Congenital Ocular Motor Apraxia as the First Sign of Joubert Syndrome: A Case Report.

Congenital ocular motor apraxia (COMA) is characterized by an inability to initiate voluntary horizontal saccades, often leading patients to compensate with characteristic head thrusts. While COMA can sometimes present as an isolated ocular finding, it is frequently a manifestation of broader neurodevelopmental disorders. We report the case of a nine-month-old female infant presenting with developmental delay and abnormal eye movements. Examination revealed marked motor developmental delay, hypertonia, truncal arching, and poor head control. Notably, oculomotor apraxia requiring compensatory h

Unusual side effects of lamotrigine therapy.

An 8-year-old boy developed tremor, unsteadiness, chorea, and eye movement abnormalities on starting lamotrigine for myoclonic jerks. Investigations for a neurodegenerative disorder were negative. Symptoms and signs resolved on stopping lamotrigine. He was well and asymptomatic on follow-up after 4 years. Another 7-year-old boy who started on lamotrigine for suspected absence seizures developed abnormalities of eye movement with associated cognitive decline, which also resolved on discontinuing the medication. Eye movement abnormalities, involuntary movements, and behavioral changes appear to

Convergence Deficits in Myoclonus-Dystonia Point to Cerebellar Impairment.

BACKGROUND: Myoclonus-dystonia (M-D) is a monogenic movement disorder, with proposed cerebellar dysfunction. Vergence eye movement deficits, characteristics of degenerative cerebellar disease, have not been studied in M-D. Cerebellar transcranial alternating current stimulation (tACS) is considered a potential therapeutic approach. OBJECTIVES: To assess vergence and prosaccade performance as markers of cerebellar dysfunction in M-D and to evaluate the effects of cerebellar 50 Hz tACS on these eye movements. METHODS: Vergence and prosaccade performance were examined in 14 M-D patients carryin

Accommodative esotropia and Brown syndrome in a girl with recessive geleophysic dysplasia.

Geleophysic dysplasia and Weill-Marchesani syndrome are acromelic dysplasias characterized by short stature, brachydactyly, and joint contractures. Recessive Weill-Marchesani syndrome typically includes spherophakia, but the ocular phenotype of recessive geleophysic dysplasia is not well defined. We describe the ocular phenotype of a girl with genetically confirmed recessive geleophysic dysplasia (biallelic ADAMTSL2 mutations). Features included high corneal astigmatism, accommodative esotropia, unilateral Brown syndrome, and no evidence for zonular disease at 12 years of age.

Ocular tilt reaction with vertical eye movement palsy caused by localized unilateral midbrain lesion.

A 60-year-old man developed diplopia and experienced difficulty moving his eyes. Vertical movement of each eye, including vestibulo-ocular reflex and smooth pursuit, was extremely limited. Horizontal eye movements were normal. His head position was tilted toward his left. There was 10 prism diopters of exotropia and 10 prism diopters of right hypertropia. Fundus photographs revealed a clockwise torsion of both eyes. These signs indicate leftward ocular tilt reaction. Magnetic resonance imaging showed a small area of an increased signal intensity localized in the midbrain dorsomedial to the red

[Multicystic encephalopathy with frontal lobe-originated gelastic seizure, ipsilateral oculogyric crisis, and horizontal epileptic nystagmus: an autopsy case].

Attacks of gelastic (laughing) seizure are usually reported as complex partial seizures of temporal lobe epilepsy and seizures associated with hypothalamic hamartomas, but are rarely reported as complex partial seizures of frontal lobe origin. We recently encountered a 29-year-old woman who had gelastic seizure attacks from age 17. She had shown severe mental retardation with cerebral palsy at 7 months, and entered precocious puberty at age 7. Attacks of gelastic seizure with ipsilateral adversive seizures, ipsilateral oculogyric crisis, and horizontal epileptic nystagmus were observed until h

Strabismus Surgery in Patients With Ocular Neuromyotonia: Potential Unmasking of the Condition and Effective Management Tool.

BACKGROUND: Ocular neuromyotonia (ONM) is a rare motility disorder in which paroxysms of tonic extraocular muscle contraction from abnormal ocular motor nerve firing result in episodic diplopia and strabismus. Medical therapy with membrane-stabilizing agents has varied success. A surgical approach to treatment has not yet been described. We report the outcomes of strabismus surgery in patients with ONM. METHODS: We describe 3 patients with sixth nerve paresis and ONM of the affected lateral rectus muscle who underwent strabismus surgery. All patients had a history of radiation therapy for int

Familial cold-induced transient diplopia.

BACKGROUND: The major precipitants of intermittent diplopia secondary to a phoria breakdown include fatigue and illness. PURPOSE: To describe cold-induced vertical diplopia in a mother and daughter. METHODS: Retrospective report. RESULTS: Mother and daughter both developed vertical diplopia after exposure to cold ambient temperatures. These symptoms remained stable over many years. Ocular motility examination suggested that the diplopia resulted from breakdown of a small vertical phoria. CONCLUSIONS: Familial cold-induced vertical diplopia is a rare disorder of unclear etiology that need n

[Systematic deviations of saccadic eye movements in Wallenberg syndrome].

A patient suffering from lateral medullary infarction (Wallenberg's syndrome) is presented, exhibiting a striking bias of all saccadic eye movements toward the side of the lesion. Oculographic tracings demonstrate this oculomotor disorder. Other disturbances of ocular motility which resemble this one superficially are discussed. Interruption of fixation leads to a gliding movement veeringly to the side of the infarction. Since all saccades generated under different circumstances are affected, it is contemplated where the pathological signal is intruded into the prenuclear level, to adulterate
