Fractures of the Humeral Ulnar Condyle in Children - a Rare but Complicated Injury.

BACKGROUND: Ulnar humeral condyle fractures are rare paediatric elbow fractures, classified as Salter-Harris IV paediatric elbow injuries. Due to constant radiological changes in the elbow with varying manifestation of ossification centres as well as late ossification of the trochlea, diagnosis of these injuries is challenging. To avoid long-term complications, the treating surgeon should be familiar with the rare injury picture, diagnostics and adequate therapeutic measures. MATERIAL AND METHODS: The present retrospective study includes data on all paediatric cases from 2002 to 2019 with pri

Periprosthetic Tibial Fracture with Nonunion and Ascending Prosthetic Joint Infection: A Case Report of an Individual Treatment Strategy.

INTRODUCTION: Chronic recurrent multifocal osteomyelitis (CRMO) is an idiopathic inflammatory disorder of bone seen primarily in children and adolescents. It is part of the clinical picture of non-bacterial osteomyelitis and typically presents a relapsing course with both remissions and spontaneous exacerbations. CRMO is typically seen in the metaphysis of long bones. Usually, the clinical symptoms include painful swellings of the affected regions. It is being reported with increasing frequency but many cases of this disease go unreported. No infective agent has been identified and antibiotics

Functional capacity evaluation of patients with mucopolysaccharidosis.

INTRODUCTION: The mucopolysaccharidoses (MPS) are rare genetic disorders caused by a deficiency in lysosomal enzymes that affect the catabolism of glycosaminoglycans and cause their accumulation, resulting in a multisystemic clinical picture. Their clinical manifestations result in limited ability to perform daily life tasks. OBJECTIVES: To evaluate functional capacity and joint range of motion (ROM) in patients with MPS followed at the reference center for lysosomal disorders at Hospital de Clínicas de Porto Alegre, Brazil. METHODS: This was a prospective longitudinal study with a convenien

Stress-induced cardiomyopathy (Tako-Tsubo syndrome) in Austria.

BACKGROUND: Tako-Tsubo syndrome (TS) is a still rarely diagnosed clinical syndrome, which is characterized by acute onset of chest pain, transient cardiac dysfunction with (frequently) reversible wall motion abnormalities (WMAs), but with no relevant obstructive coronary artery disease. METHODS AND RESULTS: Among 179 consecutive patients with proven diagnosis of TS that were retrospectively analysed in this multicentre registry, women represented the majority of patients (94%) while only 11 men (6%) developed TS. Mean age was 69.1±11.5 years (range 35-88 years). Cardinal symptoms of TS, which

Induction of interleukin 6 (IL-6) by hypoxia in vascular cells. Central role of the binding site for nuclear factor-IL-6.

The pathologic picture in ischemic tissue injury shares features with the inflammatory response, including production of proinflammatory cytokines. Hypoxia-mediated induction of interleukin-6 (IL-6), a cytokine with anti-inflammatory properties, could set in motion mechanisms limiting inflammation in ischemia. Exposure of cultured endothelial cells (ECs) to H (pO2 approximately 12-16 torr) increased transcription of IL-6, elevated levels of IL-6 mRNA, and induced elaboration of IL-6 antigen in a time-dependent manner. Exposure of mice to hypoxia increased IL-6 transcripts in the lung, and immu

Iatrogenic Dextrose Hyperosmolar Extravasation Injury in Suspected Endogenous Hyperinsulinism.

Hyperosmolar dextrose infused through peripheral venous access can extravasate into surrounding soft tissue and produce local injury. We present the case of a 44-year-old woman with a history of seizure disorder and hyperlipidemia who underwent anterior lumbar interbody fusion (ALIF). On the day of admission, she experienced a witnessed generalized seizure on the floor that terminated following administration of intravenous lorazepam. Point-of-care glucose obtained during the event was 39 mg/dL and corrected with intravenous dextrose. She was transferred to the intensive care unit (ICU), where

Dynamic view of an allosteric intermediate in a positively cooperative dimer.

UNLABELLED: Positive cooperativity in ligand binding is a hallmark of allosteric oligomers, yet how the first binding event enhances the second remains obscure, because the pivotal singly-bound intermediate (lig₁) is thermodynamically disfavored and rarely accumulates. Distinguishing concerted (MWC) from sequential (KNF) mechanisms turns on one question: when a ligand binds one protomer, does its empty partner change conformation? Here we resolve this for the allosteric homodimer chorismate mutase (CM) using mixed-labeled dimers (MLDs), in which a single maleimide crosslink stabilizes a hetero

A multi-country time and motion study to describe the experience and burden associated with the treatment of Fabry disease with enzyme replacement therapy with agalsidase alfa and agalsidase beta.

BACKGROUND: Fabry disease (FD) is a rare inherited X-linked lysosomal disorder caused by the deficiency or dysfunction of the enzyme α-galactosidase. This leads to a detrimental accumulation of globotriaosylceramide (Gb3) within multiple cell types. Enzyme replacement therapies (ERTs), including agalsidase alfa and agalsidase beta, can diminish Gb3 levels. Published real-world data on the time, cost and burden associated with the administration of ERTs are limited. These evidence gaps were addressed by generating real-world data quantifying the burden of agalsidase alfa and beta infusions for

Recurrent Vestibular Symptoms Not Otherwise Specified: Clinical Characteristics Compared With Vestibular Migraine and Menière's Disease.

Despite the huge progress in the definition and classification of vestibular disorders within the last decade, there are still patients whose recurrent vestibular symptoms cannot be attributed to any of the recognized episodic vestibular syndromes, such as Menière's disease (MD), vestibular migraine (VM), benign paroxysmal positional vertigo (BPPV), vestibular paroxysmia, orthostatic vertigo or transient ischemic attack (TIA). The aim of the present international, multi-center, cross-sectional study was to systematically characterize the clinical picture of recurrent vestibular symptoms not ot

[Semilunar bone necrosis (Kienböck's disease) - first clinical results after free microvascularised bone graft from the distal femur].

BACKGROUND: Idiopathic aseptic osseous necrosis of the semilunar bone is also called Kienböck's disease after Robert Kienböck who firstly described this disease. The clinical picture is characterised by a stage-like course. Internationally the classification according to Lichtman and Ross has achieved the most acceptance. The actual therapy is dependent on the present disease stage. Basically, various pressure-relieving operative procedures are possible, on the other hand operative interventions via revascularisation represent a therapeutic option. In the case of advanced disease only, "salvag

Global spiral patterns in galaxies: complexity and simplicity.

A coherent exposition of the density wave theory of galactic spirals is presented in a recent monograph. It is centered on the working hypothesis of quasi-stationary spiral structure, a possibility first proposed by Bertil Lindblad. This hypothesis has since been found to be widely applicable in a number of physical contexts, including the explanation of the Hubble classification system and other categorical classes. Direct empirical support of this hypothesis has been provided especially by the regularity of the infrared images frequently observed in a number of galaxies (e.g., NGC 309), and

[Magnetic resonance imaging of seronegative sacroiliitis].

INTRODUCTION: The inflammatory involvement of the sacroiliac joint is frequent during seronegative spondylarthritis. The clinical diagnosis of sacroiliitis may be very difficult, especially in the early stage, because joint motion cannot be assessed directly and the clinical picture is very similar to that of lumbar pain. Conventional radiography is negative as long as the structural change in the joint is limited to the synovial membrane and the cartilage (early stage). Computed Tomography (CT) also has many drawbacks, and thus the changes can be shown only when chondritis and enthesitis have

Rheumatoid arthritis: sequences.

OBJECTIVE: Rheumatoid arthritis (RA) is an autoimmune disorder of unknown etiology characterized by symmetric, erosive synovitis and sometimes multisystem involvement. It affects 1% of the adult population and exhibits a chronic fluctuating course which may result in progressive joint destruction, deformity, disability and premature death. We review the literature data relative to the peculiar pathologic features of the disease shown by diagnostic imaging techniques. METHODS: All our patients were classified according to the diagnostic criteria of the American Rheumatism Association (1987). P

Hepatic radiofrequency ablation induces widespread cellular activation throughout the liver.

OBJECTIVE: We investigated the extent of cellular, transcriptional, and translational activation throughout the liver following radiofrequency ablation (RFA). MATERIALS AND METHODS: RFA of the healthy liver was performed in two 8-10-week-old male C57/Bl6 mice, no/sham procedure in one. One and 7 days after, single-cell RNA sequencing (scRNAseq) was performed on distant, untreated liver to examine > 6,000 genes from normalized datasets of > 6,000 cells/sample, enabling identification of ten major cell populations. We defined cell-to-cell interactions by CellphoneDB and identified active pathwa

[Long-term Results of Arthrodesis of the Talocrural Joint.].

The authors evaluate a group of 19 arthrodeses of the talocrural joint in 16 patients after a mean interval of 6 years and three months following arthrodesis (operation). They demonstrate that the hitherto used method of evaluation of the position of the foot after arthrodesis of the ankle - measurement of the tibiotalar angle, is insufficient and does not record the changes in function and shape distally from Chopart's joint, e. g. pes cavus. Some deformities of the foot can lead to an entirely afunctional arthrodesis even when from the theoretical aspect the tibio-talar angle of arthrodesis

[Pathological physiology of gastroesophageal reflux disease. Hypothesis (Literature review)].

UNLABELLED: Currently prolonged pH-monitoring is considered as Gold standard for diagnosis of Gastro Esophageal Reflux Disease (GERD). Criteria and norms based on prolonged pH-monitoring were the basic concepts of pathophysiology of GERD. For example, it is accepted that esophageal hernia (EG) can be present without GERD, as well as GERD without EG. X-ray diagnosis compared to the pH- monitoring has a low sensitivity (70%) and specificity (74%). Meanwhile, in recent decades, it has been found that the pH-metry is not effective in a non-erosive reflux disease. We figured that the criteria and n
