Neonatal neurocritical care is an evolving subsubspecialty whose goal is to implement neuroprotective care strategies, continuous bedside monitoring of neurologic function, and therapies in order to reduce the risk of neurologic injury and improve long-term neurodevelopmental outcomes in neonates who require intensive care. The provision of neonatal neurocritical care requires a culture change across a Neonatal Intensive Care Unit (NICU) in which equal importance is placed on the neurologic care and the cardiorespiratory care of a given patient. It is a multidisciplinary framework of care in w

AIMS AND OBJECTIVES: This paper, therefore, aimed to review published literature in this area of pharmacological exploitation, to look at the therapeutic applications and clinical relevance and, by so doing, provide an accessible source for nurses to gain insight into the role of nitric oxide in the clinical setting.
BACKGROUND: Nitric oxide is a chemical mediator fundamental in the maintenance of adequate tissue perfusion and effective cardiovascular function; a major endogenous regulator of vascular tone. The use of nitrates are well established as pharmacological agents but it is only rece

BACKGROUND & PROBLEMS: During the stay of premature infants in the neonatal intensive care unit, their important tubes must be fixed to the skin with medical adhesive products to ensure they remain securely emplaced. However, premature infants have immature skin development and are particularly prone to medical adhesive-related skin injuries (MARSI), with related incidence rates reaching as high as 47.4% in our unit.
PURPOSE: The purpose of this project was to improve nurses' handling of medical adhesive products and the accuracy of their knowledge regarding MARSI in premature infants with th

OBJECTIVE: To initiate quantification of blood loss (QBL) for all cesarean births to align with professional obstetric organizations' recommendations and to reduce hemorrhage-related severe maternal morbidity (SMM).
DESIGN: Quality improvement (QI) project.
SETTING/LOCAL PROBLEM: Before December 2022, a large northern California hospital relied on estimating blood loss during childbirth, contrary to recommendations from national obstetric organizations. Estimating blood loss during childbirth has been consistently shown to be inaccurate, leading to recommendations for QBL by organizations su

Periventricular leukomalacia (PVL) is a result of injury and necrosis of myelinated fibers around the lateral ventricles. PVL is now considered the principal form of brain injury in preterm infants. This injury can have long-term effects on physical, motor, sensory, cognitive, and social development. Some proposed pharmacological treatments being considered to aid in prevention of this injury are raising concerns because they have failed to show evidence of efficacy or have potential for deleterious long-term effects. Current treatment is aimed at injury prevention; therefore, nurses play a cr

There are many things to consider when deciding upon the various instruments to use to monitor a newborn. The availability of these monitoring devices should be considered a boon both to the care of the sick newborn and to the nurse, for the data collection process is greatly enhanced in efficiency and accuracy. Nurses have assumed increasing responsibility for data collection, and with the aid of a variety of monitoring systems they will be better able to perform this task. They will also be left with more time for data synthesis and other aspects of patient care. The small amount of time tha

BACKGROUND: Family-centered care contributes to improved outcomes for preterm and ill infants. Little is known about the perceptions of neonatal intensive care unit (NICU) healthcare professionals regarding the degree to which their NICU practices or values family-centered care.
PURPOSE: The aims of this study were to describe attitudes and beliefs of NICU healthcare professionals about family-centered care and to explore professional characteristics that might influence those views.
METHODS: Data were derived from the baseline phase of a multicenter quasi-experimental study comparing usual

PURPOSE: This research evaluated the relationship between behaviors and energy expenditure in preterm infants receiving nursing interventions.
DESIGN AND METHODS: This study was an explorative secondary data analysis from a previous study. The current study investigated energy expenditure calculated using heart rate-based energy expenditure-estimate across 500 repeated measures for 37 infants.
RESULTS: Research results indicate that preterm infants expend more energy when they show the following seven behaviors: grimace, sucking, diffusion squirm, fist, gape face, salute, and sneezing.

Harlequin ichthyosis is an extremely rare and historically lethal congenital disorder of the skin caused by abnormal keratinization. This article reviews the embryology and currently understood pathophysiology of the disease, as well as current methods used to diagnose and treat these infants. There are serious implications for the family to consider: the high risk that their newborn will die soon, as well as future family planning issues.

From 1971 to 1980, 429 children with burn injuries were admitted to the Ahmadu Bello University Hospital, Zaria. These were major burns in 275 patients, moderate in 82 and minor in 72. Fourteen of the patients were neonates, 102 infants, 228 were 5 years old or younger and 85 were older. Socioeconomic factors contributing to the injuries included the use of firewood for cooking at ground-level and for warming the house and body during the cold season; loose indigenous garments; thatch-roofed huts and the post-partum rituals of mud-bed heating and hot baths. Flame burns exceeded scalds with a s

Evidence-based care of women in labor requires a thorough understanding of both "normal" and abnormal labor progress. In response to the growing cesarean delivery rate for dystocia at our institution, a multidisciplinary team of attending physicians, nurse-midwives, resident physicians, and nurses was established to review the literature and create evidence-based guidelines. This article describes the background literature and consensus guidelines reached for the diagnosis of active phase labor, active phase arrest, second-stage arrest, protraction of the active phase, and failed induction of

BACKGROUND: Inadequately treated, preeclampsia and eclampsia (PE/E) may rapidly lead to severe complications in both mothers and neonates, and are estimated to cause 60,000 global maternal deaths annually. Simulation-based training on obstetric and neonatal emergency management has demonstrated promising results in low- and middle-income countries. However, the impact of simulation training on use of evidence-based practices for PE/E diagnosis and management in low-resource settings remains unknown.
METHODS: This study was based on a statewide, high fidelity in-situ simulation training progra

AIMS: To assess what proportion of all cardiac abnormality can be suspected at birth when all clinical examination before discharge is undertaken by a small stable team of clinicians.
METHODS: A prospective audit of all the 14 572 births in a maternity unit only staffed by nurse practitioners between 1996 and 2003.
RESULTS: 1.2% of all babies born in the unit were found to have a structural defect (as confirmed by echocardiography) within a year of birth. The number not suspected before discharge declined over time, and only 6% were first suspected after discharge in the last four years of t

BACKGROUND: Globally, an estimated 275,000 maternal deaths, 2.7 million neonatal deaths, and 2.6 million third trimester stillbirths occurred in 2015. Major improvements could be achieved by providing effective care in low- and middle-income countries, where the majority of these deaths occur. Mentoring programs have become a popular modality to improve knowledge and skills among providers in low-resource settings. Thus, a detailed understanding of interrelated factors affecting care provision and mentorship is necessary both to improve the quality of care and to maximize the impact of mentori
