Addressing the Global Disparities in Access to Treatment in Spinal Muscular Atrophy.

The advent of novel disease-modifying therapeutics for spinal muscular atrophy (SMA) increased life expectancy with better motor function and potentially better quality of life. While the benefits of these therapies are well established, most trials were conducted only in high-income and middle-income countries, and there is a lack of global representation. Furthermore, although these medications are now approved in over 50 countries worldwide, they remain unavailable to many who need them, potentially widening the gap in clinical care. Moreover, with these therapies, the standard SMA phenotyp

Humanistic and Economic Burden of Spinal Muscular Atrophy in the Gulf Region.

BACKGROUND: Spinal muscular atrophy (SMA) is a severe, fatal genetic disorder. This study aimed to quantify the humanistic and economic burden of SMA in four Gulf countries, the Kingdom of Saudi Arabia (KSA), the United Arab Emirates (UAE), Oman, and Bahrain, to provide decision-makers with evidence-based strategies for burden mitigation. METHODS: A burden of disease model was developed to quantify SMA's humanistic burden via disability-adjusted life years (DALYs) and its economic burden through direct healthcare costs and indirect productivity losses experienced by patients and caregivers.

Spinal Muscular Atrophy-Survivorship and Care in a New Therapeutic Landscape.

Spinal muscular atrophy (SMA) is an inherited lower motor neuron disorder resulting from autosomal recessive variants in the survival motor neuron protein (SMN1) located on chromosome 5q. Previously the most common inherited cause of infant mortality, the development of SMN-restoring disease-modifying therapies combined with the expansion of newborn screening programs for SMA, have improved motor function and survival. With improved function and survival, new questions are emerging around the role of combining multiple SMN-restoring therapies or SMN-restoring therapies with newer therapeutic a

Pushing the boundaries: future directions in the management of spinal muscular atrophy.

Spinal muscular atrophy (SMA) is a devastating, degenerative, paediatric neuromuscular disease which until recently was untreatable. Discovery of the responsible gene 30 years ago heralded a new age of pioneering therapeutic developments. Three disease-modifying therapies (DMTs) have received regulatory approval and have transformed the disease, reducing disability and prolonging patient survival. These therapies - with distinct mechanisms, routes of administration, dosing schedules, side effect profiles, and financial costs - have dramatically altered the clinical phenotypes of this condition

In utero therapy for spinal muscular atrophy: closer to clinical translation.

5q-Spinal muscular atrophy (SMA) has been a trailblazer in the development of advanced therapies for inherited diseases. SMA is an autosomal recessive disorder affecting mainly motor neurons in the anterior horn of the spinal cord and brainstem motor nucle but currently considered a systemic disease. Advances in understanding the genetics of SMA led to the development of disease-modifying therapies, either transferring a healthy version of SMN1, the causative gene absent or altered in SMA, or modulating SMN2, a highly homologous but less functional version of SMN1, present in all patients. Nondiabetic Ketoacidosis in a Patient With Spinal Muscular Atrophy Type II.

BACKGROUND: Spinal muscular atrophy (SMA) is an autosomal recessive disorder characterized by insufficient levels of the survival motor neuron protein; this promotes progressive loss of motor neurons leading to muscular atrophy. Patients with SMA also have metabolic abnormalities that can predispose them to develop ketoacidosis. The objective of this report is to describe a 51-year-old female with SMA type II who presented with nondiabetic ketoacidosis requiring hospitalization. CASE PRESENTATION: A 51-year-old female with SMA type II presented with 1 day of diarrhea, decreased oral intake, n

Managing Spinal Muscular Atrophy: A Look at the Biology and Treatment Strategies.

Since its discovery in the late 19th century, spinal muscular atrophy (SMA) has had a significant medical and societal impact, primarily affecting newborns, toddlers, and young adults. While new pharmaceutical strategies are effective in treating SMA in a particular subset of patients, continued research is necessary to improve the well-being of patients. Treatments are needed for those who do not respond to newly approved drugs and older patients with significantly compromised neuron systems. After summarizing SMA genotypes, phenotypes, and approved pharmacological treatments, this review pre

State of the art: Pregnancy in spinal muscular atrophy in the treatment era.

An increasing number of adults with spinal muscular atrophy (SMA) wish to become parents. New disease-modifying therapies (DMT) have improved health outcomes and are expected to reduce disability in adults with SMA, but their current label prevents their use in pregnancy. While there is some information on pregnancy outcomes in the pre-DMT era, little has been published recently, and no ubiquitously accepted guidelines exist. Nonetheless, it is crucial to provide knowledgeable and open counselling, ideally in the context of treatments. Counseling for both adolescent and adult patients should i

Advances in the Potential Role and Mechanism of Fibroblasts in Spinal Muscular Atrophy.

Spinal muscular atrophy (SMA) is the most common genetic disease leading to infant mortality, primarily characterized by the deficiency of survival motor neuron (SMN) protein. The effects of SMA are not limited to the nervous system but also encompass multiple cell types. Fibroblasts have been extensively employed as primary disease model cells in SMA pathophysiological studies. Here, we present a comprehensive summary of the pivotal roles fibroblasts play in SMA research, focusing on how SMN deficiency modulates the response characteristics of fibroblasts. Our findings reveal distinct reactiv

Considerations for Treatment in Clinical Care of Spinal Muscular Atrophy Patients.

Spinal Muscular Atrophy is a neurodegenerative disease which can lead to muscle weakness, paralysis, and in some cases death. There are many factors that contribute to the severity of symptoms and those factors can be used to determine the best course of treatment for the patients. We looked through published literature to create a set of considerations for treatment in patients with Spinal Muscular Atrophy including age, type, SMN2 copies, and any familial considerations. This can serve as a guide for what to consider in the treatment of SMA patients clinically.

Reverse Split Hand as a Neurophysiological Hallmark of Spinal Muscular Atrophy.

Objective: Motor unit number estimation (MUNE) methods are crucial for estimating lower motor neuron loss in motor neuron diseases. The MScanFit MUNE (MScanFit) is a novel method that estimates MUNE values from compound motor action potential (CMAP) scans, demonstrating high sensitivity and reproducibility in detecting motor unit loss in amyotrophic lateral sclerosis (ALS) and spinal muscular atrophy (SMA). In this study, we aimed to characterize the pattern of motor unit loss in the hand intrinsic muscles of SMA patients compared to ALS patients and healthy controls (HC) using MScanFit MUNE.

Protein biomarker signature in patients with spinal and bulbar muscular atrophy.

Spinal and bulbar muscular atrophy (SBMA) is a slowly progressing disease with limited sensitive biomarkers that support clinical research. We analyzed plasma and serum samples from patients with SBMA and matched healthy controls in multiple cohorts, identifying 40 highly reproducible SBMA-associated proteins out of nearly 3,000 measured. These proteins were robustly enriched in gene sets of skeletal muscle expression and processes related to mitochondria and calcium signaling. Many proteins outperformed currently used clinical laboratory tests (e.g., creatine kinase [CK]) in distinguishing pa

Base editing rescue of spinal muscular atrophy in cells and in mice.

Spinal muscular atrophy (SMA), the leading genetic cause of infant mortality, arises from survival motor neuron (SMN) protein insufficiency resulting from SMN1 loss. Approved therapies circumvent endogenous SMN regulation and require repeated dosing or may wane. We describe genome editing of SMN2, an insufficient copy of SMN1 harboring a C6>T mutation, to permanently restore SMN protein levels and rescue SMA phenotypes. We used nucleases or base editors to modify five SMN2 regulatory regions. Base editing converted SMN2 T6>C, restoring SMN protein levels to wild type. Adeno-associated virus se
