Optic neuritis (ON) may be the initial manifestation of multiple sclerosis (MS). We report on a partially retrospective (1960-1984) and longitudinal study (1985-1995) of patients with MS that had earlier ON. The study group (ON + MS) comprised 173 confirmed cases of ON, the control group (MS) included 767 MS patients without history of initial ON. The time lapse from ON to MS was less than 5 years in 128 patients (73.99%), 6-10 years in 37 (21.39%) and more than 10 years in only 8 (4.62%). ON does not affect the course of MS, age at onset, duration of disease or life expectancy. Disability sta…

PURPOSE: To quantify automated visual field defects seen at entry in the Optic Neuritis Treatment Trial (ONTT) to determine whether particular areas of the field are preferentially affected and to determine the extent of visual field involvement in patients having "localized" field defects.
METHODS: Review of Humphrey 30-2 Visual Field (Allergan-Humphrey, Inc, San Leandro, CA) data from the involved and fellow eyes of 440 patients who were enrolled in the ONTT. Field defects were evaluated by comparing the involved eye to the fellow eye.
RESULTS: Patients with diffuse visual field defects ha…

The records of 31 male aviators seen at the Ophthalmology Branch of the USAF Aeromedical Consultation Service (formerly the USAF School of Aerospace Medicine) for a diagnosis of primary idiopathic optic neuritis (PION) were reviewed. Each subject received comprehensive ophthalmologic and neurologic examinations. The long-term follow-up data were collected through repeat examinations and by survey. Despite 39% of aviators being grounded due to complications of their PION or multiple sclerosis (MS), many aviators diagnosed with PION may be safely returned to flying duties. However, any aviator d…

PURPOSE: To report a case of acute viral disease accompanied by bilateral optic neuritis with substantial paraclinical evidence that human immunodeficiency virus was the causative agent.
METHODS: Clinical and paraclinical examination. Magnetic resonance imaging.
RESULTS: Virus and antibody titers as well as reverse lymphocytosis were consistent with acute infection by the human immunodeficiency virus-1.
CONCLUSIONS: Human immunodeficiency virus infection should be considered in the differential diagnosis of acute optic neuritis.

A 12-year-old PPD-negative girl developed bilateral optic neuritis with decreasing visual acuity 5 days after bacille Calmette-Guérin (BCG) vaccination. Laboratory findings and computerized tomography scan of brain and orbit were negative. Cerebrospinal fluid study revealed 62 leukocytes (92% lymphocytes) per cubic millimeter and a protein level of 33 mg/dl. The patient was treated with systemic steroids. The vision returned to normal in 4 weeks and the disc edema resolved in 6 weeks without residual deficit. The complications of BCG are discussed.

Detoxification of hydrogen peroxide by the antioxidant enzyme catalase suppressed the neurologic manifestations of acute experimental allerg … phagocytic mononuclear cells, may play a role in the pathogenesis of demyelination in experimental optic neuritis.

A study was done to systematically characterize visual function in eyes with recovered optic neuritis. Thirty-five eyes from 27 patients, all of whom had recovered at least 20/30 Snellen acuity after resolution of the neuritis, were included. Minimum recovery period was 6 months. Abnormalities were found in color vision (57%), contrast sensitivity (72%), perimetry (26%), stereoacuity (80%), light brightness (89%), pupillary reaction (89%), and optic disc appearance (77%). Eighty-five percent of patients complained of at least some subjective disturbance in vision. Subjective visual complaints …

Thirty five adults and two children with clinically isolated optic neuritis were examined by magnetic resonance imaging (MRI) to determine the presence of disseminated lesions within the brain at presentation and to compare these findings with the results of evoked potential studies. Of the adult patients, 61% showed lesions on the scans whereas the evoked potentials suggested the presence of lesions outside the visual system in 30%. MRI is a sensitive method for the demonstration of clinically unsuspected lesions in patients with uncomplicated optic neuritis.

A case of myasthenia gravis is described in association with optic neuritis in which brain-stem auditory and somatosensory evoked potentials were normal. CSF contained alkaline oligoclonal IgG bands. Blood lymphocyte subpopulations showed a decreased number of T-suppressor cells.

Horizontal saccadic and smooth pursuit eye movements were studied in 84 patients with multiple sclerosis (MS) and 21 patients with optic neuritis (ON). The MS patients were clinically classified as 'definite', 'probable', or 'possible'; subclinical eye movement disorder was found in 80 per cent of the definite, 74 per cent of the probable and 60 per cent of the possible category. Five of the ON patients (25 per cent) showed a subclinical eye movement deficit; these 5 were young patients with a recent history of ON. In a group of 27 MS patients with symptoms of spinal cord involvement only, 14 …

Visual functions and visual evoked potentials (VEP) were compared in 2 groups of patients with optic neuritis (ON). The subdivision was based on the occurrence of oligoclonal IgG in the cerebrospinal fluid in some patients [the ON (+) group] but not in others [the ON (-) group]. At the onset of ON there were no other signs of neurological disease in these patients. Later the majority of those in the ON (+) group had developed multiple sclerosis but only one patient in the ON (-) group. Recovery after the acute optic inflammation was the same in both groups with respect to visual acuity and fie…
