Traumatic perforations are associated with previous tympanostomy tube placement.

INTRODUCTION: Recognized late complications of tympanostomy tube placement include persistent tympanic perforation, tympanosclerosis, and focal atrophy. Based on clinical experience, we suspected that atrophy at healed tympanostomy tube sites might predispose to re-perforation following trauma. METHODS: Computerized record review of children seen in an academic pediatric otolaryngology practice from January 2001 to December 2020. RESULTS: 33 children with confirmed traumatic tympanic perforations and complete follow-up were identified. All perforations were unilateral. 19 of 33 (58%) perfora

Factors associated with retained tympanostomy tubes a case-controlled study.

OBJECTIVE: To determine factors associated with retained tympanostomy tubes in order to improve parent counseling on procedure risks and outcomes. METHODS: This is a case-control study; we conducted a retrospective chart review from 2012 to 2019 of cases of retained tympanostomy tubes compared to controls with confirmed tympanostomy tube extrusion. The study was conducted at a single tertiary care center, Boston Medical Center. A retained tympanostomy tube was defined as in place for more than two years requiring removal in the operating room. Cases were matched to two controls and assessed f

Effective but uncommon treatment of a spontaneous otogenic epidural pneumocephalus.

We describe an uncommon treatment for epidural pneumocephalus and an effective but uncommon treatment option. Complete and permanent relief was achieved in a young patient using a tympanostomy tube to eliminate excessive positive air pressure in the mastoid bone that was the cause of spontaneous pneumocephalus. The patient showed no recurrence and absence of clinical symptoms at outpatient follow-up. Post-traumatic, infectious or spontaneous pneumocephalus may require an active, usually surgical approach. Treatment with a tympanostomy tube can be an alternative in suitable patients.

Otological surgery in paediatric photosensitive patients.

There are a wide range of genetic and auto-immune conditions where UV light exposure poses a threat of UV irradiation to the external auditory canal, tympanic membrane and surrounding skin. Preoperative Ultraviolet Light (UV) measurements were taken in the operating theatre with standard operating microscope and an approved UV light meter prior to surgery on a patient with xeroderma pigmentosa. UV light meter readings of UV index 75 were taken at an operating distance of 290mm. Proceeding with otological surgery with a high UV index would result in a significant UV radiation burn. Utilising a

Shared Decision Making and Office Insertion of Tympanostomy Tubes.

Office insertion of tympanostomy tubes in young children without anesthesia is facilitated through shared decision making with families, including a candid discussion about anticipated pain (which is usually brief and transient) and how their child might react to being restrained. This commentary elaborates on the rationale for office tube insertion as an alternative to general anesthesia, including a clinical decision aid and suggestions for optimizing outcomes.

Identification of biofilms in post-tympanostomy tube otorrhea.

OBJECTIVES/HYPOTHESIS: Otitis media is a common problem in the pediatric population. Despite antibiotic therapy, post-tympanostomy otorrhea can be difficult to treat. Biofilms have been shown to play a role in chronic and recurrent otitis media and are implicated in otorrhea. This study investigated both the microbial composition and the presence of biofilm fragments rich in extracellular DNA (eDNA) and the bacterial DNA-binding protein, integration host factor (IHF), in post-tympanostomy tube otorrhea. STUDY DESIGN: Clinical samples. METHODS: Institutional review board approval was obtained

Effectiveness of the Ten-valent Pneumococcal Conjugate Vaccine Against Tympanostomy Tube Placements in a Cluster-randomized Trial.

BACKGROUND: We evaluated the impact of the new pneumococcal Haemophilus influenzae protein D conjugate vaccine (PHiD-CV10, GSK Vaccines) on tympanostomy tube placements (TTPs). METHODS: Finnish Invasive Pneumococcal disease vaccine trial was a nationwide phase III/IV cluster-randomized, double-blind trial. Children younger than 19 months received PHiD-CV10 in two thirds of clusters (N = 52) or hepatitis B or A vaccine as control in 26 clusters according to 3 + 1 or 2 + 1 schedules (infants younger than 7 months) or catch-up schedules. A secondary objective of the trial was to assess vaccine e

Interval vs massed training: how best do we teach surgery?

OBJECTIVE: To compare 2 different training paradigms, massed vs interval training, when novice students learn a surgical procedure, myringotomy with ventilation tube insertion, on a validated surgical simulator. STUDY DESIGN: Medical students were randomized into 2 training groups: the interval group (n = 19) was trained to perform the procedure in 5 trials/d over 3 days, and the massed group (n = 21) was trained to perform the procedure in 15 trials all in 1 session. One week later, all students were tested in 5 additional final trials. Pre- and posttest surveys were administered. SETTING:

Death following tonsillectomy in a child with Williams syndrome.

Williams syndrome (WS) is an uncommon genetic syndrome due to a deletion of several genes on chromosome 7. The syndrome is associated with dysmorphic facies, neurological manifestations, idiopathic hypercalcemia, and cardiac abnormalities, particularly supravalvular aortic stenosis (SVAS). Children with Williams syndrome may have chronic serous otitis media and/or obstructive sleep apnea. Hyperacusis is also commonly seen in these children. We report a case of sudden death at the time of tonsillectomy/adenoidectomy and bilateral tympanostomy tube placement in a child with Williams syndrome. Al

[The results of treatment of otitis media with suppuration in children by ventilation tubes].

The authors present the results of surgical treatment of secretory otitis media by means of drainage of tympanic cavity. The ventilation tubes used were made of Polish plastic Tarflen. The control assessments were carried out from 6 months to 6 years after the surgery, 58 children were studied, 49 of them had control examination. The improvement in hearing was found in 65 per cent of the patients. The appearance of tympanic membrane after the surgery was as follows: normal in 38.8%, retraction pockets in 10.2%, tympanosclerosis in 8.2%, secretory fluid in 6.1%, perforation of the tympanic memb

Audit of the incidence of persistent perforation of the tympanic membrane following T-tube removal or extrusion.

Audit of the incidence of persistent perforation of the tympanic membrane following T-tube removal or extrusion was carried out on 100 patients (151 ears). The incidence was found to be 11.9 per cent. This was independent of whether the tube was surgically removed or extruded; of the grade of surgeon carrying out the operation; and whether the patient treatment was carried out privately or on the N.H.S. Changes of practice have resulted from the audit.

Swimming with grommets.

A controlled prospective study was designed to find the relation of otorrhoea with swimming in ears with grommets. The children were allowed to swim in swimming pools without any precautions. They were advised not to swim during an upper respiratory infection. The results show that the incidence of otorrhoea among swimmers and non-swimmers after first 6 weeks post-operatively was remarkably similar.
