Treatment of hyperfunctioning thyroid nodules with percutaneous ethanol injection: Eight years' experience.

The aim of our study was to define the long-term efficacy and safety of percutaneous ethanol injection (PEI) for the treatment of autonomous … trol, in 7 sessions (range 2-16). During PEI treatment, 26 toxic elderly patients were treated with methimazole and propranolol. Three possible outcomes were identified for statistical analysis: failure (persistent suppression of extra nodular tissue uptake, along with elevated free thyroid hormone and undetectable TSH levels); partial cure (normal free thyro …

Evaluation of potential thiamazole exposure of owners of orally treated hyperthyroid cats.

OBJECTIVES: The objective of this study was to evaluate the presence of traces of thiamazole in the urine of owners of hyperthyroid cats treated with antithyroid drugs. METHODS: Urine was collected from 24 owners of hyperthyroid cats, five human patients treated with thiamazole and five healthy humans without any contact with antithyroid drugs. All owners of hyperthyroid cats were asked to fill out a questionnaire. Urine of hyperthyroid cats was collected by spontaneous micturition. All urine samples were stored at -20°C until analysis by ultra-high-performance liquid chromatography coupled t

Development of Graves' disease during drug-free remission of juvenile dermatomyositis.

We report a Japanese boy with Graves' disease (GD) which developed during drug-free remission of juvenile dermatomyositis (JDM). He had been diagnosed with JDM at the age of 6 years by typical skin rashes, muscle weakness, elevated serum transaminase levels, and typical findings of both magnetic resonance imaging and muscle biopsy. Although anti-melanoma differentiation antigen 5 autoantibody was positive, there was no complication of interstitial lung disease. He showed good response to methylprednisolone pulse therapy followed by oral prednisolone in combination with weekly methotrexate (MTX

Graves' disease inducing a massive cardiac tamponade.

A 23-year-old woman was diagnosed with Graves' disease 5 months ago with decompensated thyroid function, for which she is taking thiamazole and propranolol. She developed progressive respiratory dyspnoea [New York Heart Association (NYHA) class III] and frequent palpitations. On emergency admission, the patient was tachypnoeic, hypotensive (77/54 mm Hg) and tachycardic (120 beats per minute), with an oxygen saturation of 94%. She also presented with cold, swollen and shaky extremities, with extended capillary filling time, and a significant reduction in heart sounds. Echocardiogram showed mass

Rare presentations of hyperthyroidism--Basedow's paraplegia and pancytopenia.

Typical presentations of hyperthyroidism are palpitation, nervousness, tremor, malaise, and weight loss. Hyperthyroidism affects nearly every system in the body, and some patients may manifest neurologic or hematologic symptoms. Atypical presentations of hyperthyroidism often pose a great challenge in diagnosis and treatment. We report a case of Basedow's paraplegia and pancytopenia that was precipitated by hyperthyroidism. The unusual manifestations led to unnecessary examinations and delayed the treatment of hyperthyroidism. The classical symptoms of Basedow's paraplegia are subacute symmetr

Hyperthyroidism as a cause of autonomic dysreflexia.

A 25-yr-old female tetraplegic patient experienced autonomic dysreflexia episodes involving hypertension, headache, facial flushing, and tachycardia. The symptoms were not related to the bladder or bowel. The episodes did not seem to be linked to any mechanical cause. The patient was incidentally diagnosed with hyperthyroidism. Treatment with antithyroid medication resulted in resolution of the autonomic dysreflexia. This case suggests that hyperthyroidism may trigger autonomic dysreflexia in tetraplegic patients.

Generalized convulsions with diffuse spike and wave bursts emerging with Graves' disease.

Thyroid hormone lowers the seizure threshold. We present the case of a female patient who developed generalized convulsions emerging with Graves' disease, and clarified the clinico-electroencephalographic characteristics. Before treatment, electroencephalography (EEG) showed 2- to 2.5-Hz diffuse spike and wave (SW) bursts, apparent during and after hyperventilation without any distinct absence seizure. Thiamazole treatment without any anticonvulsant led to a euthyroid state, convulsions resolved and EEG abnormalities normalized promptly. Clinical seizures were generalized tonic-clonic convulsi

[Long-term supraventricular tachycardia in a patient with triiodothyronine toxicosis].

There was presented a case of a therapy-resistant paroxysmal supraventricular tachycardia 130-140/min lasting about 100 days in a 21 years old barmaid drinking 5-6 glasses of natural coffee a day within last 3 years. Supraventricular tachycardia did not impair her working abilities; she was treated with propranolol within last 3 weeks before the admission to hospital. After 4 day therapy with 800 mg of quinidine, 10% potassium chloride and 75 mg of hydroxyzine performed electrocardioversion restored the sinus rhythm 90/min, but unfortunately supraventricular tachycardia returned 4 hours later.

[Hypokalemic periodic paralysis in thyrotoxicosis].

For the first time in Bulgaria a case of hypokalemic periodic paralysis in thyrotoxicosis is described. Three periods of paralysis were observed and documented in a hospital. The thyrotoxicosis was well manifested with disturbed carbohydrate tolerance and responded favourably to methizole treatment. It is suggested that the mechanism for the development of the hypokalemic periodic paralysis is the intracellular blockade of potassium by the surplus of thyroid hormones.

Preferential inhibition of thyroxine and 3,5,3'-triiodothyronine formation by propylthiouracil and methylmercaptoimidazole in thyroid peroxidase-catalyzed iodination of thyroglobulin.

The present study was undertaken to determine whether the thioureylene antithyroid drugs propylthiouracil [6-propyl-2-thiouracil (PTU)] and methylmercaptoimidazole [1-methyl-2-mercaptoimidazole (MMI)] have a specific inhibitory effect on the thyroid peroxidase (TPO)-catalyzed conversion of diiodotyrosine to T4 (coupling reaction) independent of their well known inhibitory effect on peroxidase-catalyzed iodination. We have employed model incubation systems containing highly purified TPO to examine this question. Most experiments were performed with a model iodination system containing TPO, low

Effects of antithyroid drugs on lymphocyte function in vitro.

The in vitro influence of 6-propylthiouracil (PTU) and methimazole (MMi) on mitogenic activation of human peripheral blood lymphocytes was studied in order to assess whether antithyroid drugs may have an immunomodulatory capacity. Lymphocytes were cultured for 72 h in the presence of phytohaemagglutinin (PHA; 1.25 microgram/ml), concanavalin A (Con A; 5.0 microgram/ml), or pokeweed mitogen (PWM; 2.5 microgram/ml). Antithyroid drugs were added to yield final concentrations of 1-1000 mu mol/liter. High pressure liquid chromatographic analyses verified that the expected drug concentrations in the…

Treatment of Graves' hyperthyroidism with thionamides: a position paper on indications and safety in pregnancy.

CONTEXT: Graves' disease affects 3% of women and 0.5% of men in the general population. The first line treatment of Graves' hyperthyroidism is based on the administration of antithyroid drugs (ATD), propylthiouracil (PTU), methimazole (MMI) and carbimazole. A recent warning from the Italian Drug Agency (Agenzia Italiana del Farmaco AIFA) reported the risk of MMI-induced acute pancreatitis. In addition, AIFA highlighted the possible association of MMI treatment during the first trimester of pregnancy with congenital malformations, thus recommending the use of effective contraceptive methods in …

Hyperthyroidism: an unusual case presentation.

Hyperthyroidism is the most common disorder of the thyroid. Patients typically present with complaints consistent with a hypermetabolic stat … ith unsuppressed thyroid-stimulating hormone levels. Subsequent treatment with the antithyroid drug methimazole (Tapazole) provided complete relief of symptoms. This case report illustrates how health care providers can be diverted to pursue a neurologic etiology when muscle weakness presents as a unilateral symptom.
