The facilitatory effect of Casearia sylvestris Sw. (guaçatonga) fractions on the contractile activity of mammalian and avian neuromuscular apparatus.

Many natural products influence neurotransmission and are used clinically. In particular, facilitatory agents can enhance neurotransmission … ations, the methanolic fraction either restored neuromuscular transmission (mimicking the effect of neostigmine) or failed to cause any recovery of neurotransmission. In the presence of 3,4-diaminopyridine (3,4-DAP), the methanolic fraction decreased twitch amplitude, whereas at a high frequency of stimulation (40 Hz) there was an increase in tetanic tension. …

Characteristics and mechanism of neuromuscular block in myasthenia gravis.

The neuromuscular block of myasthenia gravis has the characteristics of an antidepolarizing (competitive block), similar to that produced by … ) type of block, including inhibition of the depolarizing action of ACh and reversibility by ACh or neostigmine, while in normal subjects the characteristics were those of a depolarizing (noncompetitive) type of block, including little or no inhibition of the depolarizing action of ACh and lack of reversal by ACh or neostigmine. The differences between the la …

Deadly Nightshade (Atropa belladonna) and Other Weed Poisonings in the Pediatric Intensive Care Unit: Analysis of 54 Children.

OBJECTIVE: Atropa belladonna, known as the deadly nightshade, is one of the most poisonous plants in the world. Deadly nightshade poisoning … contributed to extended ICU stays and elevated respiratory support requirements (P < .05). Notably, neostigmine was administered in all cases of deadly nightshade poisoning due to the unavailability of physostigmine. Our findings showed that the first signs and symptoms of acute deadly nightshade and other weed poisoning may be severe in some children, but no …

Synthesis, characterization, and biological evaluation of some novel Schiff bases as potential metabolic enzyme inhibitors.

We determined that the IC50 values of the tested molecules ranged between 16.86 and 57.96 nM for hCA I and 15.24-46.21 nM for hCA II. As a consequence, we may say that some of the Schiff base derivatives may be used as potential drug candidates in …

Retroperitoneal teratoma causing ptosis: A case report.

The present study reports the case of a patient who had undergone unsuccessful hormone therapy for ocular myasthenia gravis 14-years prior to the current presentation. The diagnosis of ocular myasthenia gravis was once again confirmed by a neostigmine test and repeat nerve stimulation study. Computed tomography scans in an external institution revealed a retroperitoneal cystic tumor with calcification above the left adrenal gland. The tumor was removed via a transperitoneal laparoscopic resection and was diagnosed as a mature cystic teratoma upon pathological examination. A teratoma is a commo…

Neuromuscular disease and anesthesia.

Patients with neuromuscular disease pose many anesthetic challenges and are at greater risk for perioperative complications, including respiratory or cardiovascular dysfunction and pulmonary aspiration. Therefore, these patients require special precautions, including interdisciplinary communication between primary care physicians, neurologists, physiatrists, surgeons, and anesthesiologists. Preoperative evaluation and optimization of comorbid conditions is critical. These patients may have adverse response to neuromuscular blocking drugs and the reversal drugs (e.g., neostigmine). They should …

Marked hypotonia in an infant of a mother with Devic disease.

A full-term female neonate was born with severe hypotonia and weakness. Her mother had been treated for neuromyelitis optica (Devic disease) for 6 years. Her previous son, born 10 years earlier and before she developed the disease, also had marked hypotonia that gradually improved over several weeks. A suspicion of neonatal myasthenia gravis arose, as a search of the literature revealed the occasional detection of anti-acetylcholine receptor antibodies in patients with Devic disease. A neostigmine test was mildly positive in the baby, but anti-acetylcholine receptor antibodies were elevated. A…

[The clinical features of myasthenia gravis affecting nonskeleton muscles].

OBJECTIVE: To propose a method for diagnosis and differential diagnosis of myasthenia gravis (MG) with heart and liver injury, anisocoria, dysacusis, impatience etc. METHODS: (1) Before and after administering cholinesterase inhibitor and immune therapy, 55 patients with MG were re-examined for heart and liver and followed up. (2) 1 mg neostigmine was injected to the MG patients with anisocoria and dysacusis, the symptoms were observed and brainstem auditory evoked potential was performed before and after 30 minutes and 2 hours. RESULTS: With the relaxation of muscular weakness and fatigabil…

Use of microdialysis for monitoring sympathetic and parasympathetic innervation of heart in conscious rats.

A microdialysis method was developed to sample norepinephrine and acetylcholine from the heart of freely moving rats. A flexible dialysis fi … sterase inhibitor was infused. Corrected basal output levels 24 h after surgery were 223 pg/ml when neostigmine was coinfused in a concentration of 100 mumol/l. In addition, the presence of local muscarinic autoreceptors on cholinergic neurons in the heart was shown. It is concluded that microdialysis is a reliable method that can be used to study the innerva …

Modifications in gastroduodenal motility induced by the extramucosal section of circular duodenal musculature in dogs.

The effect induced on the gastroduodenal motility by the extramucosal section of the circular layer of the first two parts of the duodenum, excluding the pylorus, was studied in a series of 5 dogs. The myoelectric and manometric activities of the antrum, pylorus, and duodenum were recorded by using special devices surgically implanted in the gut wall. The recordings were made 3 mo after operation, both during basal conditions and after stimulation with neostigmine. The results were statistically compared with those obtained in a control group of 4 dogs in which the electromanometric devices ha…

CONCLUSIONS: VOG is a reliable diagnostic tool for MG, particularly useful for seronegative patients, allowing for earlier and more accurate diagnosis than conventional methods.

Sugammadex for Neuromuscular Blockade Reversal: A Narrative Review.

Sugammadex represents a significant advancement in neuromuscular blockade management, enabling rapid, predictable, and highly effective reversal of steroidal neuromuscular blockers such as rocuronium and vecuronium. This review critically examines recent advances in sugammadex research, particularly over the last decade, detailing its pharmacological profile, clinical efficacy, and safety compared to traditional reversal agents, like neostigmine. Its expanding clinical applications across operating rooms, critical care units, and emergency medicine are discussed, emphasizing dosing recommendat…

History of the development of antagonists for neuromuscular blocking agents.

Muscle relaxation induced by neuromuscular blocking agents (NMBAs) is necessary for tracheal intubation and immobilization during surgery. Although acetylcholinesterase inhibitors have been successfully used as antagonists for NMBAs, they have their limitations; their effects are transient and ineffective against profound neuromuscular blockade. In the past, alternative antagonists were developed, such as germine and 4-aminopyridine, which are effective for the treatment of diseases causing muscle weakness and could potentially be used as antagonists for NMBAs. Unfortunately, these drugs did n

Origin of high slow-wave frequency in the dog colon.

Using intraparietal and intraluminal electrodes, duodenal and colonic electrical activities were simultaneously recorded in dogs under pentobarbital anaesthesia to study the temporal relationship between the slow-wave frequency of the colon and that of the proximal duodenum. The colonic electromyogram showed two distinct slow-wave frequencies, one in the range of 7-9 cycles/min, and the other in the range of 18-19 cycles/min. The latter was synchronized to that observed on the duodenum, and disappeared after removal of the entire duodenum and the proximal jejunum over 90 cm, from 8 to 98 cm fr
