Paget's disease of bone in a North African population: clinical, imaging, and therapeutic insights from a Moroccan case series.

BACKGROUND: Paget's disease of bone (PDB) is a non-malignant skeletal disease that has become rare in recent decades. It is characterized by disorganized bone remodeling. Although its presentation varies worldwide, it is infrequently reported in Africans. To our knowledge, this is the first case series of PDB reported from Morocco, highlighting clinical and imaging features in a North African population. METHODS: A retrospective case series and a literature review. Seven patients were identified during an 8-year period and diagnosed with Paget's disease of bone. Demographic profiles, clinical

Paget disease of bone in Japanese patients: a report of three cases.

Paget disease of bone (PDB) is a bone metabolic disorder causing pain, fractures, and deformity. Its incidence is estimated to be 1 to 2% in Caucasians older than 55 years, but in Asian populations the incidence is rare. We report on 2 female and one male Japanese patients aged 46 to 73 years with PDB. One patient had monostotic disease with pain around the shoulder and the other 2 were asymptomatic. All patients had elevated alkaline phosphatase (ALP) levels (range, 629-957 U/L). Two patients responded to oral bisphosphonate treatment and achieved normalised ALP levels and pain relief. One pa

[Computed tomographic criteria for the diagnosis of variable manifestations of Paget's disease in the cerebral cranium and facial bones].

OBJECTIVE: to optimize the diagnosis of different stages of Paget's disease, by determining the extent of bone structural lesions in the cerebral and visceral cranium on the basis of computed tomography data. MATERIAL AND METHODS: Computed tomographic data were assessed by keeping in mind the structure, density, outlines, shadow shapes of the described tumor-like disease and the state of involved bone structures. Twelve patients with histologically verified Paget's disease were examined. RESULTS: The findings allowed the high informative value of computed tomography in diagnosing different s

Multifocal osteogenic sarcoma in Paget's disease.

The most serious complication of Paget's disease is sarcomatous degeneration of pagetic bone. Multifocal sarcomatous degeneration occurs mainly in polyostotic Paget's disease. Multifocal Paget's sarcoma is uncommon and can arise in any site. We report two cases of synchronous multifocal sarcomatous degeneration. The two patients were elderly women (aged 77 and 86 years, respectively) who developed sarcomatous lesions concomitantly, in the first case report in left ilium, left tibia, and first lumbar vertebra and in the second case report in the skull, right ilium, and sacrum. Whether these cas

Short-term therapy with oral olpadronate in active Paget's disease of bone.

One of the aims of the treatment of Paget's disease with bisphosphonates should be the normalization of the activity of the disease with the shortest possible exposure to the drug. Olpadronate (OPD) is a new bisphosphonate characterized by the dimethylation of the amino group, its potency is near to alendronate, and more soluble in the digestive media than other aminobisphosphonates. We treated 46 patients (28 men and 18 women, mean age 70 years) with active Paget's disease with oral OPD, 200 mg/day for 12 +/- 2 days, except 2 patients who received 400 mg/day. Eight patients had never been tre

[The clinical, imaging and electrophysiological correlations in Paget's disease].

The work presents two cases of Paget disease with a complex neurological description. The first case, a woman of 59 years, presents a spastic paraparesis with multiple syndrome of cranial nerves. The second case, a woman of 60 years, presents a spastic tetraparesis with myoclonus. The cranial radiography stood out to both patients the thickness of the bones of the skull in the context of the existence of some grown titres of the alkaline phosphate. The examination CT to the second patient indicates an important cerebral atrophy. Auditory evoked potentials (AEPs) show to the first patient the i

Osteosarcoma of the orbit associated with Paget disease.

Osteosarcoma is a common primary bone malignancy most often involving the long bones and occurring in the second decade of life. Orbital involvement in this disease process is rare and is usually due to extension of the tumor from an adjacent sinus cavity. Orbital osteosarcoma arising from preexisting Paget disease is exceedingly rare. We report the case of a 78-year-old female patient with Paget disease of the forehead who presented with rapidly increasing bilateral proptosis and visual loss for several months. Examination showed bilateral aphakia, decreased motility, proptosis with resistanc

[Bisphosphonates and bone remodeling: effectiveness in Paget's disease, fibrous dysplasia and osteoporosis].

Paget's disease of bone, bone fibrous dysplasia and osteoporosis are characterized by abnormal bone remodeling, and frequently imply intervention of orthopedists. Recent significant medical therapeutics improvements have been obtained with the use of bisphosphonates. Theses drugs decrease bone remodeling. In Paget's disease, bisphosphonates relieve bone pain, allow lytic areas refilling and decrease complications risk with normal bone formation. In fibrous dysplasia, bone pain is alleviated, osteolytic areas are refilled and cortices are widened. In postmenopausal osteoporosis, bisphosphonates

Quantification of vitamin D receptor mRNA in tissue sections demonstrates the relative limitations of in situ-reverse transcriptase-polymerase chain reaction.

In situ-reverse transcriptase-polymerase chain reaction (IS-RT-PCR) is a recently described technique that is used to localize low levels of mRNA within cells and tissue sections. One of the major criticisms levelled at this technique is that positive results may be meaningless, as amplification is required to demonstrate the transcripts of interest. The use of IS-RT-PCR to demonstrate mRNA for receptors for 1,25-dihydroxyvitamin D3 (VDR) in sections of human kidney and bone has previously been described. To ascertain whether the levels of VDR mRNA detected following IS-RT-PCR were transcripti

Tubular excretion of phosphate in Paget's disease of bone. Effect of pamidronate.

Two recent reports of phosphate diabetes in pagetic patients prompted us to evaluate urinary phosphate and its variations under pamidronate therapy in Paget's disease of bone. We also investigated whether Paget's disease is associated with phosphate diabetes. Urinary phosphate excretion was determined in 75 pagetic patients with a mean age of 72 years. None of the patients had received treatment during the six months preceding the evaluation. The 30 patients with clinical, laboratory test, and/or roentgenographic evidence of active Paget's disease were given intravenous pamidronate in a dosage

Serum kinetics, bioavailability and bone scanning of 99mTc-labelled sodium olpadronate in patients with different rates of bone turnover.

The activity of olpadronate labelled with technetium-99m(99mTc) was monitored in plasma and urine samples after single oral (925 MBq 99mTc/10 mg, coadministered with 50 mg cold drug) and intravenous (925 MBq 99mTc/5 mg) administrations to two groups of patients with different rates of bone turnover. The first group comprised high bone turnover (HBTO) patients suffering from Paget's bone disease; the second group comprised patients with normal to low bone turnover (NBTO) having the diagnosis of rheumatoid arthritis and secondary osteoporosis. Kinetic variables were correlated with anthropomorph
