Sesamoid correction achieved during the learning curve for Scarf-Akin osteotomy without lateral soft-tissue release: a single-centre prospective observational study.

INTRODUCTION: Scarf-Akin osteotomy (with or without lateral soft-tissue release (LSTR)) is commonly performed to treat hallux valgus (HV). An insufficient correction of sesamoids can be a risk factor for early recurrence of the condition. We set out to determine 1) the radiographic correction achieved after Scarf-Akin osteotomy performed without LSTR and 2) the degree of correction of sesamoids obtained during the learning curve of the technique. MATERIALS AND METHODS: In this prospective single-centre study, the first 25 feet (25 patients, mean age 55.2 years, 14 left) undergone Scarf-Akin o

[Effectiveness of double metatarsal osteotomy for severe hallux valgus with increased distal metatarsal articular angle].

OBJECTIVE: To summarize the technique and effectiveness of double metatarsal osteotomy for treating severe hallux valgus with increased distal metatarsal articular angle (DMAA). METHODS: Between June 2014 and December 2017, 64 patients (94 feet) of severe hallux valgus with an increased DMAA were treated with the double metatarsal osteotomy (distal metatarsal Reverdin osteotomy+proximal metatarsal open wedge osteotomy) combined with Akin osteotomy and soft tissue surgery to correct the deformity. There were 10 males (15 feet) and 54 females (79 feet) with an average age of 44.5 years (range,

Isolated gastrocnemius tightness.

BACKGROUND: Contracture of the gastrocnemius-soleus complex has well-documented deleterious effects on lower-limb function in spastic or neurologically impaired individuals. There is scarce literature, however, on the existence of isolated gastrocnemius contracture or its impact in otherwise normal patients. We hypothesized that an inability to dorsiflex the ankle due to equinus contracture leads to increased pain in the forefoot and/or midfoot and therefore a population with such pain will have less maximum ankle dorsiflexion than controls. We further postulated that the difference would be p

Foot pressure distribution: methodology and clinical application for children with ankle rheumatoid arthritis.

INTRODUCTION:: Foot pressure measurements furnish information about distribution of pressures, forces, time and contact areas under the foot during standing and walking. Foot pressure measurement has been used in a number of rehabilitation and athletic applications in adults, however, little has been published regarding the clinical usefulness of this technology for children with disabilities. Children with juvenile rheumatoid arthritis (JRA) are reported to have various foot deformities and gait deviations and clinicians report that the children may have foot pain such as metatarsalgia. These

Small-bowel erosion by a functioning peritoneal dialysis catheter.

BACKGROUND: Hallux valgus remains a common forefoot condition where different causes may contribute to the pathology. The extent of first ray mobility and, in particular, instability of the first tarsometatarsal joint represents a key argument in the debate on the selection of an appropriate operative treatment. Until now, assessment of first ray instability has relied on clinical examination or static assessment only. For dynamic evaluation in our study, pedobarographic, clinical, and standard weightbearing radiographic findings were correlated with the radiokinematically determined first ray

[Hallux valgus treated by shortening of the first phalanx with trans-epiphyseal impaction and adductor plasty. Apropos of 49 cases with 5-years follow-up].

PURPOSE OF THE STUDY: 49 cases of hallux-valgus were treated by a shortening of the phalanx with impaction according to the technic described by Regnauld, associated with a plasty of the adductor. Patients were evaluated with a follow-up greater than 5 years. MATERIAL AND METHODS: 32 female and 5 male patients were treated. 63 per cent patients were between 30 and 60 years old. The preoperative average of metatarsus varus was 12 degrees 1. The length of first metatarsal was inferior to the second in 30 cases. The preoperative average of first phalange valgus was 30 degrees. Every operated foo

Dorsiflexion contracture after the Weil osteotomy: results of cadaver study and three-dimensional analysis.

For metatarsalgia caused by a dislocated lesser metatarsophalangeal (MTP) joint and isolated over-long lesser metatarsals, surgical treatment options without sacrificing the joint are limited. Recently, the Weil osteotomy has been advocated for the treatment of this deformity. In our experience, preliminary results with this technique have revealed a high rate of dorsiflexion contracture of the MTP joints at follow-up. We performed a cadaver study and a three-dimensional analysis on sawbones to investigate this phenomenon. In the cadaveric portion of this study, the second MTP joints of two fr

Surgical options for salvage of end-stage hallux rigidus.

When approaching patients with a painful first MTP joint that has failed conservative therapy and first-line surgical treatments (cheilectomy or minor bunion procedures), the surgeon should stratify these patients based upon diagnosis, age, and activity level (Fig. 13). For the young, active patient, an arthrodesis is the gold standard, and the primary predictors of clinical and radiographic success are proper fusion angle alignment and maintenance or restoration of length. The method of fusion site preparation and the choice of fixation have not been found to be significant factors in achievi

Cementoplasty to cryoablation: review and current status.

Recent advances in percutaneous image-guided techniques have empowered interventional radiologists with diverse treatment options for the management of musculoskeletal lesions. Of note, there is growing utility for cementoplasty procedures, with indications ranging from stabilization of bone metastases to treatment of painful vertebral compression fractures. Likewise, cryoablation has emerged as a viable adjunct in the treatment of both primary and secondary bone and soft tissue neoplasms. These treatment options have been progressively incorporated into the multidisciplinary approach to holis

How does academia respond to the burden of infectious and parasitic disease?

BackgroundAcademic research is one of the main avenues through which humans can fight the threat of infectious diseases. However, there have been concerns regarding whether the academic system has provided sufficient efforts to fight infectious diseases we potentially face. Answering these questions could contribute to evidence-based recommendations for setting research priorities and third-mission policies.MethodsWith a focus on one of the most common categories of communicable diseases, infectious and parasitic diseases (IPDs), we searched Web of Science for articles and reviews relevant to
