STUDY DESIGN: Systematic review and meta-analysis.
OBJECTIVES: Cervical spinal cord injury (SCI) and brachial plexus injury (BPI) can result in hand paralysis. Reconstruction of hand motion is associated with improved functionality and quality of life. We synthesized the outcomes of finger and thumb flexion after various nerve transfers to the anterior interosseous nerve (AIN).
METHODS: A systematic literature review and meta-analysis was performed according to the PRISMA guidelines. Successful finger and thumb flexion recovery was defined as Medical research council (MRC) grade ≥3.

Recovery of hand and arm function is critical for improving quality of life in individuals with tetraplegia due to spinal cord injury (SCI). Nerve transfer procedures can restore meaningful hand and arm function in chronic SCI, yet postoperative outcomes vary widely. We conducted a prospective, single-arm, open-label trial to assess the impact of intensive, robot-assisted rehabilitation training on functional recovery and cortical reorganization following nerve transfer. The primary endpoint was assessment of hand and arm function measured by the Box and Blocks Test. We report the results from…

A systematic review following PRISMA guidelines was conducted to address the PICO question: Does early (< 6 months) nerve transfer improve muscle strength outcomes when compared to late (≥ 6 months) nerve transfer for upper trunk palsy, as measured by the Medical Research Council (MRC) scale? The association between age and successful muscle function recovery was assessed as a secondary outcome. The quality of included studies was evaluated using the Methodological Index for Non-Randomized Studies (MINORS) tool. A total of 349 studies were screened, with 27 studies meeting criteria for systema…

BACKGROUND: Nerve transfers have been proposed solutions for urinary, bowel, and genital dysfunction after spinal cord injury (SCI). However, there remains a need to characterize nerve candidates and other treatment variables for SCI. The objective of this systematic review was to characterize nerve transfer strategies and outcomes for the restoration of bladder, bowel, genital, and lower extremity function after SCI.
METHODS: PubMed, Cochrane, MEDLINE, and Embase libraries were queried according to the preferred reporting items for systematic reviews and meta-analyses guidelines for articles…

PURPOSE: Chronic paralysis of the flexor pollicis longus (FPL) and the index flexor digitorum profundus (FDP) is commonly treated with brachioradialis transfer to the FPL and side-to-side suturing of the index FDP to the FDPs of the ulnar digits. We report the clinical outcomes of nine patients in whom the extensor carpi radialis brevis (ECRB) or longus (ECRL) was transferred to both the FPL and index FDP.
METHODS: We reviewed nine patients who underwent tendon transfer of the ECRB/L to both the FPL and index FDP. Postoperative assessments included strength grading of the FPL and index FDP us

CASE: A 49-year-old woman with West Nile encephalitis presented with right arm paralysis and left arm weakness. After limited success with nonsurgical treatments, she underwent surgery involving 3 nerve transfers based on brachial plexus reconstruction techniques. This led to gradual improvement in arm movement and restoration of ADL's.
CONCLUSION: Since treatment of the underlying disease may not be enough to restore nerve function, an algorithm incorporating brachial plexus principles may enhance treatment outcomes. Though this case study demonstrates success, we believe this technique dese…

INTRODUCTION: Medially routed biceps-to-triceps tendon transfer for elbow extension reconstruction in spinal cord injury (SCI) has proven to be a reliable procedure. This technique classically places the tendon transfer superficial to a paralyzed ulnar nerve, with a theoretical risk of compression neuropathy.
CASE PRESENTATION: A 21-year-old male with a C5 American Spinal Injury Association Impairment Scale (AIS) grade B SCI who underwent bilateral biceps-to-triceps tendon transfers presented with new-onset paresthesias in the ring and small fingers 10.5 years following initial reconstructive

Phase locking of auditory-nerve-fiber (ANF) responses to the fine structure of acoustic stimuli is a hallmark of the auditory system's temporal precision and is important for many aspects of hearing. Period histograms from phase-locked ANF responses to low-frequency tones exhibit spike-rate and temporal asymmetries, but otherwise retain an approximately sinusoidal shape as stimulus level increases, even beyond the level at which the mean spike rate saturates. This is intriguing because apical cochlear mechanical vibrations show little compression, and mechanoelectrical transduction in the rece

Cervical spinal cord injury (SCI) can cause tetraplegia. Nerve transfer has been routinely utilized for reconstruction of hand in brachial plexus injuries. Here, we report reconstruction of finger flexion (hand grasp) and extension (hand release) in a victim of cervical spinal cord injury with tetraplegia. We also focus on importance of extension phase in restoration of hand function in the tetraplegic case, in addition to provision of a detailed description of both operations including text, photographs, and a video. We used double nerve transfer, namely brachialis branches of musculocutaneou…

UNLABELLED: We compared outcomes of primary vascularized ulnar nerve grafts from the C5 root neurotizing biceps and brachialis muscles, and gracilis functioning free muscle transfer neurotized by the distal spinal accessory nerve, as a primary or salvage procedure after complete brachial plexus injury. At 45 months, three of eight primary vascularized ulnar nerve graft patients regained grade 4 elbow flexion, while one regained grade 3. All 13 primary gracilis transfer patients regained grade 4 elbow flexion. Four patients with vascularized ulnar nerve grafts failed and subsequently had salvag

BACKGROUND: Injury to long thoracic and the spinal accessory nerves can cause winging scapula as a result of weakness and paralysis of the trapezius and serratus anterior muscles. Although these nerve and muscle operations have been reported to correct winging scapula due to various causes, there is no report on comparing the outcomes of these procedures in peer-reviewed Pubmed-indexed literature. In this article, we compared the improvements in the restoration of shoulder functions in winging scapula patients after long thoracic nerve decompression (LTND) in our present study with outcomes of

BACKGROUND: Radical tumor ablation in the periauricular area often results in extensive soft tissue defects, including facial nerve sacrifice, bone and/or dura defects. Reconstruction of these defects should aim at restoring facial reanimation, wound closure, and facial and neck contours. We present our experience using free anterolateral thigh flap (ALT) in combination with masseter nerve to facial nerve transfer in managing complex defects in the periauricular area.
METHODS: Between 2011 and 2015 six patients underwent a combined procedure of ALT flap reconstruction and masseter nerve trans…

OBJECTIVE: To investigate the feasibility of contralateral C7 nerve transfer via posterior spinal route for treatment of brachial plexus root avulsion injury by anatomical study.
METHODS: Ten cadaveric specimens of 7 men and 3 women were selected, who had no obvious deformity and no tissue defect in neck neutral position. By simulating surgical exploration of brachial plexus injury, the length of contralateral C7 nerve root was elongated by dissecting its anterior and posterior divisions to the distal end, while the length of C7 nerve from the intervertebral foramen to the branching point and…

A challenging problem is the patient with a total brachial plexus injury with nerve root avulsions. In these patients nerve repair is not possible and no local functioning muscles are available for transfer. Current techniques involve either nerve repair using donor nerves from the contralateral limb or free muscle transfer neurotized by intercostal nerves. The problem with both these techniques is that they are dependent on neural regeneration, which is imperfect. To overcome the problem we propose a technique of transferring a distant muscle whilst retaining its neural supply. Gastrocnemius

Neuropathic pain in root avulsion brachial plexus injury (BPI) is severe and unrelenting. Routine analgesics and other described methods often provide inadequate pain relief. We report a patient with neuropathic pain following C8T1 root avulsion injury, which was successfully managed with end-to-side ulnar sensory branch transfer to the median nerve. Postoperatively, there was a marked reduction in pain score as determined by visual analogue score (VAS) and a marked improvement in the daily sleep interference scale (DSIS). This option may be considered in patients with severe neuropathic pain

BACKGROUND: Central nervous system (CNS) disorders, such as stroke, often lead to spasticity, which result in limb deformities and significant reduction in quality of life. Spasticity arises from disruptions in the normal functioning of cortical and descending inhibitory pathways in the brainstem, leading to abnormal muscle contractions. Contralateral seventh cervical nerve cross transfer (CC7) surgery has been proven to effectively reduce spasticity, but the specific mechanism for its effectiveness is unclear.
METHODS: This study aimed to investigate the changes in the dorsal root ganglia (D
