Morphofunctional and histological patterns of blood vessels in the superficial cervicofacial musculoaponeurotic system in midlateral face regions.

OBJECTIVE: The superficial cervicofacial musculoaponeurotic system (SMAS) is a complex network formed by mimic muscles and conjunctive tissue of the superficial fascia of the face. This study aimed to introduce new anatomofunctional data on the importance of the trans-SMAS distribution pattern of the skin microperfusion of the face and to underline the role of SMAS in maintaining the homeostasis of the vascular network that crosses it. Considering the fibrous and muscular matrix of the SMAS, using COLIII and MyoH2 antibodies, together with endothelial immunohistochemistry(IHC)intercellular adhe

[Malformations of the first branchial cleft: a case report].

First branchial cleft anomalies are extremely rare and account for less than 8% of all branchial anomalies. They are generally found in infancy although they arise during the embryonal period because of an incomplete closure of the first branchial cleft. In 1972 Work classified first branchial cleft anomalies into two types: Type I, ectodermal cysts, is a duplication of the external auditory duct; Type II, originate in both ectodermal and endodermal components and contain cartilage. The present case report describes a Type II branchial malformation. G.M., a 2-year-old male, presented painful t

Melanocytic schwannoma.

The clinical, histopathologic, immunohistochemical, and electron microscopic findings of two cases of melanocytic schwannoma are reported. One case, which occurred in the parotid gland, is believed to be the first reported in this location. The other case was located in the sacral spinal region. A review of the clinical and pathologic features of 32 previously reported cases is presented. The recurrence rate was 24%. Those tumors that were incompletely excised and/or located in the cranial nerves had the worst prognosis.

Carcinoma of the parotid gland: developing prognostic indices.

Prognosis for parotid carcinomas is not-well defined. The authors have developed models that could be useful to define subgroups of patients with differential risks. Clinical and pathological variables, and immunohistochemically studied MMP-7, MMP-9, MT1-MMP, and VEGF proteins were analysed in 42 patients with parotid gland cancer, regarding disease-specific survival and loco-regional recurrence. A prognostic index (PI) was calculated by combining age, disease stage, squamous cell carcinoma histology, and vascular endothelial growth factor immunoexpression. Based on the values of this PI, pati

Two-point Dixon fat-water separation: improving reliability and accuracy in phase correction algorithms.

PURPOSE: To propose an advanced phase-correction region-growing algorithm for two-point fat-water separation suitable for parotid assessment, and to evaluate the general performance of phase-correction algorithms. MATERIALS AND METHODS: Two region-growing algorithms were evaluated in test objects and in head images: the original phase-correction algorithm (OPC) and the advanced phase-correction algorithm with voxel size manipulation (VSM) which includes: 1) starting the region-growing process from images of lower resolution and subsequently stepping toward the original matrix size, and 2) lim

Primary osteoclast-like giant cell tumor of parotid gland: A rare extraskeletal presentation with diagnostic challenges.

Primary osteoclast-like giant cell tumor (OC-GCT) has been rarely described in extraskeletal sites. The diagnosis primarily hinges on the detection of giant cells. However, these giant cells are also seen in many giant cell lesions, thus creating diagnostic confusion and dilemma. Here, we describe a rare case of a 24-year-old male with primary extraskeletal, OC-GCT presenting as a swelling in the right parotid region and highlight its cytological, histological and immunohistochemical characteristics with diagnostic challenges.

The supraclavicular artery island and trapezius myocutaneous flaps in head and neck reconstruction.

The supraclavicular artery island flap can be readily used to reconstruct defects within the neck, parotid, lateral temporal region, and lower third of the face. Benefits of the supraclavicular flap include good color and texture match, an ease of harvest, and minimal donor site morbidity; there is also no significant post-operative monitoring required. The trapezius muscle serves as a source for multiple myocutaneous flaps of which most are considered to be salvage flaps among head and neck reconstructive surgeons.

A simple reconstructive procedure for radiation-induced necrosis of the external auditory canal.

Localized necrosis of the bone, cartilage, and soft tissue of the external auditory canal is an uncommon side effect of radiotherapy to the parotid region. Five patients developed late onset skin necrosis of a quadrant of the ear canal secondary to an underlying osteoradionecrosis of the tympanic ring. We report a one-stage procedure to excise the necrotic tissue and replace it with a local rotational flap derived from the post-auricular skin. Otological side effects of radiotherapy are discussed.

FDG-PET assessment of the effect of head and neck radiotherapy on parotid gland glucose metabolism.

PURPOSE: Functional imaging with [F-18]-fluorodeoxyglucose positron emission tomography (FDG-PET) provides the opportunity to define the physiology of the major salivary glands before and after radiation therapy. The goal of this retrospective study was to identify the radiation dose-response relationship of parotid gland glucose metabolism in patients with head and neck squamous cell carcinoma (HNSCC). MATERIALS AND METHODS: Forty-nine adults with HNSCC were identified who had curative intent intensity-modulated radiation therapy (IMRT) and FDG-PET imaging before and after treatment. Using a

Significance of incidental fluorodeoxyglucose uptake in the parotid glands and its impact on patient management.

BACKGROUND AND OBJECTIVES: Minimal-to-low grade fluorodeoxyglucose (FDG) uptake in the parotid glands is regarded as a normal variant in a whole-body survey with FDG-PET. Not frequently, however, a relatively intense or asymmetric FDG uptake is encountered in the parotid glands. The aim of this study was to determine the causes and characteristics of this 'FDG accumulation of uncertain significance' in the parotid glands in patients without any known or suspected pathologies at the time of whole-body FDG-PET. In addition, we also examined patients in whom there was no documented evidence of pa

Assessment of contour accuracy in head and neck replanning: Deep learning trained model compared with deformable image registration propagation technique.

Accurate contouring is crucial for optimal treatment outcomes, whether for nonadaptive radiotherapy with single images or adaptive radiotherapy (ART) with multiple images. For ART there are 2 common approaches for automated segmentation: deformable image registration (DIR) propagation of prior contours from a previous image to a newer replanning image or (ii) deep learning (DL) generated by models trained with datasets. The accuracy of the latter approach is impacted by the size, diversity and quality of the training dataset while the accuracy of the former approach depends on the quality of p

Embryonal mastoid rhabdomyosarcoma in a three years old child: A case report.

INTRODUCTION: Rhabdomyosarcoma is the most common malignant soft tissue tumor in the pediatric age, especially in the head and neck region; in the orbit and the nasopharynx. The middle ear is a very rare site for this neoplasm as it accounts for only 10 % of head neck rhabdomyosarcoma. PRESENTATION OF CASE: We report here the case of a three years-old child who was admitted to the emergency room for a left parotid and retro-auricular swelling with grade V facial palsy. The patient experienced chronic otorrhea with left facial palsy for two months. Two weeks later, a left parotid swelling appe

Cytological diagnosis of deep-seated cellular hemangioma of the parotid gland by using cell button technique.

Intraparotid hemangioma of the children is a rare neoplasm, posing diagnostic dilemma to the diagnosticians as well as treating clinicians. A 2-month-old male infant presented with a diffuse swelling in the parotid region since birth that was gradually increasing in size. The ultrasonography (USG) report was suggestive of a right intraparotid mass of uncertain etiology; whereas magnetic resonance imaging (MRI) report inclined toward a mass associated with chronic inflammatory pathology. Fine-needle aspiration cytology (FNAC) suggested two differentials - a vascular neoplasm of the parotid glan…

Sialendoscopic findings in patients with obstructive sialadenitis: long-term experience.

During a 9-year period from October 2002 to October 2011, 561 patients with swelling in the submandibular gland (n=474) and parotid gland (n=87) were successfully studied using sialendoscopy, and the features and pathological changes of the duct evaluated and recorded. Abnormalities were noted in 552/561 cases (98%), and normal features in 9 (2%). The pathological changes in 467 submandibular glands were identified both endoscopically and radiographically, and endoscopic findings showed 3 types: sialolith (n=423, 91%), mucus plug (n=15, 3%), and stenosis (n=29, 6%). In 12 cases foreign bodies

Sclerosing rhabdomyosarcoma of the parotid gland in an adult.

Primary rhabdomyosarcoma of salivary glands is an extremely rare neoplasm, mostly seen in children. A newly described subtype of rhabdomyosarcoma, sclerosing rhabdomyosarcoma, has not yet been reported in this location. We report on a parotid gland tumor characterized by infiltrative growth of primitive type of neoplastic cells showing strong and diffuse nuclear positivity for MyoD1 and myogenin and by prominent hyalinized/chondroid matrix with some myxoid foci. The tumor recurred several times, and in recurrent tumors, differentiation into strap myoid cells appeared. There were no distant met

Craniocervical necrotizing fasciitis secondary to parotid gland abscess.

Necrotizing fasciitis is a potentially fatal soft-tissue infection that predominantly affects the abdominal wall, perineum and extremities. It is an uncommon clinical entity in the head and neck region and an exhaustive review of the English language literature disclosed reports on approximately 160 cases. Dental pathology, post-traumatic or iatrogenic skin or mucosa injuries and parapharyngeal or peritonsillar infections were the most frequently described origins. We describe herein the first case of craniocervical necrotizing fasciitis (CCNF) due to a parotid gland abscess. The patient was s

The surgical management of advanced malignancies of the parotid gland.

Advanced malignancies of the parotid gland frequently invade the parapharyngeal space and the infratemporal fossa. The majority of these lesions have not been cured by surgery and/or radiotherapy, and palliation or cure can only be achieved by en bloc resection of this region. Stylohamular dissection is a systematic method for en bloc resection of the infratemporal fossa and lateral skull base. From January 1980 until December 1987, 18 patients with advanced parotid malignancies underwent stylohamular dissection. Pathologic examination revealed the following diagnosis: adenocarcinoma (7), squa

[Pathohistological findings in infarcted parotid glands (necrotizing sialometaplasia) (author's transl)].

In 1973 Abrams et al. described a nonneoplastic lesion named "necrotizing sialometaplasia". Since this date additional cases have been reported. The lesion might be misdiagnosed either mucoid tumor or squamous carcinoma. The cases reported in the following years were localised on the hard palate, lower lip, nose and in the major salivary glands. By the "Speicheldrüsenregister" of the Institute of Pathology of the University of Hamburg five cases of infarcted parotid glands were registered during the years of 1965--1977. All infarcts are observed in patients with tumors in the parotid region. N…
