Duration of intrapartum antibiotics for group B streptococcus on the diagnosis of clinical neonatal sepsis.

BACKGROUND: Infants born to mothers who are colonized with group B streptococcus (GBS) but received <4 hours of intrapartum antibiotic prophylaxis (IAP) are at-risk for presenting later with sepsis. We assessed if <4 hours of maternal IAP for GBS are associated with an increased incidence of clinical neonatal sepsis. MATERIALS AND METHODS: A retrospective cohort study of women-infant dyads undergoing IAP for GBS at ≥37-week gestation who presented in labor from January 1, 2003 through December 31, 2007 was performed. Infants diagnosed with clinical sepsis by the duration of maternal IAP recei…

Long-term trends in the epidemiology of neonatal sepsis and antibiotic susceptibility of causative agents.

BACKGROUND: In an era with increased maternal antibiotic use, patterns in early- and late-onset sepsis and antibiotic susceptibility may have changed. OBJECTIVES: To identify longitudinal trends in causative microorganisms for neonatal sepsis and analyze antibiotic susceptibility of all blood isolates of infants with sepsis. METHODS: Early- and late-onset sepsis cases from 29 years (1978-2006) were studied retrospectively, in five clusters of 5 years (period I-V) and one cluster of 4 years (period VI), including antibiotic susceptibility profiles of blood isolates during the years 1999-2006.…

Removal of percutaneously inserted central venous catheters in neonates is associated with the occurrence of sepsis.

BACKGROUND: Clinical signs of sepsis are frequently observed after removal of a percutaneously inserted central venous catheter (PCVC) in neonates admitted at our Neonatal Intensive Care Unit (NICU). To substantiate this finding and to evaluate the effect of antibiotics administered at the time of removal of a PCVC, we conducted a retrospective study among all infants with a PCVC, admitted at our NICU during 2002 and 2005. METHODS: Clinical data, infectious complications and use of antibiotics were studied retrospectively. RESULTS: A PCVC was inserted in 345 infants. Sepsis occurred in 90/34

Relationship of vaginal microflora to PROM, pPROM and the risk of early-onset neonatal sepsis.

BACKGROUND: Infections are among the most frequent causes of premature delivery and premature discharges of amniotic fluid. The vaginal ecosystem significantly contributes to the development of these conditions. Premature rupture of membranes (PROM) and preterm premature rupture of membranes (pPROM) are associated with an increased risk of intra-amniotic infection. The intra-amniotic infection negatively affects perinatal morbidity and mortality of newborns. OBJECTIVES: Finding of relationship of vaginal microflora to PROM, pPROM and the risk of early-onset neonatal sepsis. METHODS: A prospe…

Failure of the urinary group B streptococcal antigen test as a screen for neonatal sepsis.

The accuracy of the urinary group B streptococcal antigen latex agglutination (LA) test for screening infants at risk of group B streptococcal (GBS) sepsis in the first 24 hours of life was prospectively studied in 236 infants for six months. Infection with GBS was defined by a positive blood culture while colonisation was defined by GBS cultured from any other site. The combination of infection and colonisation was used as the gold standard for the LA test. Although the LA test had a sensitivity of 90%, the specificity was only 70%, the positive predictive value 12% and the false positive rat

Total parenteral nutrition associated cholestasis: a predisposing factor for sepsis in surgical neonates?

Of 496 neonates and infants less than 1 year of age admitted to the paediatric surgical intensive care unit (PSICU) over a 5 year period (1983-1987), 94 required total parenteral nutrition (TPN) for more than 14 consecutive days, generally due to congenital anomalies of the digestive tract. Cholestasis occurred in 15 of them and 12 of these patients developed sepsis. In contrast, of the 79 patients on TPN that remained free from cholestasis, only 23 developed sepsis. The mortality rate for the TPNAC-group was substantially higher than for the group without TPNAC. It is suggested that developme

Buffy coat transfusions in neutropenic neonates with presumed sepsis: a prospective, randomized trial.

Neonatal sepsis, accompanied by neutropenia, is associated with a high mortality. To determine whether granulocyte transfusions improve the survival of critically ill neutropenic infants, we prospectively randomized 25 infants to transfusion and nontransfusion groups, matching for birth weight (less than or equal to 1,500 g or greater than 1,500 g). Infants with necrotizing enterocolitis were randomized separately. Neutropenia was established by two successive absolute neutrophil counts less than or equal to 1,500 cells prior to randomization. The transfusion (n = 12) and nontransfusion (n = 1…

The Influence of the Variation in Sepsis Rate between Neonatal Intensive Care Units on Neonatal Outcomes in Very-Low-Birth-Weight Infants.

Sepsis is commonly known to affect neonatal outcomes. We assessed how much center-to-center variability of the sepsis rate affects the outcomes of very-low-birth-weight infants (VLBWIs). 7,493 VLBWIs registered in the Korean Neonatal Network from 2013 to 2016 were classified into three groups according to the sepsis rate: low sepsis group (LS) < 25th percentile versus intermediate sepsis group (IS) 25th-75th versus high sepsis group (HS) ≥ 75th. The incidence density of sepsis for the LS, IS, and HS groups were 1.17, 3.17, and 8.88 cases/1,000 person-days. After propensity score matching was d

Effect of vitamin D administration on sepsis score and C-reactive protein (CRP) levels in preterm infants with neonatal sepsis: a randomized controlled trial.

BACKGROUND: Neonatal sepsis remains a leading cause of mortality in preterm infants. While Vitamin D possesses immunomodulatory properties, evidence for its adjuvant role in sepsis is limited. This study evaluated the efficacy of two Vitamin D doses (400 IU vs. 800 IU) on clinical sepsis scores and C-reactive protein (CRP) levels. METHODS: An open-label, randomized controlled trial was conducted in Indonesia (August-December 2025) involving preterm infants (28-36 weeks) with sepsis. Patients were randomized into three groups for 7 days: (1) Controls (antibiotics only, nil per os [NPO]), (2) V…

Depletion of Neonatal Neutrophilic Cells Worsens the Outcome of E. coli Sepsis in Newborn Mice.

Neonates are particularly susceptible to infections because of a reduced ability to fight bacterial pathogens while simultaneously exhibitin … in larger litters. NNC-depletion significantly increased mortality and systemic inflammation during neonatal sepsis, whereas bacterial load was only minimally affected. Other immune cell populations remained unchanged. E. coli sepsis induced pronounced neutrophil infiltration into organs of adults, whereas neonates exhibit reduced numbers of NNC in affected organ …
