Infection and mucosal injury in cancer treatment.

The oral and gastrointestinal mucosa acts as an important mechanical barrier that prevents local or systemic invasion by microorganisms. Cytotoxic chemotherapy-induced mucosal injury (MI) of oral cavity and intestinal epithelium occurs in many patients treated for malignancy. Compromise of the mucosal barrier can contribute to local invasion by colonizing microorganisms and, subsequently, to systemic infection. Historically, gram-negative bacteremia has been the most problematic bacterial infection in neutropenic patients, but its incidence has reduced over time because of the use of prophylac

The role of M cells in mucosal immunity.

Mucosa-associated lymphoid tissue in the respiratory and digestive tracts are covered by a specialized epithelium, the follicle-associated epithelium, which includes M cells, which are specialized for the uptake and transcytosis of macromolecules and microorganisms. Following transcytosis, antigens are released to cells of the immune system in lymphoid aggregates beneath the epithelium where antigen processing and presentation and stimulation of specific B and T lymphocytes are achieved. Circulation of the lymphoid cells enables their homing to their original, and other, mucosal sites where th

[Dermato-mucosal manifestations of Behçet's disease].

Oral and genital aphthae are the main clinical dermatologic manifestations of Behçet's disease. They look like those that occur in other aphthosis. Cutaneous lesions include pseudofolliculitis, folliculitis, erythema nodosum-like lesions, Sweet's-like lesions and pyoderma gangrenosum-like lesions. Histologically, these lesions are frequently perivascular with proeminent infiltrates of neutrophils and/or lymphocytes. Hypersensibility to needle pricks is explored by the pathergy test which sensibility is highly variable depending on the countries. When there is no systemic lesions requiring oral

Oral presentation of an oesophageal mucosal tear.

Tears of the oesophageal wall following sudden forceful vomiting are well documented in literature. In Boerhaave's syndrome there is transmural rupture associated with complications including pneumothorax, pneumomediastinum, surgical emphysema and shock. In Mallory-Weiss syndrome mucosal tears are associated with haematemesis and shock. In neither of these conditions has intraluminal obstruction been described as an aetiological factor. We present a case with similar pathophysiology where oesophageal obstruction by a meat bolus followed by forceful vomiting led to an oesophageal mucosal tear a

Review article: oesophageal mucosal resistance.

The human oesophagus is lined by a moist, partially keratinized, stratified squamous epithelium, one important property of which is to serve as a barrier between the outside (luminal) world and the internal world of the organism. This phenomenon in clinical parlance is known as 'oesophageal mucosal resistance'. This article details the structure and functions of the oesophageal mucosa that contribute to its defence against injury upon exposure to refluxed gastric acid.

Oral mucosal wetness in hypo- and normosalivators.

After a person swallows, a film of residual saliva covers the oral hard- and soft-tissue surfaces. Here, the thickness of this film was measured at 11 selected mucosal surfaces on each side of the mouth (22 sites total) in two groups of dry-mouth and one group of normal individuals. Each group contained 25 individuals; one of the dry-mouth groups had resting flow rates < or = 0.1 ml/min while the other and the normal had flow rates above that. Residual saliva thickness was determined by placing frying-pan-shaped filter-paper strips (Sialopaper) against the mucosa at each site for 5 s and measu

[The mechanisms of gastrointestinal mucosal injury and repair].

Peptic ulcer disease are usually accompanied by diffuse inflammation over the gastroduodenal mucosa in addition to severe local inflammation at the site of ulceration. It is well known that inflammatory cytokine are the main mediators of inflammation. Cytokines may also play a part in acute gastroduodenal mucosal lesions (AGML) caused by NSAID, H. pylori, and stress. Among cytokines most involved in AGML, tumor necrosis factor-alpha (TNF-alpha), interleukin (IL) 1, IL-6, and IL-8 modulate chemotaxis, chemokinetics, and aggregation and release of lysosomal enzymes from neutrophils. Ulcer healin

Buccal mucosal grafts for urethral reconstruction.

OBJECTIVES: Patients requiring urethral reconstruction but who have a paucity of usable genital tissue present a considerable technical challenge. Herein we report the experience of three centers in the use of buccal mucosa for urethral replacement. METHODS: From 1991 to 1996, 22 urethral reconstructions were completed using a graft of buccal mucosa. Eighteen of 22 patients had previously failed hypospadias repairs, while three had bulbar urethral stricture and one had penile carcinoma. Grafts were taken from either the inner cheek or the lower lip, and seven were used as a combined graft. On

Morphology of the mucosal lesion in gluten sensitivity.

Gluten sensitivity is associated with a spectrum of mucosal lesions, arbitrarily termed pre-infiltrative, infiltrative-hyperplastic, flat-destructive and atrophic-hypoplastic. Histologically and immunohistologically these lesions are all compatible with T-cell-driven events operative at a local mucosal level. They are classifiable either in terms of antibody titres (pre-infiltrative) (see Chapter 10) or by the characteristic disposition of IELs throughout the surface and crypt epithelium. From in-vivo challenges, it has been demonstrated: (i) that all these lesions comprise a dynamically inter

Mucosal mantle cell (centrocytic) lymphomas.

The morphology, phenotype, genotype and clinical behaviour of four cases of mantle cell lymphoma (centrocytic lymphoma) presenting primarily in mucosa (two gastric, one in large bowel and one tonsillar) are reviewed. Their relationship with the broader group of mantle cell and mucosa-associated lymphoid tissue (MALT) lymphomas is also discussed. All four tumours showed a monomorphic picture of mantle cells (centrocytes) arranged in a diffuse, or vaguely nodular, pattern. Scattered non-neoplastic germinal centres were entrapped within the tumour cells, although there was no follicular colonizat

Identification of buccal mucosal mucin receptor.

A receptor for salivary mucin was isolated from epithelial cell membrane of buccal mucosa by a procedure involving membrane solubilization with octylglucoside followed by affinity chromatography on Sepharose-bound wheat germ agglutinin. The receptor protein displayed an apparent molecular weight of 97kDa and exhibited binding specific to mucin in a concentration-dependent manner. The receptor, furthermore, showed a requirement for carbohydrate chains in mucin for binding, as their removal caused a marked (89%) reduction in binding capacity. Scatchard analysis revealed a linear plot with a sing

Neuromediators in aging and gastric mucosal injury.

The hypothesis that the improvement of neurochemical changes involving the brain dopaminergic and cholinergic systems in aged rats may be related to the their capacity of coping with stress has been investigated by testing the effect of various dopaminergic and cholinergic drugs on stress-induced gastric ulcer development. The model of cold-plus-restraint stress induced gastric lesions has been used in Sprague-Dawley rats aged 24 months, compared to young animals aged 2 months. An increased incidence of gastric ulcers was found in old animals. When these animals were treated with cholinergic o

Vagal nerve and the gastric mucosal defense.

An essential role for an intact vagal nerve has been proven in the development of gastric mucosal cyto- and general protection. On the other hand, chemically-induced (ethanol, HCl, indomethacin) gastric mucosal damage is enhanced after acute surgical vagotomy. The aims of this paper were to study the possible mechanisms of the vagal nerve in the development of gastric mucosal defense. The following questions were addressed: 1) effect of surgical vagotomy on the development of ethanol- (ETOH), HCl-, and indomethacin (IND)-induced gastric mucosal damage: 2) changes in the gastric mucosal defense

Reimplantation of the ureter in a transtrigonal mucosal groove.

Reimplantation of the ureter into a groove of the bladder mucosa has been used to simplify the procedure of reimplantation and overcome problems in trabeculated bladders by obviating the creation of a submucosal tunnel. Further experience is presented regarding this procedure on 40 ureters of patients with unilateral or bilateral vesicoureteric reflux, ureteric orifice(s) stenosis, bladder diverticula and posterior urethral valves submitted to simultaneous resection. Intra-operative observations, post-operative cystography and long-term results led to the conclusion that the procedure is techn
