Effects of Lipid-Induced Magnetic Microstructure on Fat Fraction Quantification in Muscular Dystrophies.

PURPOSE: To study the impact of mesoscopic magnetic susceptibility heterogeneity on chemical shift-encoded (CSE) proton density fat-fraction (PDFF) quantification in muscular dystrophies, a subgroup of neuromuscular disorders. THEORY AND METHODS: In MRI, extramyocellular lipid deposits induce orientation-dependent Larmor frequency variations due to microstructural anisotropy, resulting in spatially varying frequency shifts between fat and water and increased transverse relaxation rates. A newly developed PDFF quantification method accounting for resonance shifts and dual R2* rates was applied

Development and Feasibility Assessment of a Multimodal Digital Health Technology for Remote Monitoring of Symptoms in Myasthenia Gravis.

INTRODUCTION: Myasthenia gravis (MG) is a chronic autoimmune neuromuscular disease. Patients with MG are typically evaluated by neuromuscular experts through in-person neurologic examinations. These assessments are time-consuming, require significant disease expertise, and capture only a snapshot of disease. METHODS: Given this need, we developed a multimodal digital health technology (DHT) called BioDigit MG, for monitoring MG symptoms and objectively measuring disease severity. BioDigit MG includes tablet-guided speech and video-based assessments, electronic patient-reported outcomes releva

Dynamic MR of Muscle Contraction During Electrical Muscle Stimulation: Potential Application to the Evaluation of Neuromuscular Diseases.

Thanks to the rapid evolution of therapeutic strategies for muscular and neuromuscular diseases, the identification of quantitative biomarkers for disease identification and monitoring has become crucial. Magnetic resonance imaging (MRI) has been playing an important role by noninvasively assessing structural and functional muscular changes. This exploratory study investigated the potential of dynamic MRI during neuromuscular electrical stimulation (NMES) to detect differences between healthy controls (HCs) and patients with metabolic and myotonic myopathies. The study included 14 HCs and 10 p

Ultrasound Detection of Vagus Nerve Atrophy in Bulbar Amyotrophic Lateral Sclerosis.

BACKGROUND AND PURPOSE: Neuromuscular ultrasound in amyotrophic lateral sclerosis (ALS) is of increasing interest. As bulbar symptoms are commonly developed by most ALS patients during disease, the aim of our study was to find possible sonographic changes of vagus nerve size in bulbar affected ALS patients. METHODS: We investigated 24 ALS patients and 19 controls without neuromuscular disorders. In ALS patients, bulbar affection was documented clinically (eg, dysarthria, dysphagia, and fasciculations) or subclinically using ultrasound and electromyography of bulbar muscles. Vagus nerve ultras

Voluntary activation deficits of the infraspinatus present as a consequence of pitching-induced fatigue.

HYPOTHESIS: Neuromuscular inhibition of the infraspinatus would be greater and external rotation muscle force would be lower after a simulated game compared with pregame values. MATERIALS AND METHODS: The sample included 21 uninjured, asymptomatic high school-aged baseball pitchers. Maximum volitional shoulder external rotation strength was assessed before and after a simulated game with a clinical dynamometer. Voluntary activation of the infraspinatus was assessed during strength testing by a modified burst superimposition technique. Performance-related fatigue was assessed by monitoring pit

[RSI in pediatric anesthesia: an online survey of actual clinical practice].

BACKGROUND: For many years, controlled rapid sequence induction (cRSI) has been recommended in children at risk of aspiration to reduce the … infants and the inconsistent use of video laryngoscopy, relaxometry, neuromuscular blocking agents, neuromuscular monitoring and mechanical pressure-controlled intermediate ventilation. The transition to classical RSI is based on individual patient circumstances regarding the risk of hypoxia. No fixed age limit could be identified.

Practical Management for Use of Eculizumab in the Treatment of Severe, Refractory, Non-Thymomatous, AChR + Generalized Myasthenia Gravis: A Systematic Review.

Myasthenia gravis (MG) is a rare autoimmune disorder caused by specific autoantibodies at the neuromuscular junction. MG is classified by the antigen specificity of these antibodies. Acetylcholine receptor (AChR) antibodies are the most common type (74-88%), followed by anti-muscle specific kinase (MuSK) and other antibodies. While all these antibodies lead to neuromuscular transmission failure, the immuno-pathogenic mechanisms are distinct. Complement activation is a primary driver of AChR antibody-positive MG (AChR+ MG) pathogenesis. This leads to the formation of the membrane attack complex

Psychopharmacological Treatments for Mental Disorders in Patients with Neuromuscular Diseases: A Scoping Review.

Mental disorders are observed in neuromuscular diseases, especially now that patients are living longer. Psychiatric symptoms may be severe and psychopharmacological treatments may be required. However, very little is known about pharmacotherapy in these conditions. We aimed to summarize the current knowledge on the use of psychopharmacological treatments for mental disorders in patients living with a neuromuscular disease. A scoping review was performed using the methodology of the Joanna Briggs Institute. Four databases were searched from January 2000 to July 2021. Articles were screened bas

Neo-epitope Peptides as Biomarkers of Disease Progression for Muscular Dystrophies and Other Myopathies.

For several decades, serological biomarkers of neuromuscular diseases as dystrophies, myopathies and myositis have been limited to routine clinical biochemistry panels. Gauging the pathological progression is a prerequisite for proper treatment and therefore identifying accessible, easy to monitor biomarkers that can predict the disease progression would be an important advancement. Most muscle diseases involve accelerated muscle fiber degradation, inflammation, fatty tissue substitution and/or fibrosis. All these pathological traits have been shown to give rise to serological peptide biomarke

Neuromuscular pharmacology in rat neonates: development of responsiveness to prototypic blocking and reversal drugs.

The neonatal pharmacology of neuromuscular drugs was studied in vivo in newborn rats and in vitro in neonatal phrenic nerve-hemidiaphragm preparations. Drugs used to probe neuromuscular development in rat neonates were physostigmine, edrophonium, neostigmine, 4-aminopyridine, d-tubocurarine (dTc), and succinylcholine. The prejunctional actions of these drugs were monitored in relation to neonatal age by the appearance of stimulus-evoked repetitive discharge initiated by motor nerve endings and the occurrence and magnitude of the resulting enhancement of twitch tension. The occurrence and incid

Operation Everest II: neuromuscular performance under conditions of extreme simulated altitude.

The force output of the ankle dorsiflexors was studied during a 40-day simulated ascent of Mt. Everest in a hypobaric chamber; both electrically activated and maximal voluntary contractions (MVCs) were employed. The purpose of this study was to establish whether, under conditions of progressive chronic hypoxia, there was a decrease in muscle force output and/or increased fatigability. We also attempted to identify the main site of any failure, i.e., central nervous system, neuromuscular junction, or muscle fiber. Muscle twitch torque (Pt), tetanic torque (Po), MVC torque, and evoked muscle com

Current issues in the respiratory care of patients with amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis is a progressive neuromuscular disease, resulting in respiratory muscle weakness, reduced pulmonary volumes, ineffective cough, secretion retention, and respiratory failure. Measures as vital capacity, maximal inspiratory and expiratory pressures, sniff nasal inspiratory pressure, cough peak flow and pulse oximetry are recommended to monitor the respiratory function. The patients should be followed up by a multidisciplinary team, focused in improving the quality of life and deal with the respiratory symptoms. The respiratory care approach includes airway clearance

Unplanned postoperative reintubation: a narrative review of risk factors, predictive tools, and preventive strategies.

Unplanned postoperative reintubation is associated with increased morbidity, mortality, and healthcare use. Unplanned postoperative reintubation represents a heterogeneous clinical endpoint arising from distinct and often overlapping etiopathogenetic mechanisms, including airway compromise, respiratory insufficiency, cardiac dysfunction, neurological impairment and acute organ failure. Early reintubation need is more commonly associated with residual anaesthetic/neuromuscular blocker effects and airway-related factors, whereas intermediate and late restoration of invasive ventilation frequentl

Pediatric Myasthenia Gravis.

Myasthenia gravis is a disorder of neuromuscular transmission that leads to fatigue of skeletal muscles and fluctuating weakness. Myasthenia that affects children can be classified into the following 3 forms: transient neonatal myasthenia, congenital myasthenic syndromes, and juvenile myasthenia gravis (JMG). JMG is an autoimmune disorder that has a tendency to affect the extraocular muscles, but can also affect all skeletal muscles leading to generalized weakness and fatigability. Respiratory muscles may be involved leading to respiratory failure requiring ventilator support. Diagnosis should

Anesthetic management of robot-assisted thoracoscopic thymectomy.

Myasthenia gravis (MG) is a rare disorder involving neuromuscular junction. In conjunction with medical therapy, thymectomy is a known modality of treatment of MG and has shown to increase the probability of remission and overall symptomatic improvement. For minimally invasive thymectomy, video-.assisted thoracoscopic surgery has been the preferred surgical approach till recently. The robotic surgical procedure must necessarily bring new challenges to the anesthesiologists to effectively meet the specific requirements of the technique. At present, there is a paucity of literature regarding the
