Thymic tumor with adenoid cystic carcinomalike features: a clinicopathologic study of 4 cases.

Thymic carcinomas are rare malignant neoplasms which comprise several histologic subtypes. Adenoid cystic carcinoma (ACC) is included among these subtypes even if it has never been formally reported. We evaluated the clinical, radiologic, morphologic, immunohistochemical, and genetic features of 4 cases of thymic neoplasms with ACC-like features retrieved from the authors' consult files. Most cases affected adult/elderly males (mean 68.5 y; range: 63 to 77 y; M:F ratio=3:1), and were asymptomatic. The clinical history (no evidence of ACC in other sites), radiologic findings (a mass in the thym

Synchronous Intra-ampullary Papillary-Tubular Neoplasm and Gallbladder Cancer.

Intra-ampullary papillary-tubular neoplasms (IAPNs) are rare precursor lesions of the ampulla of Vater, with a high propensity for progression to invasive carcinoma. Synchronous primary malignancies of the biliary tract are uncommon, and their occurrence in the absence of congenital anomalies such as pancreaticobiliary maljunction (PBM) has not been well documented. We report a case of an IAPN coexisting with a synchronous gallbladder carcinoma in a patient without identifiable predisposing anomalies. A 70-year-old woman with a history of interstitial pneumonia, acute pancreatitis, and mixed c

Investigation of -PRKACA/-PRKACB fusion genes in oncocytic tumors of the pancreatobiliary and other systems.

Intraductal oncocytic papillary neoplasms (IOPNs) of the pancreatobiliary system are tumors comprising oncocytic cells, in which three types of fusion genes involving -PRKACA/-PRKACB were recently identified. IOPNs infrequently combine with other histological subtypes of pancreatic intraductal papillary mucinous neoplasms (IPMNs) and intraductal papillary neoplasms of the bile duct (IPNBs). This study aimed to confirm the sensitivity/specificity of the fusion genes for IOPNs and to examine their significance in other oncocytic lesions. An RT-PCR, followed by DNA sequencing, was undertaken to e

Right video-assisted thoracoscopic enucleation of an upper thoracic esophageal leiomyoma.

Leiomyomas are rare benign tumors of the esophagus that account for about 4% of all esophageal tumors. Recently, the great improvements in minimally invasive techniques have made the resection of mediastinal and pulmonary neoplasms more feasible through video-assisted thoracoscopic or robotic surgery. Our video tutorial describes our approach for a right-sided 3-port thoracoscopic resection of an upper esophageal leiomyoma.

Computed tomography in pediatric diagnosis.

The use of computed tomography has revolutionized the evaluation of the neurologically ill child. This safe and noninvasive procedure may be done on an outpatient basis, eliminating for many the complications that may occur from invasive neuroradiologic procedures. Computed tomography is of particular value in the assessment of intracranial neoplasms, hydrocephalus, and vascular disease. Further advancements are expected in the technology of computed scanning and in its diagnostic use in the rest of the body.

Mucins in neoplasms of pancreas, ampulla of Vater and biliary system.

Tumors of the pancreas, the ampulla of Vater, and the extrahepatic and intrahepatic bile ducts have significant histological similarities due to the common embryonic origin of the pancreatobiliary system. This obviates the need for discovery of biomarkers with diagnostic and prognostic value for these tumors. Mucins, especially MUC-1, -2, -4 and -5AC, are important candidates for developing into such reliable biomarkers. Increased expression of MUC1 occurs in pancreatic ductal adenocarcinomas and is associated with increased degrees of dysplasia in pancreatic intraepithelial neoplasia (PanIN).

Linitis plastica of the colon due to metastases of invasive lobular breast carcinoma.

Colon metastases from breast carcinoma are relatively rare and the invasive lobular subtype tends to metastasize more frequently due to its peritoneal dissemination. A 67-year-old female with a lobular subtype breast neoplasm was treated via a surgical resection ten years previously, with subsequent radiotherapy and adjuvant chemotherapy. She is currently in clinical remission. A colonoscopy was performed due to a recent onset cachexia syndrome, which identified diffuse linitis plastica of the colon. Biopsies confirmed that it was secondary to infiltration by cancer cells of invasive lobular b

Eye-Sparing Treatment for Diffuse Invasive Conjunctival Melanoma.

The management of patients with diffuse invasive conjunctival melanoma focuses on local tumor control and screening for metastasis. Despite the lack of consensus on the benefit of sentinel lymph node biopsy for these neoplasms, the information obtained by histopathology is useful for tumor staging and treatment planning. Due to the lack of evidence of survival improvement, orbital exenteration is being performed with diminishing frequency. We describe a patient with diffuse invasive conjunctival melanoma and lymph node involvement treated by tumor debulking, brachytherapy (custom unshielded ra

Steroid cell tumor of the ovary associated with endometrial adenocarcinoma--a rare case report.

Endometrial carcinoma is the most common invasive neoplasm of the female genital tract and is associated to the elevated levels of unopposed estrogen, especially in postmenopause. Ovarian Steroid cell tumors are rare tumors and they are named according to the origin of cell. The most common cancer of the female genital tract is the endometrial adenocarcinoma and it accounts for 7% of all invasive cancers in women. In the present case report, the authors present a 59-year-old multiparous woman with a postmenopausal bleeding complaint and they discuss the case of ovarian steroid cell tumor assoc

Premalignant and early squamous cell carcinoma.

Actinic keratosis (AK) is a common sun-induced precancerous neoplasm confined to the epidermis. The AK is the initial manifestation of a continuum of clinical and histologic abnormalities that progresses to invasive squamous cell carcinoma (SCC). Bowen's disease, also known as squamous cell carcinoma in situ, represents early SCC confined to the epidermis. More than half of all SCCs contain p53 tumor suppressor gene mutations. Like SCCs, the vast majority of AKs and Bowen's disease lesions are asymptomatic. Each AK and suspicious lesion should be treated before it progresses to invasive SCC. D

Two rare cutaneous neoplasms in horses: apocrine gland adenocarcinoma and carcinosarcoma.

Two rare equine cutaneous neoplasms, an apocrine gland adenocarcinoma and a carcinosarcoma were diagnosed in a 17-year-old pony and a 14-year-old mare, respectively. The apocrine gland adenocarcinoma was present on the prepuce. Histologically, papillary projections of low cuboidal to columnar epithelial cells were generally well differentiated, and surrounded dilated acini. Stromal invasion was present, but vascular and lymphatic invasion was not seen. The carcinosarcoma was present in the right flank of the mare. Two discrete cell populations were characterized histologically. One portion of

Metamorphosis of dermal cylindroma: possible relation to malignant transformation. Case report of cutaneous cylindroma with direct intracranial invasion.

A case of locally invasive cutaneous cylindroma is reported. This neglected scalp tumor, 6.5 cm in greatest dimension, initially invaded the calvaria at the time of the first operation. A recurrence infiltrated the dura and presented as an intracranial space-occupying lesion. No brain invasion was present and distant metastases were not documented. The patient remains well 2 1/2 years after tumor resection. Light and electron microscopic appearance of the recurrent tumor revealed distinct morphologic features, which may be of use in predicting the biologic behavior of cutaneous cylindromas. Th

Spread to the nipple and areola in carcinoma of the breast.

In 40 breasts with primary carcinoma, the nipple and areola were cut horizontally in order to investigate the frequency of intraductal and invasive cancer. In 20 of these breasts the nipple and/or areola were found to be involved at a depth of 1 cm (50%; 95% confidence limits: 33.8-66.2%). Eleven neoplasms were purely intraductal, eight intraductal as well as stromal, and only one purely stromal. By means of clinical or physical findings, it was not possible to select the breasts in which the nipple and/or areola were not involved. It is concluded that the general use of surgical methods prese

Solid papillary carcinoma of the breast with high-grade cytological features: challenging the concept of indolence.

Solid papillary carcinoma (SPC) of the breast is a rare neoplasm typically regarded as a low-grade tumor with indolent behavior. We report two cases of SPC in elderly women presenting with palpable breast masses. Preoperative imaging demonstrated BI-RADS 4 suspicious lesions, and ultrasound-guided core needle biopsy suggested a papillary neoplasm consistent with SPC. Definitive histopathological examination revealed SPC with unusual high-grade cytological features, including marked atypia, increased mitotic activity, and an elevated Ki-67 proliferation index, despite the absence of conventiona

Mucinous Cystic Neoplasms in Male Patients: Histopathological and Molecular Diagnoses.

Cystic mucinous neoplasms (MCNs) of the pancreas are rare cystic tumors, accounting for approximately 2-5% of all pancreatic neoplasms. They predominantly occur in premenopausal women and are typically located in the body or tail of the pancreas. Due to their potential for malignant transformation, especially in cases associated with invasive carcinoma such as pancreatic ductal adenocarcinoma, early detection, complete surgical resection, and rigorous postoperative surveillance are essential. The occurrence of MCNs in male patients is exceedingly rare, comprising only about 2% of reported case

Myopericytoma of the thoracic spine in a pregnant woman: a case report on the management of a rare neoplasm.

Myopericytomas are perivascular myoid neoplasms that rarely exhibit malignant characteristics. They usually arise in the dermis or subcutaneous tissue and exceptionally involve deep soft tissues, with spinal localization being rare. We report the case of a previously healthy 32-year-old pregnant woman who presented with pain in the interscapular region and progressive loss of lower limb strength. Magnetic resonance imaging revealed a solid neoplastic mass at the T3 level, with spinal canal invasion and spinal cord signal abnormalities. The pregnancy was terminated, followed by lesion embolizat

Inflammatory myofibroblastic tumours of the duodenopancreaticobiliary tract.

Inflammatory myofibroblastic tumours (IMTs) are rare benign neoplasms of intermediate malignant potential that are found in the lungs and rarely at extrapulmonary sites common in children and young adults. IMTs tend to be locally invasive and have some amount of metastatic potential as well. We present two cases of IMTs involving the duodenum, pancreas and distal bile duct. The first case presented with extensive involvement of the first three parts of the duodenum and head of the pancreas, while the second presented with a pancreatic and biliary tree involvement. Sinonasal (Schneiderian) papilloma of the lacrimal sac.

Sinonasal papillomas, also known as Schneiderian papillomas, are benign but infiltrative epithelial neoplasms that arise from ectodermally derived pseudostratified ciliated (or Schneiderian) epithelium that lines the sinonasal tract and nasal cavity. We describe a rare case of a sinonasal papilloma confined to the lacrimal duct and sac in a 45-year-old man who presented with 10 months of right-sided epiphora and swelling of his medial right lower eyelid. He was found to have a 10 × 10 mm area of distension near the right lacrimal sac and underwent an external dacryocystorhinostomy. A computed

Endolymphatic Sac Tumor in Two Dogs.

Endolymphatic sac tumors (ELSTs) are rare neoplasms of the inner and middle ear described in humans. Diagnosis of such neoplasms is difficult and largely dependent on a combination of histologic, immunohistochemical, and clinical findings. Although the neoplastic cells lack cellular features of malignancy, these are clinically aggressive tumors that often invade the surrounding temporal bone. Here, we describe 2 dogs with middle ear masses that share morphologic, immunohistochemical, and clinical similarities with human ELSTs. Advanced imaging of the masses revealed evidence of aggressive beha
