Developing a group-based virtual education curriculum for cardiac rehabilitation and the associated toolkit to support implementation in Canada and across the globe.

BACKGROUND: To overcome the many barriers faced by the long-lasting pandemic, the development of new ways to deliver cardiac rehabilitation (CR)'s components was needed. OBJECTIVE: This mixed-methods study describes the process to create a virtual education curriculum and assess the level of user engagement and acceptability in CR participants. METHODS: A 4-phase approach was used to develop the new virtual education curriculum for CR and collect feedback regarding patients' engagement with and acceptability of the curriculum in a convenience sample of 80 CR participants. Data were analyzed

Sustaining Meaningful Patient Engagement Across the Lifecycle of Medicines: A Roadmap for Action.

BACKGROUND: There is increased recognition that incorporating patients' perspectives and insights into the medicines development process results in better health outcomes and benefits for all involved stakeholders. Despite the increased interest and the existence of frameworks and practical recommendations, patient engagement (PE) is not yet considered standard practice. The objective of this work was to provide a roadmap to support systematic change in all stakeholder organisations involved in medicines development across Europe, patients and patient organisations, medicines developers, acade

[The contribution of the fight against AIDS to the health democracy].

There is a debate on the place and the role of users in the healthcare system. It is important to remember that the people concerned by AIDS and associations fighting against this pathology have initiated a new type of patient involvement in care and public policies. This could be taken as a model.

Patient-assisted teledermatology practice: what is it? When, where, and how it is applied?

Recent teledermatology practice has been focused on different models made possible by robust advances in information technology leading to consistent interaction between the patient and health care professionals. Patient-assisted teledermatology practice also called patient-enabled teledermatology or home based teledermatology is one such novel model. There is a lack of scientific literature and substantive reviews on patient-assisted teledermatology practice. The present article reviews several studies and surveys on patient-assisted teledermatology practice and outlines its advantages and ba

[The concept of recovery: the importance of users' perspective and their participation].

The present paper develops the concept of recovery and its relationship with clinical aspects or symptomatic remission, as well as with other dimensions such as the experiential, physical, functional and social ones. Internal and external factors conditioning recovery are reviewed giving special emphasis to stigma, a powerful external factor that deeply affects recovery in mental illness. Users' perspective and participation in this process are considered to be essential and are compared to their place in traditional psychopharmological and psychotherapeutic treatments. As a conclusion, it is

Patient and public involvement and engagement in clinical trials at scale: Analysis of the first 3250 responses on the POrtal for Patient and Public Engagement in Dementia (POPPED).

INTRODUCTION: Patient and public involvement and engagement (PPIE) improves research quality but is often limited in scale. This study explored the potential for large-scale PPIE using a Web-based approach. METHODS: We created an online portal to gather public views on dementia research and a UK-based adaptive platform trial testing repurposed Alzheimer's disease drugs. Participants ranked four anonymized drugs and completed discrete choice experiments on treatment trade-offs. Analyses were stratified by sex, age, and dementia experience. RESULTS: Among 3250 people across 27 countries (87.4%

Advancing Equity in Breast Cancer Care Through Patient and Public Involvement and Engagement.

Disparities in breast cancer care access and outcomes are persistent and globally recognized issues driven by a complex interplay of socioeconomic, geographical, and systemic factors. Given the complex psychosocial effects on this patient group, it is crucial to incorporate Patient and Public Involvement and Engagement (PPIE) into care models. PPIE is a collaborative approach that involves patients, caregivers, and community members. It engages diverse populations and addresses the needs of underrepresented and disadvantaged groups, thereby promoting health equity. However, the multifaceted co

Enhancing the adoption of patient engagement in critical care research: Insights from a qualitative study.

BACKGROUND: In the rapidly evolving healthcare delivery, patient engagement in clinical research is emerging as a prerequisite for developing patient-centred care models, implementing value-based care, and creating metrics to enhance learning healthcare systems. In a high-acuity setting like critical care, this practice represents a paradigm shift from medical paternalism and the traditional disease-centred approach. It acknowledges the experiences and expertise of patients and families in the research cycle and clinical practice. However, patient engagement is a relatively new practice in cri

An Agenda to Support Care Partner Engagement through the Patient Portal.

Tools that help patients to digitally manage their health and health care have become increasingly important in recent years. The patient portal is the most well-established and widely used digital health tool through which patients may access their electronic medical record. Organizational efforts to promote awareness and use of the patient portal have primarily been directed at patients, yet millions of Americans with complex and costly health needs and disabilities manage their health with one or more care partners from among their family and friends. The presence and capacity of care partn

[Noncooperative, aggressive, and physically violent patients and their relatives].

The willingness of patients to cooperate is essential when it comes to attending appointments, diagnostics and the implementation of treatment measures. If a patient does not cooperate properly, this can have consequences under liability law, but also for the physician's remuneration. In addition, aggressions and verbal as well as physical violence on the part of patients and their relatives are increasing. This prevents the development of mutual trust or destroys it. Under certain conditions, physicians may then refuse or discontinue treatment. The operator of a medical facility can then also

Public and Patient Involvement in Research in the Fields of Inflammatory or Autoimmune Ocular Diseases: Protocol for a Scoping Review.

BACKGROUND: Patient and public involvement (PPI) is increasingly recognized as an important component of inclusive and patient-centered health research. However, the extent and nature of PPI in autoimmune and inflammatory ocular disease research remain unclear and inconsistently reported. OBJECTIVE: This scoping review aims to map how patients and the public have been involved in research relating to autoimmune and inflammatory ocular diseases across clinical, translational, and preclinical domains. METHODS: This scoping review follows the methodological framework proposed by Arksey and O'Ma

Integrating patient and public involvement into co-design of healthcare improvement: a case study in maternity care.

BACKGROUND: Despite recognition of the importance of patient and public involvement (PPI) in healthcare improvement, compelling examples of "what good looks like" for PPI in co-design of improvement efforts, how it might be done, and formalisation of methods and reporting are lacking. In this article, we sought to address these gaps through a case study to illustrate a principled approach to integrating PPI into the co-design of healthcare improvement. METHODS: The case study aimed to involve maternity service users in the co-design of clinical resources for a maternity improvement programme,

Negative effects of mindfulness-based cognitive therapy and cognitive behavioural analysis system of psychotherapy for patients with 'difficult to treat' depression: protocol for a systematic review and individual participant data meta-analysis.

INTRODUCTION: Mindfulness-based interventions are widely used, yet concerns about potential negative effects-particularly those related to mindfulness meditation practice-have gained increasing attention. Individuals with difficult-to-treat depression (DTD) represent a population of particular relevance due to heightened vulnerability, but comparative evidence on clinically relevant negative outcomes of mindfulness-based cognitive therapy (MBCT) versus established alternative psychotherapies in this group is lacking. This protocol describes a systematic review and individual participant data (

Low Body Mass Index Is Associated with a Long-Term Decline in Accelerometer-Measured Physical Activity in Patients with COPD: An Exploratory Analysis of Participants Co-Enrolled in Two Prospective Cohort Studies.

Background/Objectives: Reduced physical activity (PA) and increased sedentary behavior (SB) are associated with adverse outcomes in chronic obstructive pulmonary disease (COPD), but the factors underlying their long-term change are unclear; we investigated the baseline factors associated with these changes. Methods: Japanese outpatients with stable COPD co-enrolled in the SPACE (baseline) and E-PAC (follow-up) studies were analyzed retrospectively. Twenty-nine baseline clinical variables were assessed. PA and SB were measured with a waist-worn triaxial accelerometer and expressed in metabolic

Characteristics of eligible patients with knee osteoarthritis accepting versus declining participation in a randomised trial investigating the effect of weight loss versus knee arthroplasty to explore generalisability: A cross-sectional study.

BACKGROUND: The INtensive diet versus Knee Arthroplasty (INKA) trial is a randomised trial assessing weight loss as an alternative to knee arthroplasty (KA) in obese patients with severe knee osteoarthritis (OA) awaiting KA (NCT05172843). The external validity of the INKA trial may be hampered if the patients who participate differ from those who decline participation. OBJECTIVE: To compare baseline characteristics between patients who enrol in the INKA trial and those who decline participation (i.e., non-INKA [nINKA] group). METHODS: We applied a cross-sectional study design, collecting and

Are Hospital Admissions (Costs) and Mortality Rate Impacted by Guideline-driven Treatment of Heart Failure?: A Comparison between Participants in the "CorBene" CMP and Standard-care Patients on the Basis of Propensity Score Matching.

Heart failure (HF) is one of the most common diagnoses on admission to hospital in Germany, and one which incurs high costs. Integrated care in case management programs (CMPs) aims to improve treatment quality in the sense of guideline-driven treatment, while reducing hospital admissions, hospital costs, and mortality. A total of 1,844 patient data records from 11 German statutory health insurance companies enrolled in the CMP (intervention group [IG]) were compared with 1,844 standard-care patients (control group) using propensity score matching. The two groups were assessed over three follow

Sedentary Behaviour Intervention as a Personalised Secondary Prevention Strategy (SIT LESS) for patients with coronary artery disease participating in cardiac rehabilitation: rationale and design of the SIT LESS randomised clinical trial.

Patients with coronary artery disease (CAD) are more sedentary compared with the general population, but contemporary cardiac rehabilitation (CR) programmes do not specifically target sedentary behaviour (SB). We developed a 12-week, hybrid (centre-based+home-based) Sedentary behaviour IntervenTion as a personaLisEd Secondary prevention Strategy (SIT LESS). The SIT LESS programme is tailored to the needs of patients with CAD, using evidence-based behavioural change methods and an activity tracker connected to an online dashboard to enable self-monitoring and remote coaching. Management of adult patients with type 1 diabetes mellitus in Africa: A post-hoc cohort analysis of 12 African countries participating in the International Diabetes Management Practices Study (Wave 7).

There is a paucity of information on real world management of African adult patients with type 1 diabetes mellitus (T1DM). We aimed to describe a cohort of African adults with T1DM.The International Diabetes Management Practices Study is an observational survey conducted from 2005 to 2017. Data were collected in seven individual waves from countries in Asia, Africa, East Europe, and Latin America. Wave 7 was conducted from 2016 to 2017 and the African cohort included 12 countries. Questionnaires were administered to clinicians and patients. Analyses were mainly descriptive. Logistic regression

Comparison of a progestin-primed ovarian stimulation protocol with a flexible GnRH antagonist protocol in patients with polycystic ovary syndrome who are participating in an IVF programme: study protocol for a randomised controlled trial.

INTRODUCTION: Women with polycystic ovary syndrome (PCOS) undergoing in vitro fertilization (IVF) protocols are typically characterised by an increased number of oocytes retrieved. The oocytes are often of poor quality, leading to lower pregnancy rates, higher miscarriage rates and an increased risk of developing ovarian hyperstimulation syndrome (OHSS). Since our previous preliminary study showed that a novel progestin-primed ovarian stimulation (PPOS) protocol blocked the luteinising hormone (LH) surge during IVF and achieved a higher pregnancy rate with a lower incidence of OHSS, we designe
