UNLABELLED: Pre-existing inflammation, corticosteroid therapy, periapical periodontitis, longer duration of denosumab therapy, and female sex were significantly associated with an increased risk of denosumab-related osteonecrosis of the jaw after tooth extraction in patients with cancer on oncologic doses of denosumab. A short drug holiday did not protect against this complication.
INTRODUCTION: This study retrospectively investigated the relationship between various risk factors, including brief discontinuation of denosumab, and development of denosumab-related osteonecrosis of the jaw (DRON…

BACKGROUND: Apical periodontitis (AP) is a highly prevalent inflammatory condition that affects the tissue surrounding the apex of a tooth root. 7ND protein, a mutant form of monocyte chemoattractant protein-1 (MCP-1), functions as a dominant negative inhibitor of MCP-1. Previous studies have shown that 7ND protein can suppress osteoclast differentiation in peripheral blood mononuclear cells, suggesting its potential to prevent inflammatory bone destruction. However, whether 7ND protein can inhibit AP-induced osteolysis remains unknown.
METHODS: To investigate the effects of 7ND protein on os

Pulp canal obliteration (PCO), commonly following dental trauma, is characterized by progressive canal calcification and may be associated with pulp necrosis and apical periodontitis (AP). Conventional root canal treatment (RCT) may be technically unfeasible or associated with a high risk of iatrogenic complications. In the case reports, we explored endodontic microsurgery (EMS) as a primary treatment strategy for managing PCO with AP. Three patients (aged 30-46 years) presented with a history of dental trauma, severe canal calcification, and radiographic evidence of apical radiolucency, with

Actinomycosis is a rare lesion of the jaws and may present as periapical pathosis; therefore, it is essential to be correctly diagnosed and managed. This case presentation describes management of a tooth with a symptomatic apical periodontitis caused by Actinomyces species supplemented with medicine prescription. A woman was referred for endodontic management of tooth #19. The tooth had a history of previous nonsurgical endodontic retreatment. Clinically, the tooth was very sensitive to percussion. Radiographic evaluation showed a large periapical lesion. Intentional replantation (IR) was plan

AIM: We report a novel treatment option for teeth with internal root resorption (IRR) in which the lesion had perforated to the PDL and was located in the coronal aspect of the root. Arrest and calcification of IRR can be achieved by local application of calcium hydroxide without further intracanal instrumentation.
CASE REPORT: Two cases of severe IRR without periapical inflammation were treated with a novel technique: The vital pulp including the granuloma was left in place and subjected to long-term disinfection with application of calcium hydroxide in the coronal aspect of the IRR. In both

Irreversible pulpitis can develop into pulp necrosis, causing apical periodontitis (AP). The progression of apical periodontitis is always accompanied by periodontal disease. Many similar inflammatory response pathways are involved in both apical periodontitis (AP) and periodontal disease. The infiltration of specific immune cells and the activation of immune responses are thought to play important roles in the transition from irreversible pulpitis to AP and periodontal-related diseases. This study explored immune response characteristics during the transformation of pulpitis to periodontal di

AIMS: To investigate whether acute dental pain due to pulpal or periapical inflammation is associated with increased expression of cortisol and inflammatory markers and mediators in the saliva, as well as changes in salivary flow rate.
METHODS: Patients experiencing pain (n = 42) were recruited when seeking emergency dental treatment. A 0 to 10 numeric rating scale (NRS) was used as a measure of the severity of pain, and the number of days with pain sensation was also recorded. Unstimulated saliva was collected for 3 minutes (salivary flow measured in mL/minute) and stored at -80°C. Saliva wa

OBJECTIVES: Rheumatoid arthritis (RA) is a debilitating disease where numerous pro-inflammatory cytokines have a proven role in its pathology. These cytokines are also involved in the pathogenesis of apical periodontitis (AP) where they have a pro-inflammatory role and induce bone resorption. Patients with RA may therefore be more prone to develop pulpal-periapical pathology (PPP). This study systematically reviewed the existing literature evaluating the association between RA and PPP.
MATERIALS AND METHODS: Studies including human participants with both RA and PPP were included. The search w

RATIONALE: There is an increasing tendency for case reports to reveal anatomical aberrances in mandibular first molars, such as the lateral and accessory canals. Thus, clinicians should pay special attention to anatomic variances when dealing with mandibular first molars requiring endodontic treatment to prevent reinfection within the root canal system, which is responsible for the failure of root canal treatment.
PATIENT CONCERNS: This article presents the clinical report and successful root canal treatment of a 24-year-old healthy female patient with an extensive periapical lesion in a 6-ca

OBJECTIVES: Mechano-sensitive odontoblast cells, which sense mechanical loading and various stresses in the tooth structure, synthesize early signaling molecules such as prostaglandin E2 (PGE2) and nitric oxide (NO) as an adaptive response. It is thought that these synthesized molecules can be used for the diagnosis and treatment of periodontal and periapical diseases. The aim of this study was to investigate the relationship between the severity of apical periodontitis (AP) and chronic periodontitis (CP) and serum (s) TNF-α, IL-10, PGE2 and NO levels, as well as PGE2 and NO levels in gingival

This case report describes a 45-year-old male who presented with mobility and dull pain in the lower right back tooth region for two months. Initially diagnosed with localized periodontitis at a private clinic, the patient sought a second opinion. Clinical examination revealed slight obliteration of the buccal vestibule and mobility in teeth 45 and 46. Radiographic findings showed a unilocular radiolucency in the periapical region of the premolar and first molar. This case highlights the importance of including granular cell ameloblastoma in the differential diagnosis of mandibular lesions and

This case report is to present a maxillary first molar with one O-shaped root, which is an extended C-shaped canal system. Patient with chronic apical periodontitis in maxillary left first molar underwent replantation because of difficulty in negotiating all canals. Periapical radiographs and cone-beam computed tomography (CBCT) were taken. All roots were connected and fused to one root, and all canals seemed to be connected to form an O-shape. The apical 3 mm of the root were resected and retrograde filled with resin-modified glass ionomer. Intentional replantation as an alternative treatment
