[Oxygen-dependent processes in the irradiated organism].

Interaction of oxygen with the X-rayed tissues, chemical substances and elements is one of the main mechanism of radioactive damage. Radiosensibilization by oxygen (oxygen effect) is most highly exhibited under the action of high and moderate radiation doses. However there is only scanty investigations and information about oxygen-dependent processes under low doses of internal and external radiation due to their intricate character. Radiation exerts also a great influence on oxygen metabolism and oxygen consumption level. Oxygen takes active part in post-radiation recovery both at the level o

Nursing care of the patient with an intravascular oxygenator.

Conventional therapy in the management of adult respiratory distress syndrome is often associated with an increased mortality rate. Several methods to improve survival in patients with severe respiratory insufficiency are under evaluation. One recently developed method of treatment is an implantable intravascular oxygenator, which provides supplemental gas exchange for failing lungs. This device can provide temporary ventilatory support in patients with acute, potentially reversible respiratory insufficiency. Reduction in ventilator settings such as airway pressure, oxygen concentration, posit

Intravascular oxygenation for advanced respiratory failure.

Severe acute respiratory failure of varying etiology may require the temporary use of artificial gas exchange devices. So far, extracorporeal membrane oxygenation and extracorporeal carbon dioxide removal have been used successfully for this purpose. A totally implantable intravascular oxygenator (IVOX) recently became available. The authors have used IVOX in three patients who presented with severe respiratory failure secondary to pneumonia (n = 2) and post-traumatic adult respiratory distress syndrome (n = 1). At the time of implantation, all patients had hypoxemia (PaO2 less than 60) despit

[Hyperbaric oxygenation in the treatment of necrotizing fasciitis].

The authors present the results of treatment provided to 11 patients with necrotizing fasciitis who were after a surgical operation and after administration of antibiotics treated in a hyperbaric chamber. A total of 8 patients (82%) recovered completely. The authors discuss the theory of action of hyperbaric oxygen on microorganisms and draw attention to the possible reduction of mortality of this serious disease when using hyperbaric oxygenation. The latter is considered an important auxiliary method which supplements surgical treatment, rational antibiotic therapy and in particular careful i

Uses of hyperbaric oxygen therapy in the 1990s.

Hyperbaric oxygen can produce a variety of effects in addition to reducing air and gas embolism. It increases the killing ability of leukocytes and is lethal to certain anaerobic bacteria. It inhibits toxin formation by certain anaerobes, increases the flexibility of red cells, reduces tissue edema, preserves intracellular adenosine triphosphate, maintains tissue oxygenation in the absence of hemoglobin. In addition, it stimulates fibroblast growth, increases collagen formation, promotes more rapid growth of capillaries, and terminates lipid peroxidation. These actions of hyperbaric oxygen are

Oxygenation in the stomach and the effect of truncal vagotomy.

A Clark polarographic oxygen electrode allowed detailed mapping of tissue oxygen (PtO2) levels on the anterior surface of the stomach in five patients undergoing cholecystectomy. No significant difference in mean PtO2 was detectable between greater and lesser curvatures. A significant difference in mean PtO2 was detected between the body of the stomach and the pylorus (Mann-Whitney, P less than 0.01). The effect of truncal vagotomy on PtO2 was evaluated in six patients undergoing this procedure for duodenal ulceration. Mean postvagotomy stomach PtO2 levels (46 +/- 12 mmHg) were significantly l

Fetal circulatory responses to oxygen lack.

The knowledge on fetal and neonatal circulatory physiology accumulated by basic scientists and clinicians over the years has contributed considerably to the recent decline of perinatal morbidity and mortality. This review will summarize the peculiarities of the fetal circulation, the distribution of organ blood flow during normoxemia, and that during oxygen lack caused by various experimental perturbations. Furthermore, the relation between oxygen delivery and tissue metabolism during oxygen lack as well as evidence to support a new concept will be presented along with the principal cardiovasc

Oxygen free radicals and human diseases.

Free radicals and other reactive oxygen species are constantly formed in the human body. Many of them serve useful physiological functions, but they can be toxic when generated in excess and this toxicity is often aggravated by the presence of ions of such transition metals as iron or copper. Excess generation of reactive oxygen species within tissues can damage DNA, lipids, proteins and carbohydrates. Which of these is the most important target of damage depends upon the cell type subjected to the oxidative stress and upon how it is imposed.

Oxygen consumption in sepsis and septic shock.

This review article examines the pathophysiology of septic shock, with special attention to the concept of supply-dependent consumption and the implications this concept has for therapy. Patients with septic shock require higher levels of oxygen delivery (DO2) to maintain aerobic metabolism. When DO2 is inadequate, peripheral tissues switch to anaerobic metabolism and oxygen consumption decreases. The lactic acidosis that occurs is a reasonable clinical marker of supply dependency and inadequate tissue perfusion. Maximizing DO2 is an important part of the hemodynamic resuscitation of patients

Perfluorochemical oxygen carriers and ischaemic tissues.

This paper has outlined the potential beneficial effects of emulsified PFCs for ischaemic tissue rescue. There are very strong indications that PFC emulsions will come to play an important part in the management of the ischaemic tissues. However, the immediate goal of future studies should be to assess the value of improved formulations in this context together with analysis of their mechanism(s) of action, both alone and in combination with other therapeutic agents.

Practical application of oxygen transport principles.

Management of shock is generally guided by hemodynamic data, but the true aim of therapy should be optimizing oxygen delivery (DO2) and consumption (VO2). Available data do not support the hypothesis that there is a single critical threshold of DO2 below which tissue hypoxia occurs; thus, DO2 and VO2 should be addressed for each patient. Interventions that raise BP, such as infused catecholamines, may actually decrease DO2, as can mechanical ventilation with PEEP. Therefore, the clinician should avoid responding solely to hemodynamic data and should direct interventions toward delivering the o

Extracorporeal membrane oxygenation for hydrocarbon aspiration.

Extracorporeal membrane oxygenation is a pulmonary bypass procedure that has been employed in adults to provide temporary treatment for reversible acute pulmonary and cardiac insufficiency. The technology of membrane oxygenation has been used since 1977 in neonates with predictably fatal pulmonary failure due to respiratory distress syndrome, persistent fetal circulation or persistent pulmonary hypertension of the newborn, meconium aspiration syndrome, and congenital diaphragmatic hernia. The use of extracorporeal membrane oxygenation in older children with other pulmonary disorders has been l

Long-term home oxygen therapy.

Oxygen therapy improves mortality and morbidity in hypoxemic patients with COPD. For best results, oxygen should be administered continuously. The general guidelines that qualify a patient for long-term home oxygen therapy are rather explicit, although there are some gray areas, such as increasing exercise performance in normoxemic COPD patients and the treatment of dyspnea in certain patients. The physiologic goals of oxygen therapy are to reverse or prevent tissue hypoxia under the various conditions of life. Patients often experience a fall in oxygen saturation during REM sleep and during e

Re-expansion, re-oxygenation, and rethinking.

In a 1902 American Journal of the Medical Sciences case report, Riesman described "albuminous expectoration" following thoracentesis, a phenomenon that is now recognized as re-expansion pulmonary edema (RPE). Both cellular and biochemical mechanisms that produce lung injury in RPE have been described recently. Pathophysiologically, this unilateral edematous lung injury resembles the adult respiratory distress syndrome (ARDS) because both are characterized by intra-alveolar-activated neutrophils and markedly increased lung capillary permeability. Biochemical mechanisms that operate in RPE are a

Peritoneal oxygenation of normoxic and hypoxic dogs.

The ability of an oxygen saturated fluorocarbon, perfluorodecalin (C10F18), to raise the arterial partial pressure of oxygen (PaO2) of anesthetized dogs, was tested by peritoneal lavage in normoxic and hypoxic animals. While breathing room air, five of six animals showed an increased PaO2 of 5-10 torr above reference values. In cases of mild hypoxia, increases of 5-25 torr were observed. No increase in PaO2 was seen when hypoxia was severe. A mathematical model was used to analyze the data and determine a transport coefficient (k). Calculated k values were 1.18 +/- 0.151 ml O2 STPD per minute

Extracorporeal membrane oxygenation in the newborn and beyond.

Extracorporeal membrane oxygenation (ECMO) has gained notoriety recently as a treatment of last resort for neonates at term or near term who suffer from pulmonary failure. Because of serious inherent risks, the procedure is at present reserved for infants with reversible pulmonary disease who have failed conventional or high-frequency ventilation and who meet the prognostic criterion of 80 percent mortality.

Oxygen saturation in the supine hypotensive syndrome.

This study used recently available, continuous noninvasive monitoring techniques to evaluate positional variations in pulse, blood pressure, and maternal oxygen saturation in 42 women undergoing fetal nonstress testing in the third trimester. Ten nonpregnant women were similarly evaluated with the automatic sphymomanometer and pulse oximeter. Six of 42 pregnant women (14.3%) developed the supine hypotensive syndrome (defined as a mean blood pressure decrease of 15 mmHg and a sustained increase in pulse of 20 beats per minute) when in the supine position. Nine of them (21.4%) met at least one o
