Deterioration of multifocal motor neuropathy after plasma exchange.

We report a patient with motor neuropathy in whom plasma exchange (PE) was followed by a pronounced clinical worsening with the appearance of conduction blocks in previously clinically unaffected motor nerves, leading to the diagnosis of multifocal motor neuropathy (MMN). This report highlights the different response to therapy of MMN and chronic inflammatory demyelinating polyneuropathy (CIDP) because not only steroids but also PE, which is often effective in CIDP, do not improve and at times may even worsen MMN.

Ulnar nerve injury associated with removal of Norplant implants.

Improperly placed Norplant implants pose a risk to neurovascular structures at the time of removal. Appropriate attention to insertion and removal should minimize the chance of injury. However, when injuries do occur, a logical and systematic approach to evaluation of the injury can help predict the outcome and establish a plan of care for the patient.

[Post-infectious demyelinating diseases].

Post-infectious demyelinating disorders are uncommon. In the Hospital Pediatrico de la Misericordia de Santafé de Bogotá there were 14 cases in three years. The commonest age group was the new born, as is found in the literature. The infections involved in these neurological changes are probably viral, since at the time the patient is seen most symptoms have cleared up spontaneously. According to several authors, the viruses are most often found to be those of measles or mumps. Between the infection and the development of neurological symptoms there is an interval of approximately two weeks. T

Restless legs syndrome: clinicoetiologic correlates.

Despite recent attempts to better characterize restless legs syndrome (RLS), this neurologic disorder remains poorly understood. The relationship between idiopathic and secondary and between familial and sporadic (nonfamilial) cases have not been previously defined. We studied 54 patients (29 women and 25 men) who satisfied the diagnostic criteria for RLS. The mean age of the patients was 62.69 +/- 13.82 years, and the mean age at onset was 34.13 +/- 20.30 years. We found that 92% of patients with idiopathic RLS (without neuropathy) had a family history of RLS, whereas only 13% of those with n

The tendon reflexes in the electrodiagnosis of sciatica.

In order to determine the diagnostic value of the electromyographically tested tendon reflexes (ETR) in lumbosacral radiculopathies, we compared the latency and amplitude of the knee (EKTR) and Achilles tendon reflex (EATR) to their clinical testing and to the EMG and H reflex values in 200 non selected patients (126 men, 74 women) aged 24-68 years (48.6 +/- 9.4). The control group was composed of 50 healthy volunteers (28 men, 22 women) aged 25-60 yrs (44 +/- 8.5). The combined electrophysiological reflex examination was abnormal in 45% of the affected limbs, being the most frequent pathologi

Provocative exercise maneuver: its effect on nerve conduction studies in patients with carpal tunnel syndrome.

This study examined the influence of a provocative dynamic exercise maneuver on nerve conduction studies (NCS) in three study groups: Group A, controls (n = 10); Group B, patients with clinical carpal tunnel syndrome (CTS), but normal NCS (n = 10); and Group C, patients with clinical CTS and abnormal NCS (n = 10). A wrist flexion-extension exercise was designed to increase pressure on the median nerve within the carpal tunnel to aid diagnosis by revealing latent nerve conduction slowing in patients presenting with clinical CTS but normal NCS. The exercise protocol was sufficient to evoke sympt

Postsurgical idiopathic brachial neuritis.

Idiopathic brachial neuritis (IBN) is a well-recognized clinical syndrome characterized by brachial pain followed by a patchy amyotrophy of muscles in the shoulder girdle and arm innervated by individual branches of the brachial plexus. Postsurgical IBN has not been widely recognized since Parsonage and Turner's original description in which 10% of patients had antecedent surgery. We present 6 patients who 1-13 days postoperatively developed signs and symptoms which met the clinical and electrophysiologic criteria for IBN. Postsurgical neuralgic amyotrophy is an under-recognized clinical entit

Compressive radial nerve palsy induced by military shooting training: clinical and electrophysiological study.

Ten recruited soldiers developed acute left wrist drop and numbness on the back of the thumb after a three hour military shooting training. Neurological examination disclosed decreased muscle power (0-2/5) of left wrist dorsiflexion, hypalgesia and hypaesthesia on the radial side of the left hand, and diminished brachioradialis reflex. Electrophysiological studies showed prolonged distal latency, reduced amplitude and slowness of left radial nerve motor conduction velocity between the axilla and elbow. Electromyography (EMG) revealed fibrillation potentials at rest, polyphasic motor unit and a

Texture discrimination in carpal tunnel syndrome.

The ability to discriminate textured surfaces was measured in patients with carpal tunnel syndrome (CTS). Patients were initially diagnosed using clinical and electrophysiological criteria. The textured surfaces were gratings of alternating ridges and grooves. The gratings differed in their spatial period only, with the ratio of the ridge width to the groove width remaining constant (1:5). A two-alternative forced-choice paradigm was employed in which subjects, both patients and age-matched controls, rubbed their index finger (D2) or little finger (D5) back and forth across the surfaces. The g

Recurrent common peroneal palsy in association with the Ehlers-Danlos syndrome. A case report.

We report a patient with type III Ehlers-Danlos syndrome, presenting with recurrent footdrop, who had electrophysiologic evidence of a conduction block of the common peroneal nerve followed by complete resolution. We feel that this case supports the possible link between Ehlers-Danlos syndrome and multiple pressure sensitive neuropathy (tomaculous neuropathy), alluded to by Schady and Ochoa (1984).

AAEM case report #22: polymyositis.

Polymyositis usually presents with progressive proximal muscle weakness, increased serum levels of muscle enzymes, inflammatory changes on muscle biopsy, and characteristic electromyographic (EMG) abnormalities. Motor unit action potential (MUAP) changes of configuration, duration, and amplitude are the most frequently observed EMG abnormality. Fibrillation potentials are commonly seen and tend to reflect active disease, diminishing after successful medical management or disease regression. Other muscle diseases can present with similar electromyographic abnormalities, thereby necessitating mu

Forearm median nerve conduction velocity in carpal tunnel syndrome.

Reduced velocity of median nerve conduction in the forearm of patients with carpal tunnel syndrome (CTS) has been attributed to an artifact of the electrodiagnostic method rather than pathophysiologic changes in the forearm segment. Standard nerve conduction velocity (NCV) tests measure the forearm segment in combination with the distal latency and this may not represent an accurate assessment of conduction in the proximal portion of the nerve. A new technique of direct evaluation of the forearm median nerve was developed by the stimulation and recording of the forearm nerve action potential (

Rapid mechanical and thermal changes in the garfish olfactory nerve associated with a propagated impulse.

Mechanical and thermal changes associated with a propagated nerve impulse were determined using the garfish olfactory nerve. Production of an action potential was found to be accompanied by swelling of the nerve fibers. The swelling starts nearly at the onset of the action potential and reaches its peak at the peak of the action potential. There is a decrease in the length of the fibers while an impulse travels along the fibers. The time-course of the initial heat was determined at room temperature using heat-sensors with a response-time of 2-3 ms. Positive heat production was found to start a

Radio-frequency catheter ablation of cardiac arrhythmias: appraisal of an evolving therapeutic modality.

Since its introduction in 1985, there have been numerous reports of the use of radio-frequency (RF) current as an alternative energy source for catheter ablation. RF current (frequency range 150 kHz to 1 MHz), when delivered in bipolar mode, causes desiccation of tissue by creating a localized area of heat that results in discrete coagulation necrosis. An equivalent energy source such as direct current (DC) shock can be delivered at lower power (5 to 30 W) and lower voltage (30 to 80 V) for a longer pulse duration (10 to 60 seconds), thus eliminating barotraumatic effects. Because of its high

Electrodiagnosis of human colchicine myoneuropathy.

Colchicine may produce a neuromuscular disorder even when given in customary doses. We report the electrodiagnostic features in eight pathologically proven cases of colchicine-induced myoneuropathy. Myopathic motor unit potentials and early recruitment were found in proximal limb and truncal muscles, frequently with fibrillations, positive sharp waves, or complex repetitive discharges. These electromyographic findings correlated with the course of the weakness, which rapidly resolved within weeks of drug discontinuation, indicating that the major functional disturbance in the patients was myop

Carpal tunnel syndrome in the nonparetic hands of hemiplegics. Stress-induced by ambulatory assistive devices.

Three patients who had earlier sustained an acute stroke were admitted to a rehabilitation bed service. Clinically, they developed symptoms suggestive of carpal tunnel syndrome (CTS) in their nonparetic hands during a progressive ambulation training program which included ambulatory assistive devices. In each patient, the diagnosis was confirmed by electrodiagnostic evaluation. Two of the three patients were subsequently provided with forearm platform attachments for their assistive devices, and cock-up wrist splints, in an attempt to decrease compression of the median nerve. Despite these mea
