A 55-year-old man was admitted with paralysis of the left lower leg. He had purpura in the left lower extremity for three years, left calf pain for two years, and dysesthesia in the left plantar region and first toe for one year. A physical examination revealed livedo reticularis on the left leg and mononeuritis multiplex was diagnosed in the bilateral tibial and left peroneal nerve area. Anti-neutrophil cytoplasmic antibody was negative. A nerve conduction study showed decreased amplitude of compound muscle-action potential in the bilateral tibial and the left peroneal nerve, sensory nerve ac

OBJECTIVE: The objective of this study was to investigate the usefulness of muscle ultrasound for evaluating muscle changes caused by denervation in carpal tunnel syndrome (CTS), which is a focal neuropathy.
METHODS: The mean and standard deviation (SD) of echo intensity (EI) in the thenar and hypothenar muscles were calculated in 35 patients with CTS and 11 healthy subjects. Patients were assigned to three subgroups based on CTS severity as determined by electrodiagnostic tests. The ratio of thenar muscle pixel brightness to hypothenar muscle pixel brightness was used in statistical analyses

BACKGROUND: Proximal lesions of the sciatic nerve are often difficult to diagnose and to treat properly. In particular, if there are posttraumatic or postoperative alterations, imaging might not identify the level and location of lesion. Due to the sciatic nerve anatomy, the same is true for clinical and electrophysiological evaluation with a risk of delayed surgery and, thus, unsuccessful surgery. Therefore, in some unclear cases, surgical exploration of the whole sciatic nerve and its divisions could be the only means to determine the correct diagnosis and allow prompt treatment to produce t

Slimmer's paralysis is a peripheral mononeuropathy of the common peroneal (fibular) nerve (CPN/CFN), typically associated with rapid weight loss resulting in loss of subcutaneous fat pad and subsequent neural compression at the fibular head. Here, we describe a young man with a 1-year history of right-sided foot drop, which developed following a rapid intentional weight loss of 11 kg over a period of 15 days. This weight loss was preceded by rapid weight gain over 2 days owing to binge eating. Laboratory parameters for systemic/metabolic disorders were unremarkable. Radiology of the lumbosacra

Dengue fever is a mosquito-borne viral infection that spreads through Aedes aegypti mosquito and is a prevalent endemic disease in tropical countries such as India. Infecting approximately 50 to 100 million people annually in more than 100 countries, dengue as a disease is considered as a great public health challenge. Classically presenting symptoms include fever, arthralgia, headache, myalgia, and rash, but the incidence of neurological manifestations has recently been increasing due to changes in the virological characteristics of the dengue virus. Still, cranial mononeuropathy, involving t

A 72-year old woman, who had a history of psoriasis and psoriatic arthritis from age of 69, was admitted because of acute progression of dyspnea and generalized muscle weakness after initiation of ustekinumab. She had been diagnosed as having lung and eye sarcoidosis ten months before admission. Nerve conduction studies revealed multiple mononeuropathy and needle electromyography showed myogenic changes with spontaneous activities. Muscle pathology showed non-caseating epithelioid granuloma and high expression of HLA-class I in myofibers. Diagnosis of sarcoid myopathy and neuropathy aggravated

INTRODUCTION: Eosinophilic granulomatosis with polyangiitis (EGPA) patients with mononeuropathy multiplex often visit orthopedic clinics; however, orthopedic surgeons have limited experience in diagnosing EGPA because of its rarity. We report a case of EGPA that required 1 month to confirm the diagnosis.
CASE REPORT: A 48-year-old woman presented with acute onset numbness in the right lower extremity. She had muscle weakness of the right lower extremity; lumbar spine magnetic resonance imaging showed lumbar disc herniation. Despite conservative treatment, her symptoms worsened. Blood tests sh

The aim of this study was to record all patients referred to a private outpatient clinic for electrodiagnostic consultation during one calendar year. The demographic data of the patients, the specialty of the referring physicians, the referral diagnosis, the electromyographic (EMG) diagnosis and the concordance of referral and EMG diagnosis among all the referring physician's specialties were recorded. There were one thousand patients (385 men and 615 women). 65.7% of the patients were referred from orthopedists and 22.4% from neurologists. EMG was normal in 498 patients (49.8%). Abnormal in 4

BACKGROUND: Bell's palsy (BP) is the most common acute mononeuropathy of unilateral facial paralysis. Immune, infective and ischaemic mechanisms are potential contributors to the development of BP, but the precise cause remains unclear. Recently, oxidative stress has been proposed as a risk factor of various idiopathic diseases. The aim of this study was to investigate the possible role of oxidative stress in patients with BP.
METHODS: Thirty-two patients with BP and 30 healthy controls were included in this study. Serum total oxidant status (TOS), total antioxidant status (TAS) and oxidative

Hereditary Neuropathy with Liability to Pressure Palsy (HNPP) is a rare condition in childhood with a diverse range of clinical presentations. We analyzed the clinical presentation and electrophysiological data of 12 children with a confirmed PMP22 gene deletion and reviewed the published reports of HNPP in children and compared our data with the reports from the literature review. Peroneal palsy was the most common presentation (42%) followed by brachial plexus palsy in 25% of our cases. Nerve conduction studies were always suggestive of the diagnosis demonstrating 3 major patterns: multifoca

INTRODUCTION: High-resolution ultrasound (US) of the peripheral nerves is now a standard means of assessing neuromuscular disorders in many centers. Currently used in conjunction with electrodiagnostic (EDX) studies, nerve US is especially effective in the diagnosis of entrapment neuropathies.
AREAS COVERED: This article reviews the basic physics of peripheral nerve US, guidelines for its current use and future directions. Advantages of using nerve US alongside EDX studies are outlined along with current limitations of testing. The role of US in the diagnosis of entrapment neuropathy is empha

Neurological complications whether due to the uremic state or its treatment, contribute largely to the morbidity and mortality in patients with renal failure. Despite continuous therapeutic advances, many neurological complications of uremia, like uremic encephalopathy, atherosclerosis, neuropathy and myopathy fail to fully respond to dialysis. Moreover, dialytic therapy or kidney transplantation may even induce neurological complications. Dialysis can directly or indirectly be associated with dialysis dementia, dysequilibrium syndrome, aggravation of atherosclerosis, cerebrovascular accidents

BACKGROUND: Carpal tunnel syndrome is the most common mononeuropathy. Typical symptoms include pain or discomfort in the hand, paresthesia, weakness and altered temperature or dryness of the skin along with neurophysiological findings. The presence of these symptoms in the area of the median nerve distribution supports the diagnosis.
MATERIAL AND METHODS: This review article is based on current knowledge about carpal tunnel syndrome as obtained from the Medline and the Cochrane database systems.
RESULTS: The prevalence of carpal tunnel syndrome in the population is around 3%. A higher incide

An apparent "outbreak" of cumulative trauma disorders (CTDs) was noted in a subset of white-collar workers employed in medical illustration and medical graphic arts. Among graphic artists (n = 7), there were three cases of carpal tunnel syndrome (CTS), based on patient reports of personal physician diagnoses; no such reports occurred among other workers in the department (n = 39, p = .0023, Fisher's exact test). Jobs were studied to identify possible risk factors for CTDs, and workers underwent a standardized medical screening protocol. Specific work elements were found to be performed more fr

Rapidly progressive glomerulonephritis (RPGN) is rarely associated with macroscopic polyarteritis nodosa (PAN), as seen in this series of 7 out of 235 patients. The clinical symptoms of PAN were as follows: myalgias 6; fever 1; arthralgias and mononeuropathy multiplex 5; cutaneous vasculitis 3; arterial hypertension 4, 3 of which were malignant. The mean proteinuria was 2.7 g/24 h; creatininemia 458 microM/l; microscopic hematuria was present in 4 of the 7 patients; only 1 patient was anuric. Markers of hepatitis B virus were absent in all cases. Arteriography revealed microaneurysms and renal

A 22-year-old male with a typical history of pauci-symptomatic COVID-19 3 weeks earlier, confirmed by positive serology for SARS-CoV-2 (IgG), was admitted to the intensive care unit because of severe myocarditis with refractory cardiogenic shock that required extracorporeal life support. Due to a clinical presentation suggestive of Kawasaki-like disease with coronary aneurysm and severe systemic inflammation, intravenous immunoglobulins were administered in combination with tocilizumab. The initial clinical course was favourable with these treatments. However, the patient subsequently develope

BACKGROUND: Femoral nerve lesion causes significant disability. In many cases, the availability of the proximal stump is in question and further complicates surgical management by severely limiting reconstructive options and precluding nerve graft reconstruction. The purpose of this report is to describe the successful restoration of quadriceps function by distal nerve transfer at the level of the thigh without functional donor morbidity and the findings of cadaveric dissections of the obturator and femoral nerve branches.
METHODS: Eight fresh frozen cadaveric lower limbs were dissected at th

Patients presumed to have lower limb symptoms localizing to the lumbar or lumbosacral plexus require rigorous electrophysiological evaluation. Entities that cause lumbosacral plexopathies may be patchy, asymmetrical and more diffuse than initially suspected. As a result, bilateral nerve conduction studies and needle examination outside those routinely tested and clinically affected may be needed to document the extent of involvement including needle examination of the thoracic paraspinals and consideration of upper limb studies. This article outlines the lumbar and lumbosacral plexus anatomy,
