. The urolithiasis in all cases was diagnosed and all children were treated in the

Osteochondritis dissecans and brachymesophalangia: a hereditary syndrome.

A kindred with an unusual congenital hand and foot anomaly is presented. It is an autosomal dominant hereditary disorder characterized by brachymesophalangia, slight clinodactyly, deformed metacarpal heads, osteochondritis dissecans in metacarpophalangeal joints, absence or hypoplasia of the ulnar styloid processes, and brachyhypophalangia in the feet.

Ulnar drift and metacarpophalangeal joint subluxation in the rheumatoid hand: review of the pathogenesis.

Ulnar drift and metacarpophalangeal joint subluxation are complicated abnormalities with a wide variety of causes. This paper reviews certain anatomic and mechanical features of the normal hand which predispose toward these abnormalities. In the rheumatoid hand, the major deforming forces include extensor forces, flexor forces, intrinsic forces, and forces from the wrist and carpometacarpal joints. Ulnar drift and metacarpophalangeal joint subluxation result not from any singular cause, but from a combination of all or many of these forces acting upon a joint rendered unstable by rheumatoid sy

Central column reconstruction following total resection of a third metacarpal giant cell tumour.

A wide resection of a giant cell tumour involving the entire middle metacarpal is presented. Reconstruction preserving the central column and metacarpophalangeal joint was achieved using autologous iliac crest bone as a spacer and structural support. The fibro-osseous cartilage portion of the iliac graft was used as a "hemi-joint" replacement. By using a bridging bone graft and screw to fuse the adjacent proximal phalanges of the middle and index fingers, a stable "internal syndactyly" was achieved. Although independent index and middle finger motion was sacrificed, the approach allowed wide r

Septic monoarthritis of the first carpo-metacarpal joint caused by Mycobacterium kansasii.

A case of septic carpal monoarthritis due to Mycobacterium kansasii developing 16 months after accidental inoculation in a healthy laboratory technician is reported. No predisposing factor such as immunosuppression, preexisting degenerative, inflammatory arthritis or cortisone injection was present. Treatment with antituberculous oral medication alone resulted in resolution of the disease. Synovectomy was unnecessary. Ten years after the initial causative event, the patient remains free of symptoms.

A prospective study of the treatment of fractures of the little finger metacarpal shaft with a short hand cast.

This prospective study describes the use of a short hand cast, which leaves the finger metacarpophalangeal and wrist joint free, for angularly displaced little finger metacarpal shaft fractures which require reduction. Twenty-seven patients with a mean age of 25 (range, 16-39) years with little finger or combined ring and little finger displaced metacarpal shaft fractures were prospectively recruited. All were treated by closed fracture reduction and immobilization in the cast. Patients were followed up at 1 week, 4 weeks, and between 6 and 12 months. At each of these follow-up visits posteroa

Brachydactyly type C.

Brachydactyly type C is an autosomal dominant disorder with markedly variable penetrance. A patient with limited gene expression has shortening limited to the middle phalanges and the first metacarpal and an elongation of the radial side of the base of the second proximal phalanx. When completely expressed the characteristic radiographic findings are shortening of all the metacarpals, greatest in the thumb, followed by the little, ring, middle, and index fingers; brachymesophalangy (shortening of the middle phalanges); shortening of the proximal phalanges of the index and middle fingers, with

Hand fractures: a study of their site and type in childhood.

AIM: The purpose of the study was to document the site and type of fractures of the hand in children up to 16 years of age. MATERIALS AND METHODS: The radiographs of 280 children who were found to have a fracture in Accident and Emergency over a one-year period were reviewed. A total of 293 fractures of the hand and 3 distal interphalangeal joint dislocations without fracture were identified. There were 293 fractures as some children had more than one fracture. RESULTS: Distal tuft fractures were commonest in the 0 to 4-year age-group; fractures of distal phalanx (both transverse and tuft) i

Chondroblastoma of a metacarpal bone mimicking an aneurysmal bone cyst: a case report and a review of the literature.

Chondroblastoma of the metacarpal bone has been extremely rare and only seven cases have been reported in the English literature. Here we reported the eighth case of a chondroblastoma that developed on the first metacarpal bone of the right hand of a 21-year-old man. Radiographs showed an expansile osteolytic lesion with a multilocular appearance. In MR images, the lesion showed low intensity in T1 and high intensity in T2-weighted images with multiple fluid-fluid levels, which are findings resembling those of an aneurysmal bone cyst. From the pathological findings, however, it was recognized

Morphological character of the shoulder and leg skeleton in Przewalski's horse (Equus przewalskii).

The skeletal feature was observed in Przewalski's horse (Equus przewalskii) and the domestic horse (E. caballus). As results of the observations, remarkable differences were found in the scapula. The caudal border of the scapula in Przewalski's horse is curved in the caudal direction more than that of the domestic horse. Moreover, in the domestic horse, the sharp caudal border is formed by the shift of the outer muscular line (Linea muscularis) to the border. In Przewalski's horse, however, the caudal border is rounded because the outer muscular line does not shift to the border but lies besid
