Discharge of older patients with chronic diseases: What nurses do and what they record. An observational study.

AIMS AND OBJECTIVES: To describe which nursing activities are observed during the discharge of older patients with chronic diseases and to investigate the consistency between the nursing activities actually observed and those documented. BACKGROUND: The discharge from hospital of older patients with chronic diseases is a critical transition that can lead to dissatisfaction, delays in discharge, re-admission, adverse events and increased mortality. Although nurses' interventions during discharge are important for patient outcomes, little is known about the nursing activities actually performed

Transient Neonatal Diabetes Mellitus: A Challenge and Opportunity for Specialized Nursing Care.

Transient neonatal diabetes mellitus (TNDM) is a rare disorder, with a reported incidence of approximately 1 in 450,000 live births. It is characterized by insulin-requiring hyperglycemia in the neonatal period. The disease improves by early childhood, but the patient may relapse in later life. Diagnosis is made after genetic testing following presentation with hyperglycemia not conforming to Type 1 or Type 2 diabetes. Management is based on insulin and possible sulfonylurea administration. Three genetically distinct subtypes of TNDM are recognized. Type 1 TNDM is due to overexpression of gene

Are Pediatric Pain Self-Report Scores Accurate?

Review of recent evidence with translation to practice for the advanced practice nurse (APN) role is presented using a case study module for "Clinically Significant Differences in Acute Pain Measured on Self-Report Pain Scales in Children." The study results showed that there were 314 pediatric patients with acute pain evaluated for minimum (MCSD) and ideal clinically significant differences (ICSD) using the Faces Pain Scale-Revised (FPS-R) and the Color Analog Scale (CAS). These data reveal stable MCSD estimates for the FPS-R and the CAS with generalizability for acute pediatric pain. The ICS

The total artificial heart.

In the early 1800s, an awareness of potential ventricular failure stimulated interest in artificial heart replacement. In 1937 the first total artificial heart (TAH) was implanted into the chest of a dog by Russian physicians. The primary driving force for mechanical cardiac assistance developed from the necessity for circulatory assistance in order to perform corrective cardiac surgery. In 1953 the first successful closure of an atrial septal defect using extracorporeal circulation was performed. During the following decade the concept of using mechanical devices to assist the failing heart w

Artificial intelligence agent for delirium screening among patients in oncology and cardiac intensive care units: a proof-of-concept study.

AIMS: This study aims to develop and initially test an autonomous artificial intelligence (AI) agent to support nurse-led delirium screening and guideline-concordant prevention and management. METHODS AND RESULTS: We constructed a delirium-specific knowledge graph from publicly available clinical guidelines and implemented an autonomous AI agent that integrates retrieval-augmented generation with validated delirium assessment tools to simulate decision support in nursing workflows. In this proof-of-concept evaluation, the agent was benchmarked on 20 clinical patient cases to assess (i) tool s

A post-market, cluster randomised controlled trial of the effect of the TENA SmartCare Change Indicator on continence care efficiency and skin health in nursing homes.

BACKGROUND: Urinary Incontinence (UI) is highly prevalent in older residents of nursing homes. For many, management consists of using appropriate products (pads) maintained by a check and change regimen; this is often poorly managed. Digital health technology devices, like urine saturation sensors, may aid ensure timely change of pads. This study examined the efficacy and safety of the use of the TENA SmartCare Change Indicator (Change Indicator) device for nursing home residents with UI. METHODS: Cluster randomised controlled trial of device use in older nursing home residents with UI, compa

Assessing the spreading potential of an undetected case of COVID-19 in orthopaedic surgery.

BACKGROUND: With the novel coronavirus-induced disease (COVID-19), there is the fear of nosocomial infections and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) transmissions to healthcare workers (HCW). We report the case of a 64-year-old male patient who underwent explantation of a shoulder prosthesis due to a periprosthetic infection. He was tested SARS-CoV-2 positive 7 days after admission to the orthopaedic department following strict infection control measures, routinely including screening all patients for multi-drug-resistant organism (MDRO) colonization upon admission. Posterior Circulation Stroke Misdiagnosed as Benign Paroxysmal Positional Vertigo: A Case Report.

Posterior circulation strokes are frequently underrecognized in emergency departments, especially when dizziness is the presenting complaint without overt motor deficits. Diagnostic delays often occur when providers anchor on benign causes such as benign paroxysmal positional vertigo. Early computed tomography and magnetic resonance imaging may miss acute infarcts in the posterior fossa, whereas the bedside head-impulse, nystagmus, and test of skew plus hearing examination has demonstrated greater sensitivity for detecting stroke in acute vestibular syndrome. A 76-year-old male with vascular r

Nursing Students' Perceptions of Small-Group Standardized Patient Simulations to Enhance Therapeutic Communication Skills.

OBJECTIVE: Therapeutic communication is essential to nursing practice, yet many pre-licensure nursing students report discomfort applying these skills in clinical settings. Standardized patient (SP) simulation offers a controlled environment for practicing communication, yet qualitative research describing students' perceptions of these experiences remains limited. This study explored undergraduate nursing students' perspectives on small-group, communication-focused SP simulations within a psychiatric-mental health nursing course. METHODS: As part of a larger program evaluation, six in-person

Orthostatic hypotension in older nursing home residents who fall: are they dizzy?

Orthostatic hypotension (OH) and dizziness have been linked to older adult falls. As a treatable condition, the detection of OH is of clinical importance to falls prevention. Gaps in the literature about symptoms associated with OH among older adult fallers, such as dizziness led to this secondary analysis of blood pressure data and reported symptoms. Of the 194 Registered Nurse (RN) documented falls, most (n = 91; 46.9%) failed to meet the prevailing diagnostic criteria for OH; 18 (9.2%) met criteria; and 8 (4%) nearly met the OH criteria. OH determinations were incomplete or absent in 41% (n
