Tympanic membrane/middle ear pathologic correlates in chronic otitis media.

OBJECTIVE: To correlate pathologic findings of the tympanic membrane with pathologic changes in the middle ear cleft in chronic otitis media … other diagnostic tools such as multifrequency tympanometry and otoacoustic emissions to complement otoscopy for diagnosis of middle ear pathology, especially in a tympanic membrane that appears "normal."

Assessment of Prevalence of Otitis Media with Effusion and Correlation Between Otoscopic Findings with Jerger's Diagnostic Tympanograms Among Pupils in Calabar Municipality.

BACKGROUND: OME is an accumulation of serous or mucoid fluid in the middle ear cavity with an intact tympanic membrane and without any sign … . Jerger type B tympanograms had a strong positive correlation with middle ear fluid (r = 0.777) on otoscopy, Jerger type C tympanograms with dull TM (r = 0.687), and retracted TM (r = 0.774). CONCLUSION: Otitis media with effusion was more prevalent among preschool children aged 1-2 years in public schools and among female children in Calabar municipalit …

The Development and Evaluation of a Smartphone-Aided Diagnosis Application to Measure Tympanic Membrane Perforations.

ObjectivesChronic otitis media is a long-term infection of the middle ear. It is characterized by persistent discharge from the middle ear through a perforated tympanic membrane. It is one of the most common causes of preventable hearing loss, especially in developing countries. Precise estimation of the size of tympanic membrane perforation is essential for successful clinical management. In this study, we developed a smartphone-based application to calculate the ratio of the area of tympanic membrane perforation to the area of the tympanic membrane. Twelve standardized patients and 60 medica

Principles of Pediatric Endoscopic Ear Surgery.

The article describes the unique benefits and challenges of transcanal (and transmastoid) endoscopic ear surgery (EES) for management of middle ear disease in children. It provides a rationale for EES in children and describes differences in anatomy between the pediatric and adult ear. The basic principles of EES, from operating room layout, choice of surgical instruments, and tips and pearls to avoid complications specific to the endoscope, are reviewed. Finally, techniques and outcomes in pediatric EES for tympanic membrane perforation, congenital cholesteatoma, and acquired cholesteatoma ar

Incidentally detected middle ear osteoma: two cases reports and literature review.

Osteomas of the middle ear are rare benign neoplasms. To date, only a few cases have been reported. Osteomas of the middle ear are small, single, usually unilateral, peduncular growths, off-white in color, with a smooth or multilobular surface, asymptomatic or causing functional disorders. The most common symptom is conductive hearing loss because of impingement of the ossicular chain. Some cases are asymptomatic and are diagnosed incidentally. We present two cases of incidentally detected middle ear osteoma. Based on a review of the main articles in the literature and analysis of two cases ma

Natural evolution of an eardrum bridge in patients with a traumatic eardrum perforation.

Although the "eardrum bridge" of traumatic tympanic membrane perforations (TMPs) is very little seen, the underlying natural evolution during the healing process are still unknown. The aim of this retrospective study was to evaluate the natural evolution of the "eardrum bridge" of TMPs. The data for 36 patients with barotrauma-associated traumatic TMPs with an "eardrum bridge" between January 2006 and December 2007 were retrieved. The eardrum bridge was completely liquefied due to infection in one patient. The bridge gradually became necrotic and incorporated into the new eardrum in four patien

Advantages of hyaluronic acid fat graft myringoplasty over fat graft myringoplasty.

OBJECTIVES: To compare hyaluronic acid fat graft myringoplasty (HAFGM) with fat graft myringoplasty (FGM) on grade I tympanic membrane perforations (TMPs) (<25% of the tympanic membrane surface) and to assess 12-month postoperative hearing outcomes in a pediatric population. DESIGN: Prospective study. SETTING: Tertiary care pediatric center. PATIENTS Ninety-two children with a TMP were included in the study and were operated on using either the HAFGM (n = 50) or FGM (n = 42) technique. Age at surgery ranged from 4 to 17 years (mean age, 11.52 years). INTERVENTIONS: Hyaluronic acid fat graft

[Persistent stapedial artery with otosclerosis].

OBJECTIVES: A persistent stapedial artery is a rare vascular malformation. This diagnosis is based on CT scan and intraoperative findings. MATERIAL AND METHODS: The case of a 19-year-old woman with a persistent stapedial artery found during stapes surgery is reported. This vascular malformation was explored with a CT scan showing the bilaterality of this anatomical variation and signs of associated otosclerosis. RESULTS: This malformation was successfully coagulated with laser allowing the stapedotomy to be completed. CONCLUSIONS: A persistent stapedial artery is not a contraindication to s

Objective assessment of the perineural vascular plexus as a landmark for the horizontal part of the facial nerve in middle ear and mastoid surgery.

OBJECTIVE: To objectively evaluate the usefulness and the reliability of the perineural vascular plexus as a landmark for identification of the facial nerve in surgery for chronic squamous otitis media. STUDY DESIGN: Prospective case series. SETTING: Tertiary otologic center. PATIENTS: Seventy consecutive patients requiring surgery for cholesteatomatous otitis media. INTERVENTION: Use of a semiquantitative grading system intraoperatively to assess the utility and ease of using the perineural facial plexus as the pointer to the facial nerve. MAIN OUTCOME MEASURES: Description of the perine

OBJECTIVE: To present the course of tympanic membrane pathology in childhood and young adulthood after otitis media (OM) in early life. STUDY DESIGN: Prospective follow-up study. SETTING: Community study of a birth cohort. PATIENTS: Three hundred fifty-eight subjects with a positive and negative history of OM (OM+ or OM-) or ventilation tube insertion (VT+ or VT-) derived from a birth cohort that had been followed-up from preschool to adult age. OBJECTIVE: To research the practicability of otoendoscopic tympanic segment and ganglion geniculi of facial nerve decompression via the attic approach and the advantage of the operative procedure. METHOD: 12 patients with traumatic peripheral facial paralysis following temporal bone fracture (n = 11) and radical mastoidectomy (n = 1) underwent otoendoscopic tympanic segment, pyramidal segment or ganglion geniculi of facial nerve decompression via the attic approach. The injured positions were estimated before operation by high resolution CT scan, Schirmer test, acoustic reflex, taste test and

The role of Adam Politzer (1835-1920) in the history of otology.

PURPOSE: To study and understand the contribution made by Adam Politzer (1835-1920) to 20th century otology and analyze his place in the history of otology. METHOD: A study of his four reference works, some of his hundreds of publications in medical journals, and diverse publications written about him. RESULTS: All areas of otology have been studied, compiled, and improved through his assorted publications, notably his atlas of otoscopy published in 1865, the first work of its kind, expanded and reedited in 1896; his 10 tables of the anatomy of the ear amended in 1873; his textbook of the di…

Audiological findings in Down's children.

The authors have investigated the auditory function in 35 Down's subjects, aged between 1 month and 16 years. Clinical examination revealed the occurrence of impaired nasal breathing in 18 subjects (51.4%), while otoscopy results were bilaterally normal in 12 cases (34.3%). Behavioural pure-tone audiometry yielded reliable results in 10 children (28.6%), impedance tests in 28 (80.0%), and brainstem audiometry in 29 (82.9%). A clinical and audiological follow-up has been performed in 11 cases (31.4%). Pure-tone audiometry, which may be employed in all cases only beyond 8 years of age, revealed …
