Framework to Action: Developing and Implementing a Statewide Network to Expand Opioid Treatment Access Using SHIFT-Evidence in Texas.

BACKGROUND: In Texas, the expansion of access to effective treatments for opioid use disorder remains a significant public health challenge. This study aimed to demonstrate the application of the Successful Healthcare Improvement from Translating Evidence in Complex Systems (SHIFT-Evidence) framework in developing and implementing a statewide network for office-based opioid treatment (OBOT) to underserved populations. METHODS: We identified systemic gaps in access to medications for opioid use disorder (MOUD) across Texas, using a targeted needs assessment of barriers and facilitators to bupr

International Donation and Transplantation Legislative and Policy Forum: Methods and Purpose.

UNLABELLED: Organ and tissue donation and transplantation (OTDT) legislation and policies vary around the world, and this variability contributes to discrepancies in system performance. This article describes the purpose and methodology of an international forum that was organized to create consensus recommendations related to key legal and policy attributes of an ideal OTDT system. The intent is to create guidance for legislators, regulators, and other system stakeholders who aim to create or reform OTDT legislation and policy. METHODS: This Forum was initiated by Transplant Québec and cohos

Using One Health assessments to leverage endemic disease frameworks for emerging zoonotic disease threats in Libya.

Continued emergence, re-emergence and spread of zoonotic diseases demonstrates the imperative need for multisectoral communication and joint coordination of disease detection and response. While there are existing international frameworks underpinning One Health capacity building for pandemic prevention and response, often guidance does not account for challenges faced by countries undergoing long-term conflict and sociopolitical instability. The purpose of this research was to identify Libya's laboratory and surveillance networks and routes of inter- and multisectoral communication and coordi

A scoping review of paediatric latent tuberculosis infection care cascades: initial steps are lacking.

BACKGROUND AND OBJECTIVES: Identifying and treating children with latent tuberculosis infection (TB infection) is critical to prevent progression to TB disease and to eliminate TB globally. Diagnosis and treatment of TB infection requires completion of a sequence of steps, collectively termed the TB infection care cascade. There has been no systematic attempt to comprehensively summarise literature on the paediatric TB infection care cascade. METHODS: We performed a scoping review of the paediatric TB infection care cascade. We systematically searched PubMed, Cumulative Index to Nursing and A

Handoffs and transitions in critical care-understanding scalability: study protocol for a multicenter stepped wedge type 2 hybrid effectiveness-implementation trial.

BACKGROUND: The implementation of evidence-based practices in critical care faces specific challenges, including intense time pressure and patient acuity. These challenges result in evidence-to-practice gaps that diminish the impact of proven-effective interventions for patients requiring intensive care unit support. Research is needed to understand and address implementation determinants in critical care settings. METHODS: The Handoffs and Transitions in Critical Care-Understanding Scalability (HATRICC-US) study is a Type 2 hybrid effectiveness-implementation trial of standardized operating

Clinical investigation of the cystic duct variation based on the anatomy of the hepatic vasculature.

PURPOSE: Anatomical variation of the cystic duct (CD) is rare but can result in misunderstanding of the CD anatomy during laparoscopic cholecystectomy, potentially leading to bile duct injury. Therefore, the precise preoperative identification of CD variation is important. However, preoperative imaging analyses of the biliary system are not always possible or sufficient. We therefore investigated CD variations based on the anatomy of the hepatic vasculature. METHODS: This study enrolled 480 patients who underwent imaging before hepatobiliary pancreatic surgery. We assessed the variation of th

Identifying vulnerable older adult populations by contextualizing geriatric syndrome information in clinical notes of electronic health records.

OBJECTIVE: Geriatric syndromes such as functional disability and lack of social support are often not encoded in electronic health records (EHRs), thus obscuring the identification of vulnerable older adults in need of additional medical and social services. In this study, we automatically identify vulnerable older adult patients with geriatric syndrome based on clinical notes extracted from an EHR system, and demonstrate how contextual information can improve the process. MATERIALS AND METHODS: We propose a novel end-to-end neural architecture to identify sentences that contain geriatric syn

[Monoclonal gammapathy of renal significance (MGRS) at the current state: terminology, diagnosis and treatment].

In this article we discussed the current state of monoclonal gammapathy of renal significance (Monoclonal Gammopathy of Renal Significance MGRS) and revealed problems of B-cell clone secreting nephrotoxic monoclonal immunoglobulin identification. We followed 276 patients with monoclonal gammapathy including patients with non-amyloid nephropathy. The majority of patients had systemic AL-amyloidosis. We established better survival of the treated patients with systemic AL-amyloidosis in comparison with retrospective untreated cohort. We considered current treatment of patients with non-amyloid ne

The human exosome and disease.

Long before the RNA degrading exosome was first described in the yeast Saccharomyces cerevisiae, the use of autoantibodies found in the sera of certain autoimmune patients allowed the identification of a complex of polypeptides which later appeared to be the human exosome. Today, the most extensively documented association of the exosome with disease is still its targeting by the immune system of such patients. The highest frequency of autoantibodies to components of the exosome complex is found in polymyositis-scleroderma overlap patients and therefore the exosome is termed PM/Scl autoantigen

Comparison of intraoperative CT- and cone beam CT-based spinal navigation for the treatment of atlantoaxial instability.

BACKGROUND CONTEXT: Due to the complexity of neurovascular structures in the atlantoaxial region, spinal navigation for posterior C1-C2 instrumentation is nowadays a helpful tool to increase accuracy of surgery and safety of patients. Many available intraoperative navigation devices have proven their reliability in this part of the spine. Two main imaging techniques are used: intraoperative CT (iCT) and cone beam computed tomography (CBCT). PURPOSE: Comparison of iCT- and CBCT-based technologies for navigated posterior instrumentation in C1-C2 instability. STUDY DESIGN: Retrospective study.

Ophthalmic manifestations of rheumatologic disease: diagnosis and management.

Rheumatologic diseases are often associated with ophthalmic findings. Familiarity with and recognition of these ophthalmic presentations are important in the management of both the systemic and ocular manifestations of the disease. Identification of ocular signs and symptoms of rheumatologic disease will allow a more comprehensive approach to treatment of patients with rheumatic conditions.

[A Case of Bing-Neel Syndrome With Repeated Long Spinal Cord Lesions].

The patient was a 45-year-old man. Since 2019, he had exhibited repeated steroid-improved dysuria and long spinal cord lesions. At the time of recurrence in June 2020, he exhibited a marked increase in serum IgM levels, suggesting hematopoietic disease. We found an MYD88 L265P mutation in cerebrospinal fluid cells, which subsequently led to the diagnosis of Bing-Neel syndrome (BNS). The patient was treated with Burton's tyrosine kinase inhibitors and his condition progressed without dysuria or worsening of the imaging findings. This case was challenging to differentiate from intractable inflam

Neurological paraneoplastic syndromes.

Paraneoplastic neurological syndromes are uncommon, however, their diagnosis is of major practical importance. The identification of antibodies in the serum or cerebrospinal fluid in central nervous system paraneoplastic syndromes confirms the clinical diagnosis of a paraneoplastic syndrome and allows early identification of an underlying tumour at a stage when it is localised and more amenable to treatment. The failure to identify antibodies in patients with characteristic presentations of underlying neurological paraneoplastic syndromes does not exclude an underlying cancer. Necrotising myel

Identification of a mutation causing mucopolysaccharidosis type IIIA in New Zealand Huntaway dogs.

Mucopolysaccharidosis type IIIA (MPS IIIA) is an autosomal recessive disease that occurs due to a deficiency of heparan sulfate sulfamidase (SGSH). The deficiency of SGSH results in the lysosomal accumulation and urinary excretion of the glycosaminoglycan heparan sulfate. The clinical severity of MPS IIIA is predominantly characterized by severe central nervous system degeneration. Naturally occurring MPS IIIA has recently been described in New Zealand Huntaway dogs, with similar disease progression and biochemical characteristics observed in severely affected MPS IIIA patients. Here, we ident

ADHD Diagnosis and Timing of Medication Initiation Among Children Aged 3 to 5 Years.

IMPORTANCE: Early identification and treatment of attention-deficit/hyperactivity disorder (ADHD) symptoms in preschool-age children is important for mitigating social, emotional, and academic problems. Clinical practice guidelines recommend first-line behavior intervention before considering medication treatment for children aged 4 to 5 years. OBJECTIVE: To assess variation in rates of ADHD identification and rates and timing of medication initiation in children aged 3 to 5 years in primary care settings across 8 US pediatric health systems and to identify patient factors associated with the

New endobronchial ultrasound imaging for differentiating metastatic site within a mediastinal lymph node.

A newly developed endobronchial ultrasound processor system (EU-ME1; Olympus Ltd., Tokyo, Japan) improves the quality of ultrasound image by elevating cycles per minute. We report identification of mediastinal lymph node metastasis in a patient with small cell lung cancer. The new endobronchial ultrasound system images enabled visualization of metastatic site within the subcarinal lymph node by the combination of ultrasound image and Doppler mode.

Rapid identification of organisms from blood cultures.

A rapid method is described for the identification of organisms directly from blood culture broths, using antisera to group specific antigens and various commercially available serological and biochemical reagents. The technique identified clinically significant organisms within 1 to 3 h of a positive signal in the BACTECTM blood culture system. The method is of value in providing more rapid diagnoses and treatment of patients, and could be applied to other blood culture systems.

Development and validation of entrustable professional activities (EPAs) for rheumatology training in Brazil: the Reuma-EPA project.

BACKGROUND: Competency-based medical education increasingly guides postgraduate training worldwide. However, rheumatology training in Brazil still lacks a nationally standardized framework to define and assess core professional activities. Entrustable professional activities (EPAs) address this gap by translating competencies into observable clinical practice and supporting supervision, assessment, and progressive autonomy. OBJECTIVE: To develop and validate a national EPA framework for rheumatology training in Brazil and to present the conceptual and practical foundations supporting competen

The AngelMed Guardian® System in the Detection of Coronary Artery Occlusion: Current Perspectives.

Total ischemic time, which specifies the time from the onset of chest pain to initiation of reperfusion during percutaneous coronary intervention, consists of two intervals: symptom to door time and door to balloon time. A door to balloon time of 90 mins or less has become a quality-of-care metric in the management of ST elevation myocardial infarction (STEMI). While national efforts made by the American College of Cardiology (ACC) and American Heart Association (AHA) have curtailed in-hospital door to balloon time over the years, a reduction in pre-hospital symptoms to door time presents a ch
